OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Inpatient Coverage and Medical Necessity

Medical necessity and level of care

Inpatient care is covered when medical necessity is established under accepted level-of-care criteria.

Covered inpatient care: The inpatient services are medically necessary and meet accepted standards of InterQual Level of Care Criteria.

Criteria address acute, intermediate, critical, and extended-stay levels for medical, surgical, behavioral health, and pediatric conditions, including neonatal intensive care levels 3 and 4, special care level 2, newborn nursery level 1, and transitional care.

    Labor and delivery authorization pathway

Prior authorization is not required for qualifying labor and delivery inpatient care when the applicable conditions are met.

    Qualifying labor and delivery pathway

    • Spontaneous labor or induction pathway: The admission is related to spontaneous labor or induction of labor and results in delivery.

The inpatient admission length of stay must remain within the applicable federal mandate limit: 2 days for vaginal delivery or 4 days for uncomplicated cesarean delivery.

    • CHIP Perinate labor and delivery pathway: The member is a CHIP Perinate member and the inpatient admission is for labor and delivery.

    Other inpatient admissions

Inpatient admissions for conditions other than labor and delivery are reviewed against applicable benefit limitations.

Non-labor and delivery admission: The inpatient admission is for a condition other than labor and delivery.

The request is reviewed against applicable benefit limitations.

    Observation-level services

Observation-level hospital services generally do not require authorization unless a specific procedure or service has an authorization requirement.

Observation services: The service is provided at the observation level rather than as an inpatient admission.

Authorization may still be required for the specific procedure or service, such as bariatric surgery.

Preauthorization does not guarantee coverage. Services remain subject to applicable benefit limitations and exclusions and are not covered solely because preauthorization was approved.

Inpatient admissions that do not meet the referenced InterQual or applicable inpatient guidelines are referred to a TCHP Medical Director or Physician Reviewer for review, and the Denial Policy is followed.

    Authorization, Notification, and Documentation Requirements

    Prior Authorization

    Submit Inpatient Admission Authorization Requests

Submit all prior authorization requests for inpatient admissions to the Utilization Management Department through online submission, fax, phone, or mail. TCHP processes requests during normal business hours.

    Prior Authorization

    Notify Labor and Delivery Admissions

Notify TCHP of all labor and delivery-related admissions. Certain labor and delivery admissions may not require prior authorization when applicable federal length-of-stay limits and other guideline conditions are met.

    Note

Authorization Is Not a Benefit Guarantee

Preauthorization is based on medical necessity only and does not guarantee member eligibility, benefit coverage, or payment. Approval applies only to the medical necessity of the treatment or service.

    Inpatient Level-of-Care Framework

TCHP evaluates medically necessary inpatient care against accepted InterQual Level of Care Criteria. The criteria address acute, intermediate, critical, and extended-stay care for medical, surgical, and behavioral health conditions. Pediatric criteria also address neonatal intensive care levels 3 and 4, special care level 2, newborn nursery level 1, and transitional care.
