    POLICY UPDATE CHANGES

Added new FDA-approved indication for relapsed or refractory large B-cell lymphoma (LBCL) in adult patients with criteria including not being a candidate for CAR T-cell therapy.

Extended initial approval durations for Medicaid/HIM from 6 to 12 months and standardized Commercial approval to '6 months or to the member's renewal date, whichever is longer.'

Per 3Q 2026 review, updated BrECADD component use: removed Deauville 4-5 option and revised age from 18-61 to 18-60; added option for members unfit for intensive therapy for combination with dacarbazine or nivolumab.

Added option for brentuximab use with CHP as alternative to AVD if vinblastine is unavailable due to shortage, and added option for use with nivolumab for age > 60 years.

For B-cell lymphomas, clarified that DLBCL includes histologic transformation of indolent lymphomas to DLBCL and removed requirement that disease is CD30-positive.

If the requested use is not addressed in Section III (Diagnoses/Indications for which coverage is NOT authorized) and the policy does not reflect a recent label change within the last 6 months, providers should follow the plan's referral guidance. For drugs on the formulary or preferred drug list, refer to the no-coverage-criteria policy for the applicable line of business (e.g., CP.CPA.190 for commercial, HIM.PA.33 for ICHRA, CP.PMN.255 for Medicaid). For drugs not on the formulary/PDL, refer to the non‑formulary policy for the applicable line of business (e.g., CP.CPA.190, HIM.PA.103, CP.PMN.16). If neither of those pathways applies, refer the request to the applicable off‑label use policy for the member's line of business.

Non‑FDA approved indications that are not specifically addressed in this policy are excluded from coverage unless the provider submits sufficient documentation of efficacy and safety in accordance with the applicable off‑label use policy (CP.CPA.09 for commercial, HIM.PA.154 for ICHRA, CP.PMN.53 for Medicaid). Requests for such uses should include supporting evidence per those off‑label policies to be considered for coverage.

Concomitant administration of brentuximab vedotin with bleomycin is contraindicated because of the risk of pulmonary toxicity. Do not authorize combined use of these agents.

Per the 3Q 2026 annual review and NCCN updates, the prior policy criterion that required a Deauville score of 4–5 for use of brentuximab vedotin as a component of BrECADD has been removed. The BrECADD component eligibility is now governed by the revised age and regimen/fitness criteria rather than a Deauville 4–5 requirement.

Uses that are not FDA‑approved and are not specifically covered by this policy may be considered not medically necessary if adequate supporting evidence required by the applicable off‑label use policy is not provided.

Requests for brentuximab vedotin that fall outside the policy's revised age limits or the regimen/combination criteria (for example, BrECADD age limits revised to 18–60 years for certain uses) may be considered not medically necessary per plan guidelines when they do not meet the updated eligibility criteria.

Previously untreated or relapsed PTCL/sALCL1.8 mg/kg IV up to 180 mg every 3 weeks (with each chemo cycle for 6–8 doses for previously untreated; relapsed sALCL: same q3w until progression)

    Prior authorization durations updated

Prior authorization requirements and initial approval durations were updated: Medicaid/HIM initial approvals were extended to 12 months and Commercial approvals revised to 6 months or to the member's renewal date, whichever is longer.

    Step Therapy

    Sequencing / eligibility requirements

For relapsed or refractory LBCL, coverage requires that the member is not a candidate for autologous/allogeneic stem cell transplant or CAR T‑cell therapy as part of the eligibility criteria.

    • This sequencing/eligibility requirement is specified under B‑Cell Lymphomas where Adcetris is prescribed as subsequent therapy.
    • Documentation must indicate transplant/CAR T candidacy was assessed.
    Note

    CAR T‑cell candidacy considerations

Policy references CAR T‑cell candidacy for select B‑cell lymphoma pathways; for example, relapsed/refractory LBCL requires the member not be a candidate for CAR T‑cell therapy.

    • This criterion was added with the new FDA‑approved LBCL indication (RT4) and is listed among B‑Cell Lymphoma coverage conditions.
    Step Therapy

    Step/sequence therapy implications

Regimen sequencing and combination-use restrictions were modified per NCCN updates (examples: use with CHP allowed as alternative to AVD if vinblastine unavailable; added combinations with nivolumab or dacarbazine for members unfit for intensive therapy).

    • BrECADD component eligibility and age limits were revised; CHP may be used when vinblastine is unavailable.
    • Added options for combination with nivolumab or dacarbazine for older/unfit patients.
    Documentation Required

    Clinical documentation

Providers must submit clinical documentation (office chart notes, lab results, or other clinical information) supporting that the member has met all applicable approval criteria.

    • Include documentation of diagnosis, prescriber specialty/consultation, regimen, dosing, and treatment history.
    • For continued therapy, show member is currently receiving Adcetris via the Centene benefit or has received it for at least 30 days.
    Documentation Required

    Dose increase documentation

For requests involving dose increases, submit documentation that the new dose does not exceed the indication‑specific maximums listed in the policy or that the increase is supported by practice guidelines or peer‑reviewed literature with prescriber‑submitted evidence.

    • Policy lists specific maximums (e.g., LBCL: 1.2 mg/kg up to 120 mg q3w; many indications: 1.8 mg/kg up to 180 mg q3w).
    • If exceeding listed maxima, prescriber must provide supporting literature/guideline evidence.
    Documentation Required

    Continued therapy documentation

For continued therapy requests, provide documentation that the member is currently receiving Adcetris via the Centene benefit or has received it for at least 30 days, and that the member is responding positively to therapy.

    • Continued therapy criteria require evidence of clinical benefit and duration of prior therapy (≥30 days) before approval of ongoing treatment.
    Documentation Required

    Clinical documentation expectations

Document clinical justification consistent with current standards of medical practice and evidence‑based guidelines referenced by the plan (e.g., NCCN, prescribing information).

    • Prescribers should reference NCCN guidance or peer‑reviewed literature when relying on off‑label or modified regimens.
    • Ensure documentation aligns with regimen‑specific criteria in the policy (age limits, dosing, combination restrictions).
    Denial Risk

Documentation requirement — risk of denial if missing

Failure to submit required documentation (office notes, labs, or other clinical information) showing the member meets all approval criteria may result in denial of the request.

    • Missing supporting clinical documentation can trigger denial.
    • For non‑listed or off‑label uses, lack of sufficient evidence per off‑label policies may also lead to denial.
    Denial Risk

    Non‑FDA indications — documentation required

Requests for non‑FDA approved indications that are not addressed in this policy may be denied unless sufficient documentation of efficacy and safety is provided per the applicable off‑label use policy (CP.CPA.09, HIM.PA.154, CP.PMN.53).

    • Refer to the off‑label use policy for required evidence when requesting coverage for non‑FDA indications.
    • If the use is not listed in this policy, submit supporting literature and rationale per the off‑label policy referenced.
    Denial Risk

    Denial triggers for criteria nonconformance

Requests that fail to meet updated age, regimen, or line‑of‑therapy criteria (for example, revised BrECADD age limits or regimen eligibility) may be denied per plan policy.

    • Examples include the BrECADD component age revision (now age 18–60 for certain adult uses) and removal of the Deauville 4–5 option.
    • Ensure requested use matches the policy's current regimen and age criteria.
