OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Note

Infusion center or home: start IV; may switch to SC after 8 weeks

Initiate therapy with IV dosing; after eight weeks of once‑weekly IV administration, patients may be switched to once‑weekly subcutaneous dosing at the same weekly dose.

    • Recommended starting IV dose: 0.1 mg/kg IV once weekly (per dosing section).
    • After 8 weeks of once‑weekly IV, switch to once‑weekly SC at the same dosage if appropriate.

Approved indicationIndication targeted by this policy: hyperargininemia due to arginase 1 deficiency in patients age ≥2 years (Loargys)
    AbbreviationCommon abbreviation: ARG1‑D

    Background

Arginase 1 deficiency (ARG1‑D) is an inherited metabolic disorder characterized by deficient arginase‑1 activity leading to hyperargininemia. Loargys (pegzilarginase‑nbln) is a recombinant human arginase 1 enzyme replacement therapy approved for treatment of ARG1‑D in patients aged ≥2 years under accelerated approval based on reduction in plasma arginine. Continued approval may require confirmatory trial evidence; clinicians should follow the policy's documentation and monitoring requirements when requesting authorization.
