    POLICY UPDATE CHANGES

Preferred and non-preferred denosumab product designations were updated and criteria now require documented inadequate response or intolerance to specific preferred products before coverage of non-preferred products.

Dose limit of 120 mg every 4 weeks was added for prevention of skeletal-related events in individuals with multiple myeloma and bone metastases from solid tumors for a list of denosumab products.

Several denosumab biosimilars were designated as preferred products (Bildyos, Enoby, Bilprevda, Xtrenbo) and many other biosimilars moved to non-preferred status.

New HCPCS codes Q5161 and Q5162 were added to the coding section (effective April 1, 2026).

    Coverage Criteria

inv-01: Initial therapy — Osteoporosis and adjuvant/androgen-deprivation bone mass increase

Covered when ALL of the following are met

Denosumab first-line criteria: Treatment for osteoporosis when the individual has tried and had an inadequate response or intolerance to 2 generic bisphosphonates (either 2 oral medications or 1 oral medication and 1 IV medication) unless bisphosphonates are contraindicated; OR to increase bone mass in men at high risk for fracture receiving androgen deprivation therapy for nonmetastatic prostate cancer; OR to increase bone mass in women at high risk for fracture receiving adjuvant aromatase inhibitor therapy for breast cancer60 mg every 6 months

Generic bisphosphonates include alendronate (oral), ibandronate (oral), risedronate (oral) and zoledronic acid (IV). Oral bisphosphonates are considered contraindicated in individuals with esophageal disorders (e.g., achalasia, esophageal stricture, Barrett's)

inv-02: Oncology indications — Prevention of skeletal-related events and hypercalcemia of malignancy

Covered when ALL of the following are met

SRE prevention: Used for prevention of skeletal-related events in individuals with multiple myeloma OR with bone metastases from solid tumors (for breast cancer, expected survival >= 3 months; for prostate cancer, castration-recurrent disease); there must be documented inadequate response or intolerance to intravenous zoledronic acid 4 mg.120 mg every 4 weeks

Dose limited to 120 mg every 4 weeks

Hypercalcemia of malignancy: Treatment of hypercalcemia of malignancy when there is documented inadequate response or intolerance to intravenous 4 mg zoledronic acid.120 mg every 4 weeks

Oncology denosumab products require prior zoledronic acid failure

inv-03: Giant cell tumor of bone

Covered when ALL of the following are met

Giant cell tumor treatment: Denosumab may be considered for individuals with giant cell tumor of bone.

Some products require prior trial or intolerance to specified preferred denosumab products per the step hierarchy (e.g., trial of Bilprevda and Xtrenbo before certain non-preferred products).

    inv-04: Investigational / Not covered

Investigational uses: All other uses of the medications listed in this policy for conditions not outlined in this policy are considered investigational.

    inv-05: Indication-based coverage

Covered when medically indicated and when product-specific prior authorization and step criteria are met

Indications supported by evidence: Denosumab products are covered for: treatment of postmenopausal osteoporosis at high risk for fracture (reduces vertebral, nonvertebral, and hip fractures); glucocorticoid-induced osteoporosis (denosumab non-inferior/superior to risedronate for lumbar spine BMD); to increase bone mass in men receiving androgen-deprivation therapy; to increase bone mass in women receiving adjuvant aromatase inhibitor therapy; and prevention of skeletal-related events in individuals with bone metastases or multiple myeloma (Xgeva evidence).

Indications summarized from clinical trial evidence and policy updates.

inv-06: Product preference and step requirement

Coverage of specific denosumab products is contingent on product preference and prior failure/intolerance to preferred products

Non-preferred product authorization: For a requested non-preferred denosumab product to be covered, the individual must have documented inadequate response or intolerance to the two specified preferred denosumab products (for example, Bildyos and Enoby OR Bilprevda and Xtrenbo, depending on product grouping).

Policy was updated to operationalize a two-product failure/intolerance requirement before non-preferred coverage.

    inv-07: SRE prevention dosing limit

When denosumab is used for prevention of skeletal-related events (SREs) in multiple myeloma or bone metastases from solid tumors

SRE prevention dosing: Dose for prevention of skeletal-related events is limited to 120 mg every 4 weeks for specified denosumab products.120 mg every 4 weeks

Applies to Xgeva and listed biosimilars/non-preferred products per 2026 update.

All uses of the denosumab products listed in this policy that are not specifically described under the policy’s covered indications are considered investigational and excluded from coverage.

This medical policy is a guide for determining medical necessity and does not apply to Medicare Advantage; consult the member’s benefit booklet or a customer service representative for coverage determinations under Medicare Advantage plans.

When denosumab products are used for prevention of skeletal-related events (SREs) in individuals with multiple myeloma or with bone metastases from solid tumors, the policy limits dosing to 120 mg every 4 weeks. Requests for SRE-prevention dosing that exceed 120 mg every 4 weeks are not supported by this policy and may be denied.

    Note

    Biosimilar Designations & Requirements

    Billing Rule

Use assigned HCPCS/Q-codes and J-codes per policy for denosumab products

The policy assigns specific HCPCS Q-codes to several denosumab biosimilars and lists J0897 and J3590/J9999 for denosumab reporting; confirm coding per the policy when billing as benefit assignment and coverage differ by product.

    • HCPCS codes include J0897 and Q5136–Q5162 for biosimilars; J3590 and J9999 are used for unclassified/other denosumab products.
    • New HCPCS codes Q5161 and Q5162 were added effective 04/01/26.
    Note

Enoby (preferred) must be tried per preferred-pair requirement

Enoby (with Bildyos) is a designated preferred product in the denosumab reference group — documentation of trial and failure/intolerance to Enoby (and the paired preferred product) is required per the updated criteria before some non-preferred products will be covered.

    • The 2026 update moved Enoby and Bildyos to preferred status and requires inadequate response or intolerance to these products for certain non-preferred approvals.

Prior Bisphosphonate Failure Required for Osteoporosis

For osteoporosis indications, documentation must show prior inadequate response or intolerance to two generic bisphosphonates (either two oral agents or one oral plus one IV) unless bisphosphonates are contraindicated. Generic bisphosphonates include alendronate, ibandronate, risedronate (oral) and zoledronic acid (IV).

    • Acceptable bisphosphonate trials: 2 oral agents (e.g., alendronate + risedronate) OR 1 oral agent plus 1 IV (zoledronic acid).
    • If bisphosphonates are contraindicated, document reason (e.g., esophageal disorders, other contraindications per PI).
    Denial Risk

Zoledronic Acid Failure Required for Oncology SRE/Hypercalcemia

For oncology indications (prevention of skeletal-related events, treatment of hypercalcemia of malignancy when applicable), documentation must show an inadequate response or intolerance to intravenous zoledronic acid 4 mg prior to approval of denosumab agents where required.

    • Document trial of IV zoledronic acid 4 mg and reason for inadequate response or intolerance.
    • This requirement applies to SRE prevention and hypercalcemia of malignancy where specified in criteria.
    Documentation Required

    Documentation Required — Medical Records

Medical record submission is required to demonstrate that medical necessity criteria are met. Provide office visit notes containing the diagnosis and a detailed medication history that supports the requested denosumab agent.

    • Include office visit notes with diagnosis, prior medications tried (dates, doses, responses), and documentation of intolerance or adverse effects.
    • For step or failure requirements, include records that explicitly document inadequate response (e.g., fracture, continued bone loss, lab/imaging) or intolerance to preferred and/or required prior therapies.
Provider noteConsult FDA-approved indications for specific age-based eligibility.

Clinical trial evidence supports initial use in several populations

Evidence-based initial uses: Randomized controlled trials support denosumab for reducing fractures in postmenopausal women, increasing BMD and reducing vertebral fractures in men on ADT, improving BMD and reducing clinical fractures in women on aromatase inhibitors, and improving lumbar spine BMD versus risedronate in glucocorticoid-induced osteoporosis.

Summarized from trial data cited in policy.

RequirementDocumented inadequate response or intolerance to IV zoledronic acid 4 mg is required for oncology SRE prevention indications.
    Note

Bildyos is a designated preferred product — trial required per step rules

Bildyos is designated a preferred product in the policy update and, together with Enoby, forms a required preferred pair for authorizing some non-preferred denosumab products; document trials of Bildyos as part of step requirements.

    • First-line osteoporosis list explicitly names Bildyos and Enoby as first-line products.
    • Policy changes moved Bildyos to preferred status used in required prior therapy.
    Note

Bilprevda is a designated preferred product for oncology SRE step requirements

Bilprevda was designated a preferred product in the 2026 update and is used (with Xtrenbo) as the required prior therapy pair for a subset of non-preferred denosumab products used in oncology indications.

    • Bilprevda and Xtrenbo are listed as First-Line for SRE prevention; failure/intolerance to both is required before certain other products are authorized.
    Note

Xtrenbo is a designated preferred product — part of the required preferred pair

Xtrenbo is designated a preferred product in the 2026 update and must be tried (with Bilprevda) before some non-preferred oncology denosumab products will be approved.

    • Xtrenbo and Bilprevda form the preferred pair required for SRE prevention and related indications per the updated criteria.
