OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Initial Therapy

Covered when ALL of the following are met:

Initial Approval for Acne Vulgaris: 1. Prescribed by a MD/DO, PA, or NP; 2. Member is between the ages of 22 and 40; 3. Member has one of the following indications: a) Comedonal (noninflammatory) acne; OR b) Mild papulopustular and mixed (comedonal and papulopustular) acne when used in combination with (i) PDL topical Benzoyl Peroxide, OR (ii) PDL topical Benzoyl Peroxide with clindamycin or erythromycin; OR c) Moderate to severe papulopustular and mixed (comedonal and papulopustular) acne when used in combination with (i) doxycycline, minocycline, or sarecycline plus PDL topical Benzoyl Peroxide unless intolerant/contraindicated, OR (ii) if contraindicated or intolerant to tetracyclines, use a macrolide antibiotic (azithromycin or erythromycin); 4. Request is for a PDL tretinoin product.

    Approval duration: 3 months

    Continuation Therapy

Covered for continuation when ALL of the following are met:

Continued Therapy for Acne Vulgaris: 1. Currently receiving medication via Centene benefit or member has previously met initial approval criteria; 2. Member has shown a positive response to therapy; 3. If member is on a regimen with antibiotic (oral or topical) therapy, benzoyl peroxide is included; 4. Member does not have any contraindications to tretinoin therapy.

    Approval duration: 6 months

The FDA lists several indications for topical tretinoin beyond acne vulgaris, including fine facial wrinkles, mottled facial hyperpigmentation, and tactile facial roughness. These non-acne indications are recognized FDA-approved uses but are explicitly excluded from coverage under this policy, which governs the use of topical tretinoin for adult acne vulgaris only.

If state Medicaid coverage provisions conflict with this clinical policy, the state Medicaid coverage provisions take precedence. Providers should consult the applicable state Medicaid manual for specific Medicaid coverage rules that may override this policy.

This policy states that topical tretinoin is medically necessary only for adult acne vulgaris when the policy’s specified criteria are met. Use of topical tretinoin outside of those defined criteria (for example, for non-covered indications or when required prescriber, age, combination-therapy, or product formulary requirements are not satisfied) may be considered not medically necessary and subject to denial.

    Provider Actions and Requirements

    Prior Authorization

Prior authorization required for PDL tretinoin

Topical tretinoin requires prior authorization and the request must be for a PDL tretinoin product. The prescription must be written by an MD/DO, PA, or NP and include documentation showing the member meets all initial approval criteria.

    • Prescriber must be MD/DO, PA, or NP.
    • Request must be for a PDL tretinoin product.
    • Include documentation supporting all approval criteria (see Required documentation).
    Prior Authorization

    No other prior authorization specified

No additional prior authorization requirements are specified in this excerpt beyond the PDL and prescriber requirements already stated in the policy.

    Definitions

Topical tretinoin — definition and formulation details
DefinitionTretinoin is a topical acne medication that induces differentiation and decreases proliferation of APL cells.
FormulationsAvailable as creams, ointments, and gels for topical application.
Indicated use (scope)Used for treatment of adult acne vulgaris (policy governs acne vulgaris indications only).
Role in therapyTopical retinoids (including tretinoin) are recommended as initial therapy for most patients and are beneficial for comedonal and inflammatory acne.
Use with antimicrobialsWhen inflammation is present, combination with antimicrobial therapy (eg, benzoyl peroxide or topical antibiotics) is recommended to improve efficacy and reduce antibiotic resistance.

    Background

Acne vulgaris is a chronic inflammatory skin condition characterized by comedones and inflammatory lesions that can produce substantial psychosocial impact. Medical therapies target pathogenic factors such as follicular hyperproliferation, sebum production, Cutibacterium acnes proliferation, and inflammation; topical retinoids are recommended as initial therapy for many patients and are effective for both comedonal and inflammatory acne. Combination use of benzoyl peroxide is advised when antibiotics are part of the regimen to reduce resistance.

    Step Therapy

Combination therapy required by disease severity

For mild papulopustular or mixed acne, topical tretinoin must be used together with a PDL topical benzoyl peroxide product or PDL topical benzoyl peroxide combined with clindamycin or erythromycin. For moderate–severe disease, tretinoin must be combined with doxycycline, minocycline, or sarecycline plus PDL topical benzoyl peroxide unless the member is intolerant/contraindicated to tetracyclines; if intolerant/contraindicated, a macrolide (azithromycin or erythromycin) may be used.

    • Mild papulopustular/mixed: PDL topical benzoyl peroxide alone OR PDL benzoyl peroxide with clindamycin or erythromycin.
    • Moderate–severe: doxycycline, minocycline, or sarecycline PLUS PDL topical benzoyl peroxide unless intolerant/contraindicated to tetracyclines.
    • If intolerant/contraindicated to tetracyclines, use a macrolide antibiotic (azithromycin or erythromycin).
    Step Therapy

    No additional step therapy specified

This policy excerpt does not specify any step therapy sequencing requirements beyond the combination therapy and prior authorization criteria already described.

    Documentation Required

Submit office notes and labs to support criteria

Providers must submit documentation, including office chart notes and any relevant lab results, demonstrating the member meets all approval criteria for topical tretinoin.

    • Include documentation that prescriber is an MD/DO, PA, or NP.
    • Document member age (22–40) and specific acne indication.
    • If applicable, document concurrent antibiotic or benzoyl peroxide therapy and intolerance/contraindication rationale.
    Note

Policy terms are binding; claims imply acceptance

By providing services to members and/or submitting claims for payment, providers (and members/their representatives) agree to be bound by the policy terms and conditions as expressed in contractual relationships with the Health Plan.

    • Submission of claims constitutes agreement to the policy terms where no separate contract exists.
    • Providers and members are bound through existing contracts to the terms of this clinical policy.
    Denial Risk

Denial risk if approval criteria are unmet

Requests that do not meet the policy criteria for adult acne vulgaris are at risk for denial. Common reasons include failure to: document an eligible prescriber or member age (22–40), demonstrate the required indication, request a PDL tretinoin product, or show required combination therapy where specified.

    • Missing or incorrect prescriber type (must be MD/DO, PA, or NP) may lead to denial.
    • Member age outside 22–40 is not eligible under this policy.
    • Requests lacking required combination therapy (per disease severity) or not for a PDL tretinoin product are at risk for denial.
    Denial Risk

State Medicaid precedence may cause denial

When state Medicaid coverage provisions conflict with this clinical policy, state Medicaid rules take precedence and the request or claim may be denied accordingly.

    • Refer to the state Medicaid manual for coverage provisions that override this policy.
    • Claims/services that conflict with state Medicaid provisions may be denied.
