      • There is evidence of viral replication and liver inflammation.
    • Chronic hepatitis B virus infection — pediatric individuals

      • Individual is aged 3 years or older and is non-cirrhotic.
      • Individual has HBeAg-positive chronic hepatitis B (CHB).
      • There is evidence of viral replication and elevations in serum alanine aminotransferase (ALT).
    • Treatment of chronic hepatitis C virus infection when used in accordance with current guidelines.
    • Chronic hepatitis D virus infection

      • Individual has compensated liver disease.
      • Pegasys (peginterferon alfa-2a) is used as a single agent for 48 weeks.
    • First- or second-line treatment of an individual with essential thrombocytopenia.
    • Myelofibrosis

      • Individual is aged 18 years or older.
      • Individual has myelofibrosis with a JAK2 V617F mutation.

    Unlisted Indications

Uses of Pegasys (peginterferon alfa-2a) for conditions not outlined in this policy are considered investigational and are not medically necessary under this policy.

Any indication or use of Pegasys other than the covered indications and applicable criteria listed in this policy is considered investigational.

COVERAGE CRITERIA: All other uses of Pegasys (peginterferon alfa-2a) for conditions not outlined in this policy are considered investigational.

    Authorization and Documentation Requirements

    Prior Authorization

Document medical necessity for initial authorization

Coverage requires documentation that the individual meets the applicable, condition-specific medical necessity criteria for Pegasys. Initial authorization may be approved for up to 12 months.

    Prior Authorization

    Support reauthorization with continued response

Reauthorization may be granted for up to 12 months when the medical necessity criteria continue to be met and the individual has a positive clinical response to Pegasys therapy, supported by submitted medical records.

    Documentation Required

    Submit required clinical records

    Peginterferon Alfa-2a Overview

Peginterferon alfa-2a is an alpha interferon that helps regulate immune responses and may inhibit viral replication and abnormal cell growth. The policy addresses its use in chronic hepatitis B, chronic hepatitis C, chronic hepatitis D, essential thrombocytopenia, and myelofibrosis with a JAK2 V617F mutation. Pegasys is managed through the pharmacy benefit.

    Clinical Definitions

    Hepatitis B surface antigen (HBsAg)
LocationFound on the surface of the hepatitis B virus.
InfectiousnessAll persons with HBsAg are infectious.
Clinical implicationDetection of HBsAg indicates the person is infectious.
    Hepatitis B e-antigen (HBeAg)
LocationFound in the inner core of the hepatitis B virus.
InfectiousnessPersons with positive HBeAg are described as most infectious.
Clinical implicationA positive HBeAg result indicates a high level of infectiousness.

    Denial Risk

    Avoid unlisted indications

Use of Pegasys for a condition or indication not outlined in the policy is considered investigational and is not covered as medically necessary under this policy.

    Denial Risk

    Demonstrate positive response for renewal

Reauthorization requires that the medical necessity criteria remain met and that the individual continues to demonstrate a positive clinical response to therapy. Without evidence of continued positive response, reauthorization requirements are not met.

    HBV DNA
Molecular definitionA partially double-stranded DNA molecule.
LocationFound in the inner core of the hepatitis B virus.
Infectivity relationshipElevated HBV DNA is associated with higher infectivity.
    Chronic hepatitis B (CHB)
Diagnostic durationHBsAg positive for at least 6 months.
Viral and biochemical findingsHBV DNA may range from undetectable to several billion IU/mL, and ALT and/or AST levels may be normal or elevated.
Liver findingsLiver biopsy results show chronic hepatitis with variable necroinflammation and/or fibrosis.
    Immune-active CHB
HBeAg-positive thresholdHBV DNA levels greater than 20,000 IU/mL in HBeAg-positive CHB.
HBeAg-negative thresholdHBV DNA levels greater than 2,000 IU/mL in HBeAg-negative CHB.
Disease activityALT and/or AST levels are intermittently or persistently elevated, with moderate or severe hepatic necroinflammation and with or without fibrosis.
    HBeAg seroconversion
    HBeAg statusLoss of HBeAg.
    Anti-HBe statusDetection of anti-HBe.
Prior statusOccurs in a person who was previously HBeAg positive and anti-HBe negative.
    Early virologic response (EVR)
    Response measureClearance of HCV RNA.
Assessment periodDuring the first 12 weeks of therapy.
Partial response distinctionA partial response is a minimum 2-log decrease in hepatitis C viral load after 12 weeks of therapy.
    Sustained virologic response (SVR)
Response measureClearance of HCV RNA by qualitative analysis.
Assessment timingTwenty-four weeks after therapy cessation.
DefinitionAn SVR is clearance of HCV RNA 24 weeks after therapy cessation.
    Sustained virologic response (SVR)
DefinitionClearance of HCV RNA by qualitative analysis 24 weeks after therapy cessation.
Response benchmarkThe current gold-standard measure of response to chronic HCV pharmacotherapy.
Clinical significanceAchievement of an SVR has been associated with resolution of liver injury, reduction in hepatic fibrosis, a low likelihood of relapse, and survival.
    Early virologic response (EVR)
    Viral markerHCV RNA.
    Response definitionClearance of HCV RNA.
Assessment periodDuring the first 12 weeks of therapy.
    Partial response
Response measureDecrease in hepatitis C viral load.
    Minimum decreaseAt least 2 log.
Assessment timingAfter 12 weeks of therapy.
    Relapse
Initial responseInitial achievement of a sustained virologic response.
Loss of responseThe sustained virologic response is not maintained.
Time courseThe response is not sustained over time.
    Nonresponse
Response statusNever achieving a sustained virologic response.
SVR statusNo SVR is achieved at any point.
Distinction from relapseUnlike relapse, nonresponse does not involve an initial achievement of SVR.
