OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • The medication is prescribed by or in consultation with a hepatologist or gastroenterologist.
    • The member is at least 18 years old.Age ≥ 18 years
    • Cholestyramine has failed at up to maximally indicated doses, unless it is contraindicated or causes clinically significant adverse effects.

For Illinois HIM requests, this step-therapy requirement does not apply under IL HB 5395.

    • Lynavoy is not prescribed concurrently with another ileal bile acid transporter (IBAT) inhibitor, such as Livmarli or Bylvay.
    • The dose does not exceed 80 mg (2 tablets) per day.Maximum 80 mg (2 tablets) per day

      • Refer according to formulary or PDL status

        • For a drug on the commercial or health insurance marketplace/ICHRA formulary, or on the Medicaid PDL, refer to the applicable no-coverage criteria policy: CP.CPA.190 for commercial, HIM.PA.33 for health insurance marketplace/ICHRA, or CP.PMN.255 for Medicaid.
        • For a drug not on the commercial or health insurance marketplace/ICHRA formulary, or not on the Medicaid PDL, refer to the applicable non-formulary policy: HIM.PA.103 for health insurance marketplace/ICHRA or CP.PMN.16 for Medicaid.

Avoid use in patients with decompensated cirrhosis or prior or active hepatic decompensation events, including variceal hemorrhage, ascites, or hepatic encephalopathy.

Lynavoy must not be prescribed concurrently with another ileal bile acid transporter (IBAT) inhibitor, such as Livmarli or Bylvay.

Non-FDA-approved indications not addressed in this policy are not authorized unless sufficient efficacy and safety documentation supports coverage under the applicable off-label use policy or evidence of coverage documents.

    Note

Other Indications Require Applicable Policy Review

Requests for other diagnoses or indications must be reviewed under the applicable policy rather than under the PBC-associated cholestatic-pruritus criteria.
