    Coverage Criteria

    All Indications

Covered when ALL of the following criteria are met:

    ALL of the following

    • Documentation that the member has an infection caused by, or strongly suspected to be caused by, a pathogen and site of infection within the FDA label or compendia-supported indications.
    • The prescriber attests that they have reviewed the member's medication profile and that the member is not concurrently taking a monoamine oxidase (MAO) inhibitor (e.g., phenelzine or isocarboxazid), selective serotonin reuptake inhibitor (SSRI), or selective norepinephrine reuptake inhibitor (SNRI), or that the member will discontinue the concurrent interacting medication and be monitored.
    • For IV linezolid requests, documentation that the member is converting from IV linezolid to oral linezolid, or documentation of medical necessity for continued IV therapy.

    Continuation of Therapy

For indications with compendia-approved treatment durations longer than 4 weeks, continuation of therapy is covered when ALL of the following are met:

    ALL of the following

    • Documentation that the member is not experiencing hematologic or neurotoxic adverse effects.
    • The prescriber attests that the member is being monitored for myelosuppression and changes in vision.

    Prescriber and Dosing Requirements

The following requirements apply to every request:

    ALL of the following

    • Linezolid is prescribed by or in consultation with an infectious disease specialist.

If prescribed in consultation, consultation notes must be submitted with the initial request and reauthorization requests.

    • Dosage, frequency, and total treatment duration are supported by FDA labeling or compendia-supported dosing for the prescribed indication.

    FDA-Approved Uses

FDA-approved uses include infections caused by susceptible Gram-positive bacteria in adults and children:

ONE of the following FDA-approved uses

    • Nosocomial pneumonia caused by Staphylococcus aureus, including methicillin-susceptible and methicillin-resistant isolates, or Streptococcus pneumoniae.
    • Community-acquired pneumonia caused by Streptococcus pneumoniae, including cases with concurrent bacteremia, or Staphylococcus aureus (methicillin-susceptible isolates only).
    • Complicated skin and skin structure infections, including diabetic foot infections without concomitant osteomyelitis, caused by Staphylococcus aureus (methicillin-susceptible or methicillin-resistant isolates), Streptococcus pyogenes, or Streptococcus agalactiae. Linezolid has not been studied for decubitus ulcers.
    • Uncomplicated skin and skin structure infections caused by Staphylococcus aureus (methicillin-susceptible isolates only) or Streptococcus pyogenes.

    Compendial Approved Off-Labeled Uses

    Compendia-supported off-label uses include:

ONE of the following compendia-supported uses

    • Anthrax, systemic infection.
    • Health care-associated central nervous system infection, including cerebrospinal fluid shunt infection.
    • Moderate to severe acute pulmonary exacerbation in cystic fibrosis.
    • Endocarditis treatment.
    • Intracranial abscess, including brain abscess and intracranial epidural abscess, or spinal epidural abscess.

To reduce the development of drug-resistant bacteria and maintain the effectiveness of linezolid and other antibacterial drugs, linezolid should be used only to treat or prevent infections that are proven or strongly suspected to be caused by bacteria.

Linezolid is not indicated for the treatment of Gram-negative infections.

Gram-negative infections are not an indicated use of Zyvox.

Known hypersensitivity to linezolid or any other product component is a contraindication to coverage.

Use of linezolid with a monoamine oxidase inhibitor (MAOI), or within two weeks of taking an MAOI, is contraindicated. All other uses of linezolid are considered experimental or investigational and are subject to Molina's Off-Label policy.

Vancomycin-resistant Enterococcus faecium infections, including cases with concurrent bacteremia.
  1. The safety and efficacy of linezolid formulations used for longer than 28 days have not been evaluated in controlled clinical trials.
  2. Bacterial meningitis.
  3. Osteomyelitis and/or discitis.
  4. Prosthetic joint infection.
  5. Septic arthritis.
  6. Toxic shock syndrome.
  7. Drug-resistant tuberculosis.
    • Linezolid is not preferred for infections requiring prolonged therapy because serious hematologic and neurologic toxicity risks increase with longer treatment.
    Documentation Required

    Dose and Duration Validation

The requested linezolid dosage, frequency, and total treatment duration must be supported by FDA labeling or compendia-supported dosing for the prescribed indication.

    • Validate dose, frequency, and total duration against the indication-specific dosing reference.
    • Requests that exceed supported dosing or duration require additional medical rationale and documentation.

    Unsupported block type: documentation

    Documentation Required

    Indication and Safety Documentation

Document the infection diagnosis and site and establish that the requested use is supported by the FDA label or compendia. The documented indication must be consistent with a susceptible pathogen and an approved or supported linezolid use.

    • Include the clinical diagnosis, infection site, and pathogen information.
    • Supported diagnoses include the listed pneumonia, skin and skin structure, and vancomycin-resistant Enterococcus faecium infections, as well as other compendia-supported indications identified in the policy.
    Documentation Required

    IV Therapy Documentation

For an IV linezolid request, document that the member is converting from IV linezolid to oral linezolid or provide medical necessity for continued IV treatment.

    • Explain why oral conversion is not appropriate when continued IV therapy is requested.
    Documentation Required

    Continuation Monitoring Documentation

For indications with compendia-approved treatment durations longer than 4 weeks, document that the member is not experiencing hematologic or neurotoxic adverse effects and that monitoring is ongoing.

    • Prescriber attestation of monitoring for myelosuppression.

    Documentation Required

    Specialist Consultation Documentation

Linezolid must be prescribed by, or in consultation with, an infectious disease specialist. When prescribed in consultation, submit the consultation notes with the initial request and each reauthorization request.

    Documentation Required

    Infection and Organism Documentation

Document a proven or strongly suspected bacterial infection caused by a susceptible Gram-positive organism and identify the infection site and supporting clinical information.

    • Use linezolid only when the infection is proven or strongly suspected to be bacterial.
    • Include culture, susceptibility, or other available test results supporting the organism and treatment selection.
    Denial Risk

    Insufficient Supporting Documentation

Molina may deny reimbursement or take additional appropriate action when the submitted documentation does not support medical necessity, non-investigational use, benefit coverage, or the appropriateness of the billing or treatment practice.

    • Provide adequate records, test results, provider credentials, and member-specific clinical information.
    • Additional documentation may be requested before or after the initial determination.
    Denial Risk

    Unresolved Interacting Medication

An unresolved concurrent MAO inhibitor, SSRI, or SNRI may make the request unsupported. The prescriber must document discontinuation of the interacting medication or the required resolution and monitoring plan.

    • MAO inhibitors include agents such as phenelzine and isocarboxazid.
    • Document the medication review and the plan for any concurrent interacting therapy.
    Denial Risk

    Unjustified Continued IV Therapy

Continued IV linezolid is unsupported unless the request documents conversion to oral linezolid or establishes medical necessity for ongoing IV treatment.

    • Explain the clinical reason oral therapy is not suitable or feasible.
    • Do not rely on IV administration alone as justification for continued IV treatment.
    Denial Risk

    Toxicity or Inadequate Monitoring

Linezolid use may be unsupported when prolonged treatment is associated with toxicity or when required monitoring is absent. For treatment beyond 4 weeks under an applicable compendia-supported indication, document absence of hematologic and neurotoxic adverse effects and monitoring for myelosuppression and vision changes.

    • Address hematologic toxicity and myelosuppression monitoring.
    • Address neurotoxicity and changes in vision.
    • Requests lacking required monitoring or showing relevant toxicity may require additional review or may be denied.
    Denial Risk

    Unsupported Pathogen or Infection

Linezolid is limited to supported bacterial infections caused by susceptible Gram-positive organisms. Requests for unsupported pathogens or infections, including Gram-negative infections, may be denied.

    • Zyvox is not indicated for Gram-negative infections.
    • Confirm that the organism and infection site are consistent with FDA-labeled or compendia-supported use.
    Denial Risk

    Unsupported Infection Type

Nonbacterial infections and Gram-negative infections are not indicated uses of linezolid and are not supported under the FDA-labeled limitations of use.

    • Linezolid should be used only for infections proven or strongly suspected to be caused by bacteria.
    • Document susceptibility when available.
    Denial Risk

    Unlisted Use

Uses of linezolid other than those supported by the FDA label or applicable compendia are considered experimental or investigational and follow Molina's Off-Label policy.

    • Submit the applicable off-label rationale and documentation under Molina's Off-Label policy when requesting an unlisted use.
    Denial Risk

    Contraindication

Linezolid is contraindicated in members with known hypersensitivity to linezolid or any product component and in members taking an MAO inhibitor or who have taken an MAO inhibitor within the previous 2 weeks.

    • Screen and document hypersensitivity history.
    • Confirm that the member is not currently taking an MAO inhibitor and has not taken one within 2 weeks.
    Denial Risk

    Extended Treatment Duration

The safety and efficacy of linezolid formulations used for longer than 28 days have not been evaluated in controlled clinical trials. Requests for extended treatment require careful justification, supported indication, duration validation, and safety monitoring.

    • Initial authorization is limited to 28 days; continuation may be authorized for up to 2 months when criteria are met.
    • Address the increased risk of serious hematologic toxicity after more than 2 weeks and neurologic toxicity after more than 4 weeks.
Continuation documentationDocumentation must show no hematologic or neurotoxic adverse effects and prescriber monitoring for myelosuppression and changes in vision.
