    Authorization and Documentation Actions

    Prior Authorization

Confirm age and covered indication before requesting authorization

Coverage is provided when the patient is at least 18 years of age and decitabine is requested for a covered condition: MDS, AML, adult classic Hodgkin lymphoma, BPDCN, accelerated/blast-phase MPN, or MDS/MPN overlap neoplasms.

    • For MDS/MPN overlap neoplasms, covered uses include CMML-1 or CMML-2, MDS/MPN with neutrophilia, MDS/MPN NOS, or MDS/MPN with SF3B1 mutation and thrombocytosis.
    Prior Authorization

Meet all continued-treatment requirements for renewal

Authorization may be renewed when the patient continues to meet the relevant indication-specific criteria, has no unacceptable toxicity from decitabine, and has documented disease stability or improvement based on at least one specified hematologic or marrow measure.

    • Renewal criteria include applicable requirements such as concomitant therapy requirements, performance status, and other relevant criteria identified for the indication.
    • Document the absence of unacceptable toxicity, including serious myelosuppression such as anemia, neutropenia, or thrombocytopenia.

    Clinical Background

Intravenous decitabine is an antineoplastic therapy used under this policy for adults with selected hematologic malignancies, including myelodysplastic syndromes (MDS), acute myeloid leukemia (AML), adult classic Hodgkin lymphoma (cHL), blastic plasmacytoid dendritic cell neoplasm (BPDCN), accelerated/blast-phase myeloproliferative neoplasms (MPN-AP/BP), and myelodysplastic/myeloproliferative overlap neoplasms (MDS/MPN). Covered MDS/MPN uses include CMML-1 or CMML-2, MDS/MPN with neutrophilia, MDS/MPN not otherwise specified, and MDS/MPN with SF3B1 mutation and thrombocytosis.

Treatment is administered intravenously using disease-specific regimens and cycle schedules. Continued coverage requires that the patient remain eligible for the applicable indication, have no unacceptable drug toxicity, and show documented disease stability or improvement, such as a decrease in bone marrow blasts or an increase in platelets, hemoglobin, or WBC/ANC compared with pretreatment values.

    Documentation Required

    Submit complete clinical authorization documentation

Include the patient’s age, diagnosis and disease subtype, applicable indication-specific treatment requirements, toxicity status, and objective evidence of disease stability or improvement with the authorization request or renewal.

    • Identify the applicable covered indication and, when relevant, the MDS/MPN overlap subtype.
    • For renewal, document continued eligibility, absence of unacceptable toxicity, and objective disease stability or improvement.
    Documentation Required

Document objective response or stability for renewal

For renewal, document at least one objective response or stability measure compared with pretreatment values.

    • Decrease in bone marrow blast percentage
    • Increase in platelets
    • Increase in hemoglobin
    • Increase in WBC/ANC
    Denial Risk

Age or indication outside criteria may result in denial

Coverage may be denied when the patient is younger than 18 or when the requested use is not for one of the covered conditions listed in the policy.

    • Covered conditions are MDS, AML, adult classic Hodgkin lymphoma, BPDCN, accelerated/blast-phase MPN, and MDS/MPN overlap neoplasms.
    • MDS/MPN overlap coverage includes CMML-1 or CMML-2, MDS/MPN with neutrophilia, MDS/MPN NOS, and MDS/MPN with SF3B1 mutation and thrombocytosis.
    Denial Risk

Stay within the disease-specific dosing limit

Requests exceeding the applicable disease-specific maximum may be denied. Submit dosing within the policy’s maximum billable units for the patient’s indication.

    • Adult classic Hodgkin lymphoma: 250 billable units (250 mg) per 21 days
    • MDS: 900 billable units (900 mg) per 84 days
    • AML: 500 billable units (500 mg) per 28 days
    • MDS/MPN, BPDCN, and MPN-AP/BP: 250 billable units (250 mg) per 28 days
    Denial Risk

Address toxicity and response requirements before renewal

Renewal may be denied when the patient has unacceptable toxicity, lacks adequate documentation of disease stability or improvement, or no longer meets the relevant indication-specific criteria.

    • Unacceptable toxicity includes serious myelosuppression such as anemia, neutropenia, or thrombocytopenia.
    • Document at least one qualifying improvement or stability measure: decreased bone marrow blasts, increased platelets, increased hemoglobin, or increased WBC/ANC over pretreatment values.
    Blastic plasmacytoid dendritic cell neoplasm.
