inv-01: Initial Diagnostic Injection

Covered when ALL of the following are met

Initial Diagnostic Facet Joint Injection/Medial Branch Block: Pain is exacerbated by facet loading maneuvers on physical examination (e.g., hyperextension, rotation); AND clinically significant improvement has not occurred (pain >= 3/10) after a minimum of 4 weeks of conservative care (including but not limited to pharmacotherapy, exercise, or physical therapy); AND clinical findings and imaging studies suggest no other cause of the pain (e.g., spinal stenosis with neurogenic claudication, disc herniation with radicular pain, infection, tumor, fracture, pain related to prior surgery); AND the spinal motion segment is not fused; AND a radiofrequency joint denervation/ablation procedure is being considered.see node text

inv-02: Confirmatory Diagnostic Injection

Covered when ALL of the following are met

Second Facet Joint Injection/Medial Branch Block to confirm initial response: Administered at the same level and side as the initial block; AND the initial diagnostic Facet Joint Injection produced a positive response for at least the expected minimum duration of the effect of the local anesthetic; AND functional improvement specific to the individual's previously limited physical functions is documented; AND a radiofrequency joint denervation/ablation procedure is being considered.see node text

inv-03: Unproven / Not Medically Necessary

inv-07: Therapeutic Interventions — Evidence Summary

Evidence/coverage considerations:

Therapeutic medial branch blocks and intra-articular injections: Therapeutic medial branch blocks are performed similarly to diagnostic blocks but current evidence does not support their use as therapeutic interventions; guideline reviews (NASS, WFNS, ASIPP, ASRA) note insufficient or low-quality evidence for many therapeutic facet procedures and intra-articular steroid injections, and selected studies show no clinically meaningful improvement at 6 months when based on a single diagnostic injection with 50% relief.

Consider conservative and palliative alternatives and document risks/benefits when contemplating therapeutic procedures.

Therapeutic facet joint and medial branch block injections at the cervical, thoracic, and lumbar levels are described in this policy as unproven and not medically necessary due to insufficient evidence of efficacy and safety. The policy specifically lists therapeutic use when a radiofrequency ablation is not being considered, use for treating spinal pain after diagnostic injections are completed, and repeat same-level/same-side injections (i.e., more than two) as unproven.
