All listed requirements must be satisfied.

    Initial Therapy - Crohn's Disease

Covered for Crohn's disease when ALL of the following are met, including at least ONE treatment-pathway option.

    Crohn's disease criteria

    • Qualifying treatment pathway

      • Conventional immunosuppressive therapy failure: Documented failure, contraindication, or ineffective response at maximum tolerated doses to a trial of corticosteroids or immunomodulators, such as azathioprine, 6-mercaptopurine, or methotrexate.Maximum tolerated dose
      • TNF modifier failure: Documented failure, contraindication, or ineffective response at maximum tolerated doses to a minimum 3-month trial of a TNF modifier, such as adalimumab, certolizumab, or infliximab.Minimum 3 months
      • High-risk disease: Member has evidence of high-risk disease for which corticosteroids or immunomodulators are inadequate and biologic therapy is necessary.High-risk disease
      • Established therapy:

    Initial Therapy - Ulcerative Colitis

Covered for ulcerative colitis when ALL of the following are met, including at least ONE treatment-pathway option.

    Ulcerative colitis criteria

    • Qualifying treatment pathway

      • Conventional therapy failure: Documented failure or ineffective response to a minimum 3-month trial of conventional therapy, including aminosalicylates, corticosteroids, or immunomodulators such as azathioprine, 6-mercaptopurine, or methotrexate, at maximum tolerated doses, unless contraindication or intolerance exists.Minimum 3 months; maximum tolerated doses

Three months of corticosteroids are not required if early non-response is confirmed.

      • TNF modifier failure: Documented failure, contraindication, or ineffective response at maximum tolerated doses to a minimum 3-month trial of a TNF modifier, such as adalimumab, golimumab, or infliximab.Minimum 3 months

    Renewal Criteria

Renewal is not available under this policy.

Authorization duration: The duration of authorization has not been exceeded; however, the initial authorization is limited to 11 weeks and prior authorization validity may not be renewed.Initial authorization limited to 11 weeks

The initial authorization covers the induction course; renewal is not available under this policy.

Therapy must not be initiated when the member has an active infection, including a clinically important localized infection. The provider must confirm that no active infection is present before intravenous Skyrizi induction begins.

Coverage requires that the member is not receiving concurrent treatment with another biologic therapy or targeted synthetic therapy when intravenous Skyrizi induction is initiated.

The provider must confirm that live vaccines will not be administered during therapy. Live vaccination during intravenous Skyrizi induction does not meet the policy’s initial approval requirements.

    Policy Background

Skyrizi (risankizumab-rzaa) is authorized under this policy as intravenous induction therapy for adults with moderate to severe active Crohn’s disease or ulcerative colitis. Induction is administered at Weeks 0, 4, and 8, followed by subcutaneous maintenance therapy. Before treatment begins, the policy requires objective baseline disease assessment and safety checks addressing infection, vaccination status, tuberculosis, laboratory results, and concurrent biologic or targeted synthetic therapy.
