    Coverage Criteria

Initial therapy - osteoporosis and adjuvant therapy

Covered when ALL of the following are met for osteoporosis and related indications (drug-specific distinctions apply):

Osteoporosis (first-line denosumab biosimilars Bildyos/Enoby): Treatment of osteoporosis when the individual has tried and had an inadequate response or intolerance to 2 generic bisphosphonates (either 2 oral medications or 1 oral medication and 1 IV medication) unless bisphosphonates are contraindicated; if oral bisphosphonates are contraindicated, a trial of IV zoledronic acid is required.
Adjuvant therapy indications: To increase bone mass in men at high risk for fracture receiving androgen deprivation therapy for nonmetastatic prostate cancer, or in women at high risk for fracture receiving adjuvant aromatase inhibitor therapy for breast cancer.
Dose limit for osteoporosis indications: Dose limited to 60 mg every 6 months.

Oncology, hypercalcemia, and giant cell tumor criteria

Oncology and other high-dose indications (second-line or agents for skeletal-related events, hypercalcemia, giant cell tumor):

Prevention of skeletal-related events (Bilprevda/Xtrenbo first-line): Bilprevda and Xtrenbo may be used for prevention of skeletal-related events in multiple myeloma or bone metastases from solid tumors; for breast cancer the individual has expected survival ≥ 3 months and for prostate cancer the individual has castration-recurrent disease.
Second-line oncology agents and requirements: For other denosumab products (e.g., Aukelso, Bomyntra, Jubereq, Osenvelt, Oziltus, Wyost, Xbryk, Xgeva) to be medically necessary for prevention of skeletal-related events: documented inadequate response or intolerance to Bilprevda AND Xtrenbo, and dose limited to 120 mg every 4 weeks.
Hypercalcemia of malignancy: Bilprevda and Xtrenbo may be used for treatment of hypercalcemia of malignancy when there is a documented inadequate response or intolerance to IV zoledronic acid; other listed denosumab products require prior inadequate response/intolerance to Bilprevda AND Xtrenbo.
Giant cell tumor of bone: Bilprevda and Xtrenbo may be used for treatment of giant cell tumor of bone; other products require prior inadequate response/intolerance to Bilprevda AND Xtrenbo.

    Investigational / Not covered uses

    Investigational uses

Investigational uses: All uses of the medications listed in this policy that are not outlined in the policy indications are considered investigational.

Product preference groups and dosing limitation

Coverage and preferrence groups with required prior failure logic and dosing limitation:

Preferred products: Bildyos (denosumab-nxxp) and Enoby (denosumab-qbde) designated preferred; coverage for these preferred agents is allowed under the medical benefit.
Non-preferred group A: Boncresa, Bosaya, Conexxence, Jubbonti, Ospomyv, Osvyrti, Prolia, and Stoboclo are non-preferred; coverage requires documented inadequate response or intolerance to Bildyos AND Enoby.
Non-preferred group B (SRE prevention dose-limited): Aukelso, Bilprevda, Bomyntra, Jubereq, Osenvelt, Oziltus, Wyost, Xbryk, Xgeva, and Xtrenbo are non-preferred for SRE prevention and require documented inadequate response or intolerance to Bilprevda AND Xtrenbo; dose limited to 120 mg every 4 weeks for prevention of skeletal-related events in multiple myeloma and bone metastases from solid tumors.120 mg every 4 weeks

    Coverage additions and classification changes

Policy history states coverage criteria were added or changed for multiple denosumab products and dose limits.

Biosimilar coverage additions: Coverage criteria were added for multiple biosimilar denosumab products including Bildyos, Bilprevda, Bomyntra, Conexxence, Jubbonti, Osenvelt, Ospomyv, Stoboclo, Wyost, Xbryk, Aukelso, Bosaya, Enoby, Xtrenbo, Boncresa, Jubereq, Osvyrti, and Oziltus.

    Dose limitation for SRE prevention

Specific dosing limit for certain oncology indications.

SRE prevention dosing: Dose limited to 120 mg every 4 weeks for prevention of skeletal-related events in individuals with multiple myeloma and individuals with bone metastases from solid tumors.120 mg every 4 weeks

All uses of the medications listed in this policy that are not specifically described in the coverage criteria are considered investigational. The policy lists multiple denosumab products by name and states that indications outside those outlined in this policy fall under investigational status.

None explicitly stated in this section of the policy.

This policy and its coverage criteria apply to commercial members and other applicable lines of business but does not apply to Medicare Advantage members. Denosumab products listed as managed through the medical benefit are described separately; consult member benefit booklets for Medicare Advantage scope.

Coverage for non-preferred denosumab products requires documented inadequate response or intolerance to the specified preferred agents. The 2026 update designates Bildyos (denosumab-nxxp) and Enoby (denosumab-qbde) as preferred; requests for non-preferred products will require evidence of prior failure or intolerance to these preferred agents before coverage is allowed.

Services or requests that do not meet the policy's specified coverage criteria, or that fall outside the listed indications for the denosumab products in this policy, may be considered not medically necessary and subject to denial. The policy history notes scope and classification changes that affect coverage decisions.

    • Dose for prevention of skeletal-related events is limited to 120 mg every 4 weeks.

    Step Therapy Requirements

    Step requirementDetails
Must document prior bisphosphonate trials or contraindication
For osteoporosis indications: trial and inadequate response or intolerance to 2 generic bisphosphonates (either 2 oral agents or 1 oral + 1 IV) is required unless bisphosphonates are contraindicated; if oral bisphosphonates are contraindicated, trial of IV zoledronic acid is required.
    Step requirementDetails
Trial and failure of preferred agents required before coverage of non-preferred denosumab products
Per 2026 update: coverage of many non-preferred denosumab products requires documented inadequate response or intolerance to the specified preferred agents (e.g., inadequate response or intolerance to Bildyos and Enoby, or to Bilprevda and Xtrenbo depending on the product group) before non-preferred products will be considered.
    Step/formulary preferenceDetails
Preferred designation may impose step or formulary preference requirements
Policy changes designated Xtrenbo, Bildyos, Enoby and Bilprevda as preferred; these preferred designations may require trial/failure of preferred biosimilars (or make certain biosimilars first-line), affecting step edits and formulary preference prior to coverage of non-preferred denosumab products.

    Initial Therapy Criteria

    Initial therapy rules

Initial agent selection and required prior therapies

Osteoporosis initial therapy: Bildyos and Enoby may be considered medically necessary for osteoporosis after required bisphosphonate trials; dose 60 mg every 6 months.
Prevention of skeletal-related events initial therapy: Bilprevda and Xtrenbo may be considered medically necessary for prevention of skeletal-related events in multiple myeloma and bone metastases; specific survival and disease-state requirements apply for breast and prostate cancer.

    Continuation & Renewal Criteria

    Continuation and approval durations

    Duration and renewal

Continuation approvals up to 2 years: Selected products may be approved up to 2 years when documentation shows the condition has stabilized or improved and the individual has not experienced serious or intolerable side effects.
Other products renewal up to 6-12 months: Other listed products may be approved up to 6 months (some up to 12 months) depending on product class and documentation of continued benefit.

    Quantity Limits & Dose Restrictions

Site of Care / Benefit Handling

    Billing Rule

Administered subcutaneously; verify medical vs. pharmacy benefit and site

Products are administered subcutaneously by a healthcare professional and may be managed under the medical or pharmacy benefit; determine the appropriate benefit and site (office or infusion center) for billing.

    • Bildyos and similar products are administered every six months and managed through medical and pharmacy benefits.
    • Aukelso, Bilprevda, Xgeva and others are managed through the medical benefit.
    Billing Rule

Medical-benefit products: bill for administration in medical settings

Denosumab products managed through the medical benefit are administered in medical settings such as infusion centers; bill under the medical benefit accordingly.

    • Examples managed through medical benefit: Aukelso, Bilprevda, Bomyntra, Xgeva, Xtrenbo, and others listed in the policy.

    Biosimilar & Product-specific Guidance

    Billing Rule

Bildyos and Enoby preferred for osteoporosis after bisphosphonate trials

Bildyos and Enoby may be considered medically necessary for osteoporosis after required bisphosphonate trials; other denosumab products have additional prior-failure requirements per the policy.

    • Dose for osteoporosis with Bildyos/Enoby is limited to 60 mg every 6 months.
    • Other products (e.g., Prolia and several biosimilars) require prior failure of Bildyos AND Enoby for osteoporosis coverage.
    Billing Rule

Bilprevda and Xtrenbo preferred for SRE prevention; others require prior failure and dose limit

Bilprevda and Xtrenbo may be used first-line for prevention of skeletal-related events; other oncology denosumab products require documented inadequate response or intolerance to Bilprevda AND Xtrenbo and the dose is limited to 120 mg every 4 weeks for SRE prevention.

    • For breast cancer SRE prevention, expected survival must be ≥ 3 months; prostate cancer requires castration-recurrent disease where specified.
    • Dose limit for SRE prevention: 120 mg every 4 weeks.

    Background

Denosumab products are monoclonal antibodies that act as RANK ligand inhibitors, reducing osteoclast-mediated bone resorption. They are used to increase bone mass in osteoporosis and in settings to prevent or treat skeletal complications of malignancy (for example, prevention of skeletal-related events, treatment of hypercalcemia of malignancy, and treatment of giant cell tumor of bone). Coverage determinations in this policy reference FDA-approved dosing and prescribing information and vary by indication and product.

  1. Non-preferred coverage requires documented failure or intolerance to the named preferred agents (see product groups).
  2. Prior Authorization

Prior authorization applies to denosumab products and updated HCPCS codes

This medical policy requires prior authorization for denosumab products and the associated HCPCS codes; coding updates in 2026 added new HCPCS codes that are subject to the policy's authorization requirements.

    • New HCPCS codes added: Q5161, Q5162 (effective 04/01/26) and Q5165, Q5166, Q5167, Q5171 (effective 07/01/26) — these codes are covered under the policy's prior authorization rules.
    • Some biosimilars are listed parenthetically for J3590; prior authorization still applies.
    Step Therapy

    Bisphosphonate step requirement for osteoporosis

For osteoporosis indications, document a trial and inadequate response or intolerance to two generic bisphosphonates (either two oral agents or one oral + one IV). If oral bisphosphonates are contraindicated, a trial of IV zoledronic acid is required.

    • Generic bisphosphonates named in policy: alendronate, ibandronate, risedronate (oral) and zoledronic acid (IV).
    • Oral bisphosphonates are considered contraindicated in individuals with esophageal disorders (e.g., achalasia, esophageal stricture, Barrett's esophagus, esophageal varices).
    Step Therapy

Step/failure therapy required before non-preferred denosumab coverage

Coverage of many non-preferred denosumab products requires documented inadequate response or intolerance to two preferred denosumab products (preferred agents vary by product group — e.g., Bildyos and Enoby or Bilprevda and Xtrenbo).

    • Non-preferred group A requires failure of Bildyos AND Enoby.
    • Non-preferred group B (SRE prevention) requires failure of Bilprevda AND Xtrenbo and is dose-limited to 120 mg every 4 weeks.
    Note

Policy history — preferred status changes may affect formulary/step edits

Policy history shows preferred status changes (e.g., Bildyos, Enoby, Bilprevda, and Xtrenbo designated preferred and several previously preferred agents moved to non-preferred). These changes may affect formulary and step-edit requirements.

    • Provider should verify current preferred/non-preferred status when submitting authorizations because step requirements depend on product designations.
    • Effective date for these changes was August 7, 2026, after a 90-day provider notification.
    Documentation Required

Required medical record elements — document diagnosis and medication history

Medical records submitted for review must document that the medical necessity criteria are met. At minimum, include office visit notes with the diagnosis and full medication history.

    • Chart notes should demonstrate trials of prior therapies, reasons for inadequate response or intolerance, and current dosing.
    • For non-preferred product requests, identify the specific denosumab product requested and prior therapy details.
    Documentation Required

Product and prior therapy documentation required for medical benefit submissions

When requesting coverage under the medical benefit, identify the exact denosumab product and provide evidence of prior inadequate response or intolerance to required prior therapies (bisphosphonates or listed preferred denosumab agents as applicable).

    • Specify the product name (e.g., Aukelso, Bildyos, Bilprevda, Xtrenbo, Prolia, etc.) on the request.
    • Include documentation of trial duration, outcomes, and reasons for discontinuation or intolerance.
    Documentation Required

Include references and prescribing information with requests

Consult the prescribing information and references for the specific denosumab product when preparing an authorization request to ensure correct dosing and to document safety considerations.

    • Prescribing information for Prolia and Xgeva is referenced in the policy; product-specific labels are listed in References.
    • Use cited prescribing information to support dose and contraindication claims.
    Denial Risk

Denial risk — prior therapy not met

If documentation does not show required prior therapy trials or evidence of inadequate response/intolerance (e.g., to two generic bisphosphonates or to required preferred denosumab agents), the request may be denied for prior therapy not met.

    • For osteoporosis, absence of trials of two generic bisphosphonates (or one oral + IV when required) is a reason for denial.
    • For non-preferred denosumab products, lack of documented failure/intolerance to the specified preferred agents will risk denial.
    Denial Risk

Denial risk — non-preferred product prior failure required

Requests for non-preferred denosumab products will be denied if the individual has not had an inadequate response or intolerance to the specified preferred agents named in the policy for that product group.

    • Examples: Non-preferred group A requires failure of Bildyos AND Enoby; non-preferred group B (SRE prevention) requires failure of Bilprevda AND Xtrenbo.
    • Ensure documentation clearly states prior agents tried and clinical response.
    Denial Risk

Denial risk — services not meeting the medical policy criteria may be denied

Services that do not meet the medical policy's coverage criteria or that are for indications considered investigational may be denied. Note that some products (e.g., Xgeva) were moved from another policy, reflecting scope and coverage updates.

    • All uses not outlined in this policy are considered investigational and not covered.
    • Confirm the indication aligns with the policy's covered indications before requesting authorization.
    Note

No site-of-care restriction specified here; check member benefits

No specific site-of-care restriction is listed for infusion center use in this section; providers should consult member benefit booklets for any site-specific restrictions or coverage details.

    • General scope and benefit limits apply; confirm member-specific coverage with the benefit booklet or customer service.
    Note

Designated preferred denosumab products (confirm current list)

The policy designates specific products as preferred denosumab agents in the 2026 update (e.g., Bildyos, Enoby, Bilprevda, Xtrenbo); confirm preferred product when preparing prior authorization and step-therapy documentation.

    • 2026 changes moved multiple products to preferred or non-preferred status and updated required prior-failure logic.
    • Use the current preferred list when documenting prior therapy failures or formulary exceptions.
    Note

Xtrenbo designated preferred in 2026 update

Xtrenbo (denosumab-qbde) is listed as a designated preferred denosumab product per the 2026 policy update; use that designation when determining step requirements and prior-authorizations.

    • Preferred designations were changed effective August 7, 2026 following the 2026 update and coding changes.
    Billing Rule

Designated preferred denosumab product (verify product name on requests)

The policy identifies 'denosumab' biosimilars and reference products with designated preferred status; verify the exact branded/biosimilar name on the request and follow the policy’s preferred/non-preferred rules.

    • Preferred denosumab product names and biosimilar suffixes are listed in the drug names and product groups in the policy update.
