Coverage Criteria for Non-wheelchair Adaptive Seating

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Initial medical necessity

Covered when ALL of the following are met

Initial coverage criteria for non-wheelchair adaptive seating: The member has postural instability or weakness requiring external supports to maintain proper alignment of head, trunk, or lower extremities or to maintain safe seated positioning; the member requires specialized positioning to safely perform essential, age-appropriate activities of daily living within the home (including feeding, bathing, toileting) or during vehicle transport; the adaptive seating system is prescribed by a physician; a specialized seating/mobility evaluation has been performed by a therapist or a professional independent from the vendor supplying the equipment; the member had a successful trial of the requested device; adaptive seating accessories and positioning components contribute to the therapeutic function of the adaptive seating system (examples include postural lateral supports, chest or pelvic straps, wedge, headrest or pommel, cushions, and trays); and required documentation has been provided.

    Tilt/recline medical necessity

Tilt/recline feature covered when member meets adaptive seating criteria above AND any ONE of the following

Tilt/recline necessity: Member's condition (for example, extensive weakness, contractures, or abnormal tone) requires full body support; OR member has a medical need that requires the tilted or reclined position when upright; OR member requires pressure relief when sitting (for prevention or treatment of pressure sores).

    Replacement

Replacement coverage when ALL of the following are met

Replacement criteria: Member meets all relevant medical necessity criteria for adaptive seating; AND member has outgrown the device OR the device no longer meets the member's needs OR the device is no longer functional through normal wear and tear (expected to last at least five years), and replacement is not due to misuse or abuse.

    Repairs

Repairs considered medically necessary when ALL of the following are met

Repair criteria: Either Select Health of South Carolina originally authorized and purchased the equipment and accessories, OR the durable medical equipment supplier provides clinical documentation establishing prior medical necessity for equipment previously authorized by another health plan; AND repairs are not the result of misuse or abuse; AND the cost of repair is less than replacement.

Adaptive seating is not medically necessary when the member has a functioning wheelchair that already provides adequate postural support and positioning components, because the adaptive seating would be duplicative durable medical equipment.

Adaptive seating is considered investigational / not clinically proven and therefore not medically necessary when it is duplicative of an adequate wheelchair; when the equipment is used solely for personal care, comfort, or convenience for the member or caregiver (including requests for more than one of the same item); or when accessories and positioning components do not contribute to the therapeutic function of the adaptive seating system.

Provider Actions, Prior Authorization, and Documentation

    Prior Authorization

Prior authorization required with physician prescription and independent seating evaluation

Prior authorization is required for adaptive seating systems; the device must be prescribed by a physician and supported by a specialized seating/mobility evaluation performed by a therapist or professional independent from the vendor. The request must also document a successful trial and meet the policy's documentation requirements.

    • Physician prescription is required.
    • Specialized seating/mobility evaluation by a therapist or professional independent from the vendor must be provided.
    • Successful trial of the requested device must be documented.
    • All required documentation elements (see documentation block) must be included with the prior authorization request.
    Step Therapy

Documented successful trial required before coverage

A successful trial of the requested adaptive seating device must be documented prior to coverage; the trial should be described in the evaluating therapist's PT/OT evaluation as evidence the member can use and benefit from the equipment.

    • Therapy evaluation must detail the trial of the device and demonstrate benefit.
    • Trial documentation is a required element for prior authorization and coverage.
    Documentation Required

Required documentation: equipment details, clinical evaluation, trial, education, and growth accommodation

Include complete equipment details and clinical documentation with requests: item name, detailed description, manufacturer and product number, and a description of equipment features (especially when billing HCPCS E1399); plus the member's specific functional limitations and underlying condition, a PT/OT evaluation with rationale and trial, parent/caregiver education, and the device's ability to accommodate growth.

    • Item name, description, manufacturer, and product number.
    • Description of equipment features not available with standard equipment (required for HCPCS E1399).
    • Description of the member's specific functional limitations and underlying medical condition.
    • Physical/occupational therapy evaluation detailing ability to use and benefit, rationale for product selection, and trial of the device.
    • Documentation of parent/caregiver education on device use.
    • Evidence the device can support growth in height and weight over time.
    Denial Risk

Denial triggers — duplicative equipment, comfort/convenience, or non-therapeutic accessories

Requests will be denied as not medically necessary when the adaptive seating duplicates a functioning wheelchair that provides adequate postural support, when the equipment is requested solely for personal care/comfort/convenience or as duplicate items, or when accessories/positioning components do not contribute to therapeutic function.

    • Denial if member has a functioning wheelchair providing adequate postural support and positioning components (duplicate DME).
    • Denial for equipment requested only for personal care, comfort, or convenience, including requests for more than one of the same item.
    • Denial if accessories and positioning components do not contribute to the therapeutic function of the adaptive seating system.

    Background and Scope

Non-wheelchair adaptive seating (therapeutic chairs) provides external postural support to assist with activities of daily living for children and other individuals with special health care needs. Evaluations for adaptive seating are typically multidisciplinary (for example, physical or occupational therapists and physicians) and should consider factors such as trunk and head control, balance, limb function, orthopedic anomalies, recent clinical changes, and growth. Adaptive seating may be customized or non-customized and includes devices used for transport, bathing/toileting, feeding, and other daily activities.

    Definitions

    Non‑wheelchair adaptive seating — definition
DefinitionAdaptive, portable, multi-positional seating that provides postural support to assist with activities of daily living; may be customized or non‑customized.
Typical usesProvides external trunk/head support to enable safe seated positioning for feeding, toileting, bathing, therapeutic activity, and participation in age‑appropriate ADLs.
Device examplesIncludes therapeutic chairs, adaptive strollers (for limited use in younger children), and seating systems used for transport or home activities.
CustomizationMay be provided as customized or non‑customized equipment depending on clinical need and growth considerations.
    Complex rehabilitation technology — definition
DefinitionSpecialized equipment individually configured for mobility disabilities to help perform activities of daily living safely, independently, and within a reasonable amount of time.
ScopeEncompasses advanced wheelchairs, adaptive seating systems, and other mobility devices that are individually configured to meet unique physical and functional needs.
PurposeSupports complex positioning, mobility, and independent function when standard equipment cannot meet the member's needs.

    Medical Necessity Criteria (DME)

    DME medical necessity

DME medical necessity: Device must provide external postural support to maintain alignment or safe seating for essential, age-appropriate ADLs; be prescribed by a physician; have an independent specialized seating/mobility evaluation; have a documented successful trial; accessories and positioning components must contribute to therapeutic function; and required documentation (item name, description, manufacturer/product number, feature description for E1399, member functional limitations and underlying condition, PT/OT evaluation with rationale and trial, caregiver education, and growth accommodation) must be provided.

    Rental and Purchase Rules

    ItemRental vs Purchase Rule
    Adaptive seating / postural support
Not specified in this policy. The document describes repair and replacement rules but does not explicitly state whether adaptive seating/postural support items are to be rented or purchased.

    Replacement Policy

Adaptive seating/postural support — replacement interval
Expected lifetimeAdaptive seating/postural support devices are expected to last at least five years under normal use.
Replacement reasonsReplacement may be medically necessary if the member has outgrown the device, the device no longer meets the member's needs, or the device is nonfunctional through normal wear and tear (not due to misuse/abuse).
Repair vs replacementRepairs are considered when repair cost is less than replacement and the issue is not the result of misuse or abuse; replacement allowed when device cannot be restored to function.
