Requirements for Disclosure Statements filed with the Department on or after the effective date:

Disclosure Statements filed, amended, or refiled on or after April 24, 2026 must comply with the Handbook.

Disclosure Statements submitted after the effective date that do not conform to the Handbook may be returned to the provider for correction or revision.

Providers are strongly encouraged to review the Handbook in full prior to submitting a Disclosure Statement.

Disclosure Statements must include all required disclosure sections and appendices using the standardized section titles set forth in the Handbook, and must include required tables and statutory financial ratio disclosures including the Financial Snapshot formatted per Handbook specifications.

Formatting, captioning, document structure, and accessibility requirements in the Handbook must be followed to ensure clear, resident-facing presentation.

The legal entity responsible for providing continuing care must be clearly and affirmatively identified; use of trade names or 'doing business as' designations must not obscure the legal provider identity.
