This policy governs medical-necessity review of the site where covered provider-administered specialty infusions are delivered. Drug-specific prior-authorization criteria must be satisfied first; the requested site is then evaluated as preferred or as a medically necessary exception.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Site-of-Care Coverage Criteria

    Nonpreferred site-of-care exceptions

A nonpreferred site of care can be considered medically necessary when ANY of the following criteria are met.

    Nonpreferred-site medical exception

    • New infusion initiation: The medication is a brand-new infusion medication and treatment is within the first 60 days.first 60 days

Subsequent doses are subject to preferred-site requirements unless additional documentation supports an exception.

    • Unmanageable prior adverse event: There is documentation of a previous adverse event that cannot be appropriately managed with premedications and requires a higher level of care.
    • Comorbidity or instability: There is a documented history of comorbidity or instability that precludes treatment at a less intensive site of care.
    • Access and home-infusion limitation: A preferred site of care is not accessible to the member, and the member's home is not suitable for home infusion.

Both access limitations must be present.

    Preferred site of care

Preferred-site requirements apply after drug-specific coverage criteria are met.

Site-of-care coverage framework: The medication meets the applicable drug-specific prior-authorization criteria, and the requested site is evaluated under the preferred-site-of-care policy.

Nonpreferred sites are generally medically unnecessary because they significantly increase cost versus available alternatives unless an exception is documented.

A nonpreferred site of care is considered medically unnecessary when no documented medical exception applies. A nonpreferred site may be considered medically necessary when the member is within the first 60 days of initiating a brand-new infusion medication, has a prior adverse event that cannot be managed with premedications and requires a higher level of care, has a comorbidity or instability preventing treatment at a less intensive site, or cannot access a preferred site and has a home unsuitable for home infusion.

A nonpreferred site of care that significantly increases the cost of care compared with available alternatives is considered medically unnecessary when it does not meet a documented medical exception. After the first 60 days of a brand-new infusion medication, subsequent doses are subject to preferred-site requirements unless additional documentation supports an exception.

    Prior Authorization and Documentation

    Prior Authorization

    Meet drug-specific prior authorization criteria

Before requesting a site-of-care review, confirm that the provider-administered medication meets the applicable prior-authorization criteria for coverage. Once drug-specific coverage criteria are met, the requested site is evaluated under this policy.

    Documentation Required

    Document the medical exception

For a nonpreferred site exception, submit documentation of the applicable medical circumstance supporting medical necessity.

    • A previous adverse event that cannot be appropriately managed with premedications and requires a higher level of care.
    • A documented history of comorbidity or instability that precludes treatment at a less intensive site of care.
    • A preferred site that is not accessible to the member together with a home that is unsuitable for home infusion.

    Policy Overview

This policy guides site-of-care utilization for provider-administered specialty infusions after applicable drug-specific coverage and prior-authorization criteria are met. Covered medications may require administration at a designated preferred site of care; other locations are generally considered nonpreferred because they significantly increase the cost of care compared with available alternatives. A nonpreferred setting may still be medically necessary when treatment initiation, clinical risk, patient instability, access limitations, or an unsuitable home for home infusion supports a documented exception.

    Site-of-Care Definitions

    Preferred site of care
DefinitionA designated location identified for administration of covered provider-administered medications.
When requirements applyPreferred-site requirements apply after drug-specific coverage criteria are met.
Policy purposeThe preferred site is used to avoid the substantially higher cost associated with nonpreferred locations.
    Nonpreferred site of care
DefinitionA site of care other than the designated preferred location.
General coverage statusA nonpreferred site is generally considered medically unnecessary because it significantly increases the cost of care versus available alternatives.

    Initial Infusion Period

    Initial infusion period

A nonpreferred site may be medically necessary during initiation of a brand-new infusion medication.

New infusion initiation: The medication is a brand-new infusion medication and administration is within the first 60 days.first 60 days

    Subsequent Infusion Doses

    Subsequent infusion doses

After initiation of a brand-new infusion medication, subsequent doses are subject to preferred-site requirements.

Continuation site requirement: Subsequent doses must be administered at a preferred site of care unless additional documentation supports a medical exception for a nonpreferred site.after the first 60 days

    Site-of-Care Selection

    Prior Authorization

    Use the designated preferred site

Covered provider-administered medications may require administration at a designated preferred site of care. A nonpreferred site is generally considered medically unnecessary unless a documented exception applies.

    Documentation Required

    Home-infusion access exception

A nonpreferred site may be medically necessary when a preferred site of care is not accessible to the member and the member’s home is not suitable for home infusion.

    • Both access limitations must be documented: the preferred site is inaccessible, and the member’s home is unsuitable for home infusion.
