    Coverage Criteria

Initial therapy — osteoporosis and adjuvant/androgen-deprivation indications

Covered when ALL of the following are met for specified products and indications

Osteoporosis indication (primary care/osteoporosis): Treatment of osteoporosis when the individual has tried and had an inadequate response or intolerance to 2 generic bisphosphonates (either 2 oral medications or 1 oral medication and 1 IV medication) unless bisphosphonates are contraindicated.2 bisphosphonates

If oral bisphosphonates are contraindicated (e.g., esophageal disorders), only IV zoledronic acid is required as prior therapy.

Adjuvant/androgen-deprivation therapy indications: To increase bone mass in men at high risk for fracture receiving androgen deprivation therapy for nonmetastatic prostate cancer OR to increase bone mass in women at high risk for fracture receiving adjuvant aromatase inhibitor therapy for breast cancer.

Dose limited to 60 mg every 6 months.

Oncology / skeletal-related events / hypercalcemia / giant cell tumor

Oncology and skeletal-related event prevention criteria

Prevention of skeletal-related events: Bilprevda (denosumab-nxxp) and Xtrenbo (denosumab-qbde) may be used for prevention of skeletal-related events in individuals with multiple myeloma OR bone metastases from solid tumors; for breast cancer treatment the individual has expected survival of 3 months or greater; for prostate cancer the individual has castration recurrent disease.expected survival >= 3 months for breast cancer

Dose limited to 120 mg every 4 weeks for these indications.

Second-line oncology or alternative denosumab products: For other denosumab products (Aukelso, Bomyntra, Jubereq, Osenvelt, Oziltus, Wyost, Xbryk, Xgeva), coverage for prevention of skeletal-related events requires documented inadequate response or intolerance to Bilprevda and Xtrenbo and documented inadequate response or intolerance to intravenous 4 mg zoledronic acid where specified.prior failure of Bilprevda and Xtrenbo (and zoledronic acid where specified)

Dose limited to 120 mg every 4 weeks.

Hypercalcemia of malignancy: Bilprevda and Xtrenbo may be considered for treatment of hypercalcemia of malignancy when there is a documented inadequate response or intolerance to intravenous 4 mg zoledronic acid. Other denosumab products require prior trial/failure of Bilprevda and Xtrenbo.

    Investigational

    Investigational uses

Not covered (Investigational): All other uses of the medications listed in this policy for conditions not outlined in this policy are considered investigational.

Specific products are listed as investigational for other uses within the policy.

    Product preference and dosing criteria

    Coverage and product-preference rules:

Preferred products - general: Bildyos (denosumab-nxxp), Enoby (denosumab-qbde), Bilprevda (denosumab-nxxp), and Xtrenbo (denosumab-qbde) are designated preferred denosumab products as of the 2026 update.

Preferred status reflected in 2026 Update.

Non-preferred products - prior failure requirement: Specified non-preferred denosumab products require inadequate response or intolerance to two named preferred agents before coverage; examples include requirement to try Bildyos AND Enoby before some non-preferred products, or Bilprevda AND Xtrenbo before other non-preferred products.

See 2026 Update for exact product pairings.

Dose limit for SRE prevention: For prevention of skeletal-related events in individuals with multiple myeloma and bone metastases from solid tumors, dose is limited to 120 mg every 4 weeks for specified denosumab products.120 mg every 4 weeks

Applied to Aukelso, Bilprevda, Bomyntra, Jubereq, Osenvelt, Oziltus, Wyost, Xbryk, Xgeva, and Xtrenbo.

    Product-specific coverage and preference

Policy includes product-specific coverage criteria and preferred product designation.

Product listing and coverage: Specific denosumab products and biosimilars are listed with coverage criteria; some products are labeled preferred (for example, Xtrenbo) and others have updated coverage requirements established in the policy history.

See product list and history for exact product names and coverage status.

Skeletal-related events (SRE) prevention — dosing limit

Covered when ALL of the following are met:

SRE prevention dosing: Indication for prevention of skeletal-related events in multiple myeloma and bone metastases from solid tumors is covered with a dose limit of 120 mg every 4 weeks.120 mg every 4 weeks

Dose limit added/affirmed in 2026 updates.

All uses of denosumab products that are not specifically described as covered within this policy are classified as investigational. The policy lists specific denosumab products and indications; uses outside those outlined here do not meet medical necessity criteria and are considered investigational.

The updated policy establishes a dosing ceiling for prevention of skeletal-related events: requests for dosing that exceed 120 mg every 4 weeks in individuals with multiple myeloma or bone metastases from solid tumors are not supported by the policy and may be denied.

This medical policy does not apply to Medicare Advantage members. Member benefit specifics and coverage should be confirmed via the member's benefit booklet or customer service representative.

Policy history documents reflect multiple product listing and coding changes over recent reviews, including the transfer of Xgeva into this policy, additions of numerous biosimilar denosumab products, designation changes to preferred status for certain products, and the addition of new HCPCS/Q-codes effective April 1 and July 1, 2026. Providers should reference the history and coding sections for exact product names, preferred status, and applicable codes when submitting requests.

    Background and Clinical Rationale

Bone remodeling is a dynamic balance between osteoclast-mediated resorption and osteoblast-mediated formation; with aging and certain therapies this balance shifts toward increased resorption leading to decreased bone mass and increased fracture risk. Denosumab and its biosimilars are RANK ligand inhibitors that reduce osteoclast activity by blocking RANK ligand, thereby decreasing bone resorption and increasing bone mass. In oncology contexts, RANK ligand inhibition is used to reduce the risk of skeletal-related events (SREs) such as pathologic fracture, spinal cord compression, or the need for radiation or surgery to bone.

inadequate response or intolerance to 4 mg IV zoledronic acid
Giant cell tumor of bone: Bilprevda and Xtrenbo may be used for treatment of giant cell tumor of the bone. Other denosumab products require prior inadequate response or intolerance to Bilprevda and Xtrenbo before coverage.prior failure of Bilprevda and Xtrenbo for alternative products
  1. Non‑preferred products were updated to require inadequate response or intolerance to Bildyos AND Enoby, or to Bilprevda AND Xtrenbo, depending on the non‑preferred product pairing.
  2. Ensure PA documentation explicitly states prior trials of the named preferred agents and reasons for intolerance or inadequate response.
  3. Prior Authorization

Submit PA with the denosumab product name and appropriate HCPCS/Q‑code

Prior authorization requirements apply to denosumab products billed under the HCPCS/Q‑codes and unclassified biologics codes listed in the coding section; include the product name and matching HCPCS/Q‑code on the PA submission.

    • Covered and newly added codes include J0897, J3590, J9999, Q5136, Q5157–Q5159, and Q5161–Q5162, Q5165–Q5167, Q5171 as noted in the policy history.
    • When using J3590 or other unclassified codes, include the denosumab product name in documentation as indicated in coding guidance.
    Step Therapy

Document required bisphosphonate trials for osteoporosis before denosumab

For osteoporosis indications, the policy requires prior trials of two generic bisphosphonates (either two oral agents or one oral agent plus one IV zoledronic acid) with documented inadequate response or intolerance before denosumab is covered, unless bisphosphonates are contraindicated.

    • If oral bisphosphonates are contraindicated (e.g., esophageal disorders), only IV zoledronic acid is required as prior therapy.
    • Document dates, agents tried, duration, and reason for inadequate response or intolerance in the medical record and PA submission.
    Step Therapy

Complete denosumab‑to‑denosumab step therapy: document trials of specified preferred products

Step therapy for certain non‑preferred denosumab products requires documented inadequate response or intolerance to the specified preferred denosumab agents (examples: Bildyos and Enoby, or Bilprevda and Xtrenbo) before coverage of the non‑preferred product will be authorized.

    • Confirm which preferred pair applies to the requested non‑preferred product per the 2026 product‑preference updates and include prior trial details in the PA.
    • Failure to document these preferred‑agent trials may result in denial (see denial‑risk blocks).
    Step Therapy

Ensure denosumab step therapy to preferred products is documented

Step therapy for some denosumab products specifically requires documented inadequate response or intolerance to the named preferred denosumab products (for example, Bildyos and Enoby or Bilprevda and Xtrenbo) before non‑preferred alternatives are covered.

    • Review the policy product pairings to determine which preferred agents must be tried and failed for the requested non‑preferred product.
    • Include objective evidence of inadequate response or intolerance in the PA submission.
    Documentation Required

Provide office visit notes and medication history with PA

Include office visit notes that contain the diagnosis and complete medication history in the medical record submitted for review to support medical necessity for denosumab coverage.

    • Office visit notes should document the indication, relevant clinical findings, prior therapies tried, and reasons for inadequate response or intolerance.
    • These notes must be available for PA review and continuation/renewal considerations.
    Documentation Required

Identify the specific denosumab product and prior‑therapy details in documentation

Documentation must identify the exact denosumab product requested and provide evidence of prior inadequate response or intolerance to the required prior therapies (preferred denosumab agents or bisphosphonates) when applicable.

    • State the product name (e.g., Bildyos, Enoby, Bilprevda, Xtrenbo, Prolia, Xgeva, etc.) in the PA and medical records.
    • Include dates and reasons for discontinuation or failure of prior agents (e.g., adverse events, lack of efficacy).
    Billing Rule

Match product name to HCPCS/Q‑code in records and billing

Document the denosumab product name and the HCPCS/Q‑code used for billing to match policy coding updates and product‑specific coverage criteria; include product name when unclassified codes (J3590, J9999) are used.

    • Policy history added multiple new HCPCS/Q‑codes (Q5161–Q5162, Q5165–Q5167, Q5171) and parenthetical product listings for J3590; ensure billing reflects the current code for the exact product.
    • Mismatch between documented product and submitted code may lead to coding/coverage denial.
    Step Therapy

Step requirement — two bisphosphonate trials required for osteoporosis

For osteoporosis, the PA must show trials of two generic bisphosphonates (two oral agents or one oral + IV zoledronic acid) with inadequate response or intolerance before denosumab will be approved, unless bisphosphonates are contraindicated.

    • If oral bisphosphonates are contraindicated, document that and show trial/failure of IV zoledronic acid instead.
    • Include duration and clinical outcome of each bisphosphonate trial in the submission.
    Step Therapy

Step therapy — document inadequate response/intolerance to preferred denosumab agents

Step therapy requires documented inadequate response or intolerance to the specified preferred denosumab products (e.g., Bildyos and Enoby or Bilprevda and Xtrenbo) before covering specified non‑preferred denosumab alternatives.

    • Verify which preferred agents apply to the requested non‑preferred product per the 2026 updates and include evidence of trials in the PA.
    • Lack of this documentation may result in denial of coverage for the non‑preferred product.
    Note

Account for 2026 preferred‑product designations when planning therapy

Preferred product designations changed in the 2026 update; these preferences affect which agents must be tried first and may imply substitution expectations at PA review.

    • Bildyos, Enoby, Bilprevda, and Xtrenbo were changed to preferred status in the 2026 update.
    • Confirm current preferred/non‑preferred status in the policy history when preparing the PA.
    Denial Risk

Denial risk — missing bisphosphonate trial documentation

Requests may be denied if the individual has not tried and had an inadequate response or intolerance to the required generic bisphosphonates (two oral or one oral plus one IV) and documentation of those trials is not provided.

    • Include explicit documentation of prior bisphosphonate trials and the clinical rationale for denosumab to avoid denial.
    • If oral bisphosphonates were contraindicated, document the contraindication and IV zoledronic acid trial as specified.
    Denial Risk

Denial risk — missing oncology prior‑therapy (zoledronic acid / denosumab) documentation

Oncology‑use denosumab at 120 mg every 4 weeks requires documentation of inadequate response or intolerance to intravenous 4 mg zoledronic acid and prior trials/failures of the specified denosumab products per the policy; lacking this documentation may lead to denial.

    • For prostate cancer, castration‑recurrent disease plus documented inadequate response or intolerance to IV 4 mg zoledronic acid is required.
    • For SRE prevention with other denosumab products, the individual must have tried and failed Bilprevda AND Xtrenbo where specified.
Oral generic bisphosphonate referenced as prior therapy option
Zoledronic acidIV bisphosphonate (required as part of 1 oral + 1 IV pathway or when oral agents contraindicated)
Skeletal-related events (SREs) — definition and examples
DefinitionClinical complications from bone metastases such as pathologic fracture, spinal cord compression, or need for radiation or surgery to bone
ExamplesPathologic fracture; spinal cord compression; requirement for radiation to bone; need for surgery to bone
RelevancePrevention of SREs is an indicated oncology use (e.g., Xgeva shown noninferior to zoledronic acid for delaying first SRE)
    Denosumab dosing intervals
Prolia-type dosing (osteoporosis)60 mg subcutaneously every 6 months
Xgeva-type oncology dosing (SRE prevention)120 mg every 4 weeks (dose-limited by policy for specified products)
AdministrationSubcutaneous injection by a healthcare professional; benefit administration (medical or pharmacy) depends on product
SREs — contextual definition for clarity
SREs — concise definitionAdverse outcomes from bone metastases (eg, pathologic fracture, spinal cord compression, need for radiation or surgery to bone)
Clinical importancePrevention of SREs reduces morbidity; supported by trials of Xgeva vs zoledronic acid
Policy contextSRE prevention is a covered oncology indication with specific dosing and prior-therapy requirements in the policy

Administration — Typical administration and initial-use context

    Typical administration and initial-use context

Administration route and interval: Denosumab (Prolia-type and listed biosimilars) is administered subcutaneously by a healthcare professional every six months for osteoporosis-type indications; Xgeva-type dosing (SRE prevention) is administered per indication (e.g., 120 mg every 4 weeks).

Administration setting managed through medical and pharmacy benefit per product.

    Initial therapy (not detailed here)

Policy history documents additions of products and coverage criteria; initial therapy requirements are defined in the main policy body

Initial therapy: Specific initial therapy criteria exist for denosumab products but are not fully detailed in these history chunks; refer to the main policy criteria sections for product-specific initial therapy rules.
