When Change Reviews Are Required and Procedures
Formal Change Review criteria
A Formal Change Review is required when any of the specified IFSP elements are proposed to change. The Service Coordinator must complete the listed actions for every Formal Change Review.
ALL of the following

- Payor source
- IFSP outcome
- Adding a new service
- Removing an existing service
- Proposed changes in the frequency, intensity, duration, method, or location of an existing service.
ALL of the following
- Complete prior written notice.
- Notify IFSP team members of the time, location, and reason for the meeting. IFSP team members may participate by phone or face to face.
- Provide the family with a copy of the IFSP.
Administrative Change Review criteria
Administrative Change Reviews are permitted only for limited, administrative edits to the IFSP as listed below. The family must be provided a copy of the updated IFSP after the change.

- Change in Service Coordinator and/or service coordination agency.
- Change in Early Intervention Service (EIS) Provider and/or agency.
- Change 'no provider available' to identified EIS provider.
- Provide the family with a copy of the updated IFSP.
Provider Responsibilities and Required Steps
Prior authorization: email BabyNet with BRIDGES ID to request EIBI authorization
E-mail BabyNet@scdhhs.gov with the child’s BRIDGES ID number and state that the Service Coordinator is seeking prior authorization from the IDEA/Part C State Office for initiation of Early Intensive Behavior Intervention (EIBI) services. Ensure BRIDGES contains the information required by the Procedures for Early Intervention Services in Natural Environments before submission, as IDEA/Part C State Office will consider authorization only after those fields are completed.
- Include the child’s BRIDGES ID number in the message.
- Confirm required BRIDGES fields (per the Procedures for Early Intervention Services in Natural Environments) are completed before requesting authorization.
