OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Outpatient Observation Coverage Criteria

Outpatient hospital observation services are covered only when they are reasonable and necessary and Medicare coverage criteria are met. All applicable criteria below must be satisfied.

    ALL of the following

    • The outpatient observation services are reasonable and necessary for the patient's condition and the reason for observation is documented in the medical record.
    • Observation care is initiated and ended in accordance with a physician's order, with the start and end times documented in the patient's medical record.
    • The patient remains under the care of a physician throughout observation; outpatient registration, discharge, and progress notes are timed, written, and signed by the physician.

Physician involvement and supporting documentation must be present in the medical record.

    • Observation is provided on an hourly basis for a minimum of 8 hours.>= 8 hours

The number of units reported must equal or exceed 8. Observation services of less than 8 hours are not eligible for Medicare reimbursement and should be billed with the appropriate E/M level.

    • All hours in the entire observation period are reported, including hours spanning more than one calendar day; the date of service reported is the date observation care begins.
    • Observation services are not billed for routine care or concurrently with diagnostic or therapeutic services or outpatient surgery/procedures.

If a procedure interrupts observation services, the hospital determines the most appropriate way to account for the observation time.

    • No procedure with a 'T' status indicator is reported on the same day as, or the day before, observation care is provided.

A T-status procedure on the same day or the preceding day is incompatible with reporting the observation service.

    • The observation stay does not exceed 72 hours.<= 72 hours

Claims billed with observation exceeding 72 hours will be denied.

    • Duration context

      • In the majority of cases, the decision to discharge the patient after resolution of the reason for observation or to admit the patient as an inpatient can be made in less than 24 hours.< 24 hours
      • In rare and exceptional cases, a reasonable and necessary outpatient observation stay may exceed 48 hours, but the decision to discharge or admit can generally be made in less than 48 hours and must not exceed 72 hours.> 48 hours only rarely and exceptionally; <= 72 hours
