    OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage criteria for Functional Electrical Stimulation (FES) for ambulation

    Initial coverage criteria for FES for walking

    Functional electrical stimulation (FES) may be considered medically necessary in members with spinal cord injury to enhance their ability to walk when ALL of the following are met:

    Primary coverage criteria I

    • A: Member had intact lower motor units (L1 and below), both muscle and peripheral nerve;
    • B: Member has muscle and joint stability for weight bearing at upper and lower extremities that can demonstrate balance and control to maintain an upright support posture independently;
    • C: Member demonstrates brisk muscle contraction to FES and has sensory perception to electrical stimulation that is sufficient for muscle contraction;
    • D: Member possesses high motivation, commitment, and cognitive ability to use such devices for walking;
    • E: Member can transfer independently and can stand independently for at least 3 minutes;
    • F: Member can demonstrate hand and finger function to manipulate controls;
    • G: Member is at least 6 months post recovery from spinal cord injury and restorative surgery;
    • H: Member does not have hip or knee degenerative disease and has no history of long bone fracture secondary to osteoporosis;
    • I: Member is willing to use the device long-term.

    Replacement devices

    Replacement of a FES for walking

    Replacement may be considered medically necessary if the original FES met criteria I, is no longer under warranty, and cannot be repaired.

    Explicit contraindications for use of functional electrical stimulation (FES) for ambulation include the presence of a cardiac pacemaker; severe scoliosis or severe osteoporosis; skin disease or cancer at the area of stimulation; irreversible contracture; autonomic dysreflexia; and certain neurological disorders (e.g., multiple sclerosis).

    Functional electrical stimulation (FES) is not medically necessary when the required coverage criteria (I.A–I.I) are not fully met or when any of the listed contraindications are present. Requests failing to document intact lower motor units, sufficient muscle/joint stability and balance, brisk muscle contraction and adequate sensory perception to stimulation, required functional abilities (transfers, standing ≥3 minutes, hand/finger control), appropriate timing post-injury (≥6 months), absence of hip/knee degenerative disease or osteoporotic fracture history, and long-term willingness to use the device should be considered non-covered.

    Authorization, documentation, and denial triggers for providers

    Prior Authorization

    Prior authorization recommended/required for devices and unlisted codes

    Prior authorization is recommended for unlisted codes and may be required for FES devices. When requesting authorization for E0764, A4595, or an unlisted code, include complete medical records demonstrating that the member meets all coverage criteria (I.A–I.I) and absence of listed contraindications.

    • Include all pertinent medical records and chart notes.
    • Document how the member meets each criterion I.A–I.I from the coverage criteria.
    • If submitting an unlisted code for a potentially covered service, prior authorization is recommended to avoid post-service denial.
    Note

    Verify coverage criteria and replacement conditions before ordering

    Confirm the member meets every element of the primary coverage criteria (I.A–I.I) before ordering or billing a FES device and verify warranty status for replacements; requests that do not meet criteria may be denied.

    Background on Functional Electrical Stimulation (FES)

    Functional electrical stimulation (FES) uses timed electrical impulses delivered via surface electrodes to activate paralyzed or weak muscles in coordinated sequence. In properly selected spinal cord injury patients, FES targets intact lower motor units and groups muscles to produce stepping movements that can enable ambulation following training.

    Typical candidacy requires preserved peripheral motor units (L1 and below), adequate muscle and joint stability for weight bearing, brisk contractile response to stimulation with sufficient sensory perception, and functional abilities such as independent transfers, standing for at least 3 minutes, and hand/finger control to operate the device. FES devices are intended as long‑term assistive systems and may be considered when these clinical and functional criteria are met after recovery and rehabilitation.

    Definitions

    Functional Electrical Stimulation (FES) definition
    DefinitionA technique that uses electrical impulses transmitted via surface electrodes to activate paralyzed or weak muscles in precise sequence to enable functions such as walking.
    MechanismTransmits impulses in same manner as neuromuscular electrical stimulation (NMES), enabling selective and sequential stimulation of lower extremity muscle groups.
    Primary indicationUsed to enable ambulation in selected spinal cord injured (SCI) patients after training program completion, when coverage criteria are met.

    • Ensure member is at least 6 months post recovery from spinal cord injury and restorative surgery (I.G).
    • For replacements, verify the original device met criteria I and is no longer under warranty and cannot be repaired.
    Documentation Required

    Required medical records, history, exam, and treatment plan

    Provide complete medical records at the time of the request, including history, physical examination, and a treatment plan that documents how the member meets each required coverage criterion and the absence of contraindications.

    • History: relevant neurologic and functional history supporting intact lower motor units and timing since injury.
    • Physical examination: documentation of brisk muscle contraction to FES, sensory perception to stimulation, transfer and standing ability (≥3 minutes), hand/finger function, and absence of hip/knee degenerative disease or osteoporosis-related fractures.
    • Treatment plan: intended long-term use, training program completion if applicable, and plans for device manipulation and ambulation.
    Denial Risk

    Denial risk: failure to meet criteria or presence of contraindications

    Requests will be considered not medically necessary — and may be denied — when the coverage criteria I.A–I.I are not fully met or when any listed contraindications are present; unlisted codes submitted for non-covered services will be denied.

    • Contraindications that trigger noncoverage include cardiac pacemakers, severe scoliosis or severe osteoporosis, skin disease or cancer at the stimulation site, irreversible contracture, autonomic dysreflexia, and certain neurological disorders (e.g., multiple sclerosis).
    • Submission of an unlisted code for a non-covered service addressed in this policy will be denied as not covered.
