    Rhinoplasty Coverage Criteria

    Site of Service

The site-of-service medical-necessity criteria in this policy do not apply to Indian Health Services (IHS) facilities; for IHS facilities, apply the procedure medical-necessity criteria only. For applicable facilities, elective rhinoplasty is covered when performed at the most appropriate, safe, and cost-effective site of service.

    ALL of the following

    • The elective rhinoplasty is performed at an ambulatory surgical center, an off-campus outpatient hospital or medical center, or an on-campus outpatient hospital or medical center.
    • A hospital outpatient department is medically necessary only when one of the following exceptions applies

      • There is no qualifying ambulatory surgical center within 30 miles that can provide the necessary care because

        • No geographically accessible ambulatory surgical center has the necessary equipment to perform the procedure.
        • No geographically accessible ambulatory surgical center is available at which the individual's physician has privileges.
        • An ambulatory surgical center's specific guideline prohibits use of the center because of the individual's health condition or weight.
      • The individual is aged 18 years or younger.
      • The service is performed in conjunction with an additional service that requires use of a hospital outpatient department, and the procedures are performed in the same operative session.
      • The individual has a clinical condition that increases the risk for complications, including

        • Anesthesia risk

          • ASA classification III or higher.
          • Personal history of an anesthesia complication.
          • Documented alcohol dependence or history of cocaine use.
          • Prolonged surgery greater than 3 hours.
        • Cardiovascular risk

          • Uncompensated chronic heart failure, NYHA class III or IV.

    Rhinoplasty for Deformity

Rhinoplasty may be considered medically necessary when the following criterion is met:

    ALL of the following

    • The procedure corrects a nasal deformity secondary to cleft lip or cleft palate or another congenital craniofacial deformity.

    Rhinoplasty for Nasal Obstruction

Rhinoplasty may be considered medically necessary for nasal obstruction when all of the following criteria are met:

    ALL of the following

    • The individual has one of the following anatomic or traumatic causes

      • Clinical findings of a collapsed internal nasal valve at rest or a collapsed external nasal valve (lateral walls) with inspiration, consistent with nasal vestibular stenosis.
      • A nasal deformity secondary to trauma that causes significant functional impairment, such as a nasal bone fracture causing nasal airway obstruction.
    • The individual has symptoms of nasal obstruction, such as difficulty breathing through the nasal passages and/or sinus cavities, affecting quality of life.
    • Infection, allergy, rhinitis, and polyps have been ruled out as the primary cause of nasal obstruction, as evidenced by

    Rhinoplasty for Prevention

Rhinoplasty may be considered medically necessary when performed to prevent development of nasal obstruction after removal of a large cutaneous defect, such as a cutaneous malignancy.

    ALL of the following

    • A large cutaneous defect has been removed.
    • The rhinoplasty is performed to prevent development of nasal obstruction after removal of the defect.

Rhinoplasty performed for the sole purpose of changing the appearance of the nose is considered cosmetic and is not covered.

Off-campus outpatient hospital or medical center, on-campus outpatient hospital or medical center, and inpatient hospital or medical center sites are considered not medically necessary for elective rhinoplasty when the applicable site-of-service medical-necessity criteria are not met. A hospital outpatient department may be medically necessary only when an applicable exception is present, including lack of a qualifying accessible ambulatory surgical center, age 18 years or younger, a same-session service requiring a hospital outpatient department, or a clinical condition that increases risk for complications.

An off-campus outpatient hospital or medical center, on-campus outpatient hospital or medical center, or inpatient hospital or medical center site is not medically necessary for elective rhinoplasty when the site-of-service criteria are not met. The preferred medically necessary sites are an ambulatory surgical center, an off-campus outpatient hospital or medical center, or an on-campus outpatient hospital or medical center, subject to the hospital outpatient exceptions described in this policy.

    Documentation and Review Requirements

    Prior Authorization

    Medical Necessity Review Required

Medical necessity review is required for elective rhinoplasty, including review of the procedure indication and applicable site of service. The procedure should be performed in the most appropriate, safe, and cost-effective setting unless a listed hospital exception applies.

    • Review the rhinoplasty indication against the applicable deformity, obstruction, prevention, or other medical-necessity criteria.
    • Review the proposed site of service, including whether an ambulatory surgical center is appropriate or a hospital setting exception is documented.
    Documentation Required

    Document Congenital or Cleft-Related Deformity

For rhinoplasty to correct a deformity, submit records documenting a nasal deformity secondary to cleft lip, cleft palate, or another congenital craniofacial deformity.

    Clinical Background

Nasal obstruction is a breathing symptom characterized by insufficient airflow through the nose. It may result from mucosal conditions such as rhinitis, sinusitis, or polyps, or from structural abnormalities including congenital deformity, trauma, tumors, and nasal valve disorders. Rhinoplasty may be reconstructive or functional when it corrects abnormal nasal structures or restores breathing; a procedure performed solely to improve appearance is cosmetic. Trauma-related or congenital nasal deformities may require complex or staged reconstructive procedures.

Document Nasal Obstruction and Functional Impairment

For rhinoplasty for obstruction, submit clinical findings confirming nasal valve collapse or documentation of a trauma-related nasal deformity causing significant functional impairment. The records must also document nasal obstruction symptoms severe enough to affect quality of life.

    • Collapsed internal nasal valve at rest or collapsed external nasal valve with inspiration may support medical necessity.
    • Alternatively, document a trauma-related nasal deformity, such as a nasal bone fracture causing nasal airway obstruction, with significant functional impairment.
    • Document difficulty breathing through the nose and resulting quality-of-life impact, such as chronic rhinosinusitis.
    Documentation Required

Rule Out Alternative Causes and Document Conservative Treatment

Before attributing nasal obstruction to a structural condition, document that infection, allergy, rhinitis, and polyps have been ruled out as the primary cause. Also document persistent symptoms despite at least 8 weeks of conservative management.

    • Document appropriate conservative treatment, such as decongestants or antihistamines, nasal lavage, oral or intranasal steroids, antibiotics for rhinosinusitis, or allergy assessment and treatment.
    • Record the duration of treatment and persistence of obstructive symptoms.
    Denial Risk

    Cosmetic-Only Rhinoplasty Is Not Covered

Rhinoplasty performed solely to change the appearance of the nose is considered cosmetic and is not covered as medically necessary.

    • Do not submit cosmetic-only rhinoplasty as a medically necessary procedure.
    Denial Risk

    Failure to Meet Obstruction Criteria

Rhinoplasty for obstruction may fail medical-necessity review when the records do not establish qualifying nasal valve collapse or a trauma-related deformity with significant functional impairment, clinically significant obstruction affecting quality of life, and the applicable treatment requirements.

    • Document collapsed internal or external nasal valve, or a trauma-related deformity causing significant functional impairment.
    • Document symptoms affecting quality of life.
    • Document that alternative causes were ruled out and that symptoms persisted despite at least 8 weeks of conservative management.
    Denial Risk

    Nonqualifying Site of Service

An inpatient or hospital outpatient site is not medically necessary for an elective surgical procedure when the applicable site-of-service criteria are not met. The site may be considered medically necessary only when a listed exception applies.

    • Consider an ambulatory surgical center when it can safely provide the necessary care.
    • A hospital outpatient department may qualify when no appropriate ASC is accessible within 30 miles, the individual is 18 or younger, the procedure is combined with a service requiring a hospital outpatient department, or a documented clinical risk requires the hospital setting.
    • Document applicable clinical risks or other site-of-service exceptions, such as ASA classification III or higher, significant cardiopulmonary disease, end-stage renal disease, morbid obesity, pregnancy, or a bleeding disorder requiring specialized treatment.
    Note

    IHS Site-of-Service Exception

The policy’s site-of-service medical-necessity criteria do not apply to Indian Health Services facilities. For procedures performed at an IHS facility, apply the medical-necessity criteria for the rhinoplasty procedure itself.

    • Do not apply the policy’s site-of-service criteria to an IHS facility.
    • Review the rhinoplasty indication under the procedure-specific medical-necessity criteria.
