    Coverage and Medical Necessity Criteria

inv-01: General coverage criteria for manual wheelchairs

Covered when ALL of the following are met and additionally F or G is met:

General criteria A-E: A. The member has a mobility limitation that significantly impairs ability to participate in one or more mobility-related activities of daily living (MRADLs) such as toileting, feeding, dressing, grooming, and bathing in customary locations in the home. A mobility limitation is one that: 1) prevents the member from accomplishing an MRADL entirely; or 2) places the member at reasonably determined heightened risk of morbidity or mortality secondary to attempts to perform an MRADL; or 3) prevents the member from completing an MRADL within a reasonable time frame. B. The member’s mobility limitation cannot be sufficiently resolved by the use of an appropriately fitted cane or walker. C. The member’s home provides adequate access, maneuvering space, and surfaces; a home assessment documenting these conditions is required. D. The manual wheelchair will significantly improve the member’s ability to participate in MRADLs and will be used regularly in the home. E. The member is willing to use the provided manual wheelchair.

    All criteria A-E required

Ability to self-propel or caregiver assistance: F. The member has sufficient upper extremity function and other capabilities to safely self-propel during a typical day (consider strength, endurance, range of motion, coordination, pain, deformity, and presence/absence of extremities). OR G. The member has a caregiver available, willing, and able to provide assistance with propulsion and transfers as needed.

Either F or G must be met

inv-02: Specific manual wheelchair base criteria

Specific device-type criteria (each device type has its own additional requirements):

Transport chair (E1037, E1038, E1039): Transport chairs are covered as an alternative to a standard manual wheelchair when basic coverage criteria A-E and caregiver assistance (G) are met.

    Requires A-E and G

Standard hemi-wheelchair (K0002): Covered when the member requires a lower seat height of 17 to 18 inches because of short stature or to enable the member to place his/her feet on the ground for propulsion.seat height 17-18 inches
Lightweight wheelchair (K0003): Covered when both: 1) the member cannot self-propel in a standard wheelchair in the home; and 2) the member can and does self-propel in a lightweight wheelchair.

    Both conditions required

Equipment that is used primarily for non‑medical purposes is not considered durable medical equipment. In particular, backup chairs are explicitly denied as not reasonable and necessary; payment is made for only one wheelchair at a time. If the manual wheelchair will be used inside the home and the coverage criteria are not met, the device will be denied as not reasonable and necessary.

The policy requires that a custom manual wheelchair base (K0008) meet the same general coverage criteria as other manual wheelchair bases and only be used when the configuration required to address the member’s physical or functional deficits cannot be met using standard bases plus appropriate seating systems, cushions, options, or accessories. Policy history adds that when K0008 is applicable the custom base must include a lifetime warranty on side frames and cross braces.

A custom manual wheelchair is considered not reasonable and necessary when the expected duration of need is less than three months (for example, short‑term post‑operative recovery). In such cases the custom base (and related accessories) will be denied.

    Medical Necessity Rules

inv-26: DME medical necessity criteria (general) — A-E and either F or G logic

Medical necessity for manual wheelchair bases requires meeting the general coverage criteria A–E and either F or G:

A: Mobility limitation significantly impairs MRADLs (prevents accomplishment, increases morbidity/mortality risk, or prevents completion within a reasonable time).
B: Limitation cannot be sufficiently resolved by an appropriately fitted cane or walker.
C: Home provides adequate access, maneuvering space, and surfaces; a home assessment documenting these conditions is required and any issues affecting use in the home must be addressed and documented.

What Providers and Suppliers Must Do

    Prior Authorization

Coverage requires documented criteria and may require prior documentation

Manual wheelchairs for use inside the home (listed HCPCS/K-codes) are covered only when the policy’s general coverage criteria (A–E) and either F or G are documented and met; required documentation must be obtained prior to delivery when ACA 6407 applies.

    • Applies to E1037, E1038, E1039, E1161, K0001–K0009 per policy listing.
    • Documentation demonstrating criteria A–E and F or G must be in the record.
    Note

Prior authorization note — coding & specialty evaluation updates

Policy notes coding updates and that specialty evaluation requirements were added per CMS guidance; the document does not state a universal prior authorization requirement but requires the listed coverage criteria and any ACA 6407 face-to-face documentation when applicable.

    • HCPCS formatting changed from spans to individually listed codes (per CMS).

    Required Documentation and Evaluations

    Documentation Required

Obtain specialty evaluation, face-to-face exam, and document prior to delivery for select bases

For ultra-lightweight, tilt-in-space, and custom bases, obtain and document a specialty evaluation and the required face-to-face exam prior to delivery; ensure RTS/RESNA ATP involvement where specified.

    • LCMP evaluation that documents medical necessity must be on file
    • RTS must employ a RESNA-certified ATP with direct, in-person involvement for applicable bases
    Documentation Required

Home assessment must document access, maneuvering space, surfaces, and obstacle resolution

Home assessment must document adequate access and maneuvering space and identify surfaces and obstacles; document how obstacles were addressed before delivery.

    • Describe access paths, turning/maneuvering clearances, flooring/surface suitability, and remediation steps taken

    FeaturePolicy detailNotes / coding/history
Custom manual wheelchair base (K0008)Covered only when required configuration cannot be met using standard bases plus appropriate seating systems, cushions, options or accessories; custom base must meet criteria for unique individual construction.Policy history: K0008 was added (K0009 deleted). Custom base additions require a lifetime warranty on side frames and cross braces.

    Exclusions and Non-Covered Items

Not covered items include backup chairs and manual wheelchair bases intended for use inside the home when the policy’s coverage criteria are not met. Additionally, custom bases are not covered when the expected duration of need is less than three months. If the base is not covered, related accessories will also be denied.

Billing for construction materials (for example, titanium or carbon) by adding HCPCS code K0108 in addition to the wheelchair base is considered incorrect coding and is not allowed; construction materials are considered part of the manual wheelchair base and should not be separately billed.

No additional explicit not‑covered equipment is listed beyond the denials and coding exclusions described elsewhere in this policy.

    Background and Scope

A manual wheelchair is a manually propelled mobility device typically characterized by a folding cross‑brace frame, large rear wheels with push rims, casters, armrests, footrests, and push handles; most require the user or a caregiver to propel. Coverage in this policy focuses on devices intended to improve participation in mobility‑related activities of daily living (MRADLs) inside the home.

    Key Definitions

inv-25: Mobility-related activities of daily living (MRADLs) definition
DefinitionActivities such as toileting, feeding, dressing, grooming, and bathing in customary locations in the home that require mobility
Requirement for coverageMember must have a mobility limitation that significantly impairs participation in one or more MRADLs
Impairment criteriaImpairment is defined as preventing accomplishment, increasing morbidity/mortality risk, or preventing completion within a reasonable time

    Replacement Rules

inv-31: Replacement policy key-values (placeholder from inventory)
Replacement allowancePayment is made for only one wheelchair at a time; backup chairs are denied
Rental rule exampleK0001: One month's rental covered if member-owned wheelchair is being repaired

    High strength lightweight wheelchair (K0004):
Covered when either: 1) the member self-propels while engaging in frequent activities in the home that cannot be performed in a standard or lightweight wheelchair; OR 2) the member requires seat dimensions not accommodated by other bases and spends at least two hours per day in the wheelchair.
    >=2 hours/day for criterion 2

Rarely reasonable if expected duration < 3 months

Ultra-lightweight wheelchair (K0005): Covered if: 1) the member is a full-time manual wheelchair user; 2) requires individualized fitting/adjustments not accommodated by K0001–K0004; 3) specialty evaluation by a licensed/certified medical professional (LCMP) documents medical necessity; and 4) provided by a Rehabilitative Technology Supplier (RTS) that employs a RESNA-certified ATP with direct, in-person involvement.

LCMP should generally have no financial relationship with supplier; hospital-owned supplier exception applies

Heavy duty and extra heavy duty (K0006, K0007): K0006 is covered if the member weighs more than 250 pounds or has severe spasticity. K0007 is covered if the member weighs more than 300 pounds.>250 lb for K0006; >300 lb for K0007
Tilt in space (E1161): Covered if general coverage criteria are met and: 1) specialty evaluation by an LCMP documents medical necessity; and 2) provided by an RTS employing a RESNA-certified ATP with direct, in-person involvement.

    LCMP requirements as with ultra-lightweight

Custom manual wheelchair base (K0008): Covered if the specific configuration required to address the member’s physical and/or functional deficits cannot be met using standard bases plus appropriate seating systems, cushions, options or accessories such that individual construction of a unique manual wheelchair base is required. If K0008 is used to describe a prefabricated base (even modified), the claim will be denied for incorrect coding.

Billing for construction materials (e.g., titanium, carbon) as a separate code is incorrect coding

Manual wheelchair use will significantly improve participation in MRADLs and will be used regularly in the home.
E: Member is willing to use the provided manual wheelchair.
F or G: F: Member can safely self-propel during a typical day (consider strength, endurance, ROM, coordination, pain, deformity, absence of extremities). OR G: A caregiver is available, willing, and able to provide assistance with propulsion and transfers.

    Either F or G required

inv-27: Referenced necessity criteria — history indicates clinical and home assessment requirements

Policy history and documentation requirements referenced by this policy indicate clinical specialty evaluation and home assessment requirements are necessary for coverage decisions:

Specialty evaluation: For ultra-lightweight (K0005), tilt-in-space (E1161), and custom bases (K0008) a specialty evaluation must be performed by a licensed/certified medical professional (e.g., PT, OT, or treating practitioner with specific training and experience in rehabilitation wheelchair evaluations) and must document the medical necessity for the wheelchair and its special features.

LCMP should generally have no financial relationship with the supplier; exception when supplier is hospital-owned

Face-to-face documentation per ACA: Documentation must include the face-to-face exam required by ACA 6407 within six months prior to the written order when applicable, and such documentation must be received prior to delivery.

    Required when ACA 6407 applies

Home assessment: A home assessment documenting adequate access, maneuvering space, and surfaces is required; any issues that would affect use of the wheelchair in the home must be addressed and documented in the home assessment.

Home assessment is required prior to delivery and is part of coverage determination

RTS/ATP involvement: For certain complex bases (e.g., ultra-lightweight, tilt-in-space), the wheelchair must be provided by a Rehabilitative Technology Supplier (RTS) that employs a RESNA-certified Assistive Technology Professional (ATP) with direct, in-person involvement in selection and fitting.

Used when general coverage criteria for manual wheelchairs are met
    inv-10: hemi_seat_height
Seat height range17 to 18 inches
    Applies to HCPCSK0002 (Standard hemi-wheelchair)
IndicationRequired when member needs lower seat height due to short stature or to enable feet to reach the ground for propulsion
    inv-11: ultra_lightweight_full_time_use
RequirementMember must be a full-time manual wheelchair user
Applies to HCPCSK0005 (Ultra-lightweight manual wheelchair)
Additional needRequires individualized fitting/adjustments not met by K0001-K0004 and specialty evaluation documentation
    inv-12: ultra_lightweight_duration_minimum
Minimum expected durationUnlikely reasonable if expected duration of need is less than three months
Applies toCustom and certain specialty manual wheelchairs (policy notes duration threshold for custom bases and high strength/ultra-lightweight contexts)
ExamplePost-operative recovery needs under 3 months would generally not meet medical necessity for custom/manual bases
  1. Specialty evaluation language updated (CMS) and hospital-owned supplier exception noted.
  2. Step Therapy

Conservative options requirement — cane or walker trial

Demonstrate that the member’s mobility limitation cannot be sufficiently resolved by an appropriately fitted cane or walker before manual wheelchair coverage is approved.

    • Criterion B: limitation cannot be sufficiently resolved by an appropriately fitted cane or walker.
    Denial Risk

Denial risk if device-specific criteria unmet

If specific device-type criteria for the requested wheelchair are not met, the wheelchair (and related accessories) will be denied as not reasonable and necessary.

    • Denial applies to the base and any related accessories when base criteria are unmet.
    Documentation Required

Specialty evaluation and ACA face-to-face documentation

Specialty evaluations required for certain wheelchairs must be performed by a licensed/certified medical professional (PT, OT, or treating practitioner with training/experience in wheelchair evaluations) and include the ACA face-to-face exam when applicable.

    • LCMP must document medical necessity and have no financial relationship with supplier (exception for hospital-owned suppliers).
    • Face-to-face examination must occur within six months preceding the written order when ACA 6407 applies and documentation must be received prior to delivery for certain DME.
    Documentation Required

Home assessment — document access, maneuvering space, surfaces

Document a home assessment showing adequate home access, maneuvering space, and surfaces; any obstacles or issues affecting use in the home must be addressed and recorded.

    • Added Dec 5, 2024: any issues to the use of the wheelchair in the home must be addressed and documented in the home assessment.
    Documentation Required

Documentation and evaluator requirements — CMS revisions

Per CMS updates, the policy language was revised to allow the treating practitioner to perform the specialty evaluation; HCPCS codes are listed individually rather than as spans—ensure documentation names the treating practitioner and cites individual HCPCS codes as shown in the policy.

    • Documentation should reflect the treating practitioner (vs. physician) per CMS wording.
    • Use individually listed HCPCS codes in documentation consistent with the policy format.
    Documentation Required

Home assessment — document home appropriateness and community use allowances

Document whether the DME is appropriate for home use; note that for certain programs (e.g., MassHealth/HNE BeHealthy) DME appropriate for the home may also be used in the community — include this in the home assessment record when applicable.

    • Home assessment must document criteria allowing use in the home; specify community use when program-specific allowances apply.
    Denial Risk

Denial for unmet specific device criteria (examples)

If the requested manual wheelchair does not meet the device-specific criteria in the policy (for example, seat height for K0002; weight thresholds for K0006/K0007; specialty-evaluation or RTS/ATP involvement for K0005/E1161), deny as not reasonable and necessary.

    • K0002 requires 17–18 inch seat height for hemi-wheelchairs.
    • K0006 required when member weighs >250 lb; K0007 when >300 lb.
    • K0005 and E1161 require documented specialty evaluation and RTS with RESNA-certified ATP involvement.
    Billing Rule

Single wheelchair limitation — no backup chairs

Payment is allowed for only one wheelchair at a time; backup or duplicate chairs are denied as not reasonable and necessary.

    • One month’s rental for K0001 is allowed if member-owned wheelchair is being repaired, but a second/backup chair is not payable.
    Note

ACA applicability — face-to-face and documentation requirements

Items subject to this policy may also be subject to Affordable Care Act (ACA) 6407 requirements — ensure ACA face-to-face exam and required documentation are completed and received prior to delivery when applicable.

    • ACA 6407 requires a face-to-face exam within six months preceding the written order; documentation must describe the medical condition for which DME is prescribed.
    Documentation Required

Document specialty evaluation per CMS; treating practitioner and hospital-owned supplier exceptions

Per CMS updates, document specialty evaluations consistent with policy: a treating practitioner (not just 'physician') may perform evaluations; hospital-owned supplier exception permits hospital PT/OT/practitioner to perform the specialty evaluation.

    • If supplier is hospital-owned, PT/OT/practitioner in hospital may perform the evaluation
    • Record evaluator role as 'treating practitioner' when applicable
    Documentation Required

Apply CMS documentation and HCPCS-formatting changes (evaluator and coding)

Per CMS edits, use 'treating practitioner' terminology and individually listed HCPCS codes in documentation and ensure specialty evaluator qualifications are recorded.

    • Reflect CMS wording changes in all documentation and claims submissions
