Prior authorization is required for requests for the gemcitabine intravesical system (HCPCS J9183). Providers must submit supporting clinical documentation demonstrating the member meets all approval criteria.

    • Submit office chart notes, lab results, pathology reports, imaging, and any prior treatment records (including BCG therapy history and documentation of BCG-unresponsive or BCG-refractory status)
    • Requests lacking required documentation or evidence that the member meets all approval criteria may be denied
    • Prescriptions should be by or in consultation with an oncologist or urologist
    Documentation Required

    Required Documentation

Providers must include office notes, lab results, procedure reports (e.g., TURBT), and any other clinical information necessary to show the member meets the policy's approval criteria. Failure to provide adequate documentation may result in denial of the request.

    Background on Inlexzo and Indication

Inlexzo (gemcitabine intravesical system) is an intravesical antineoplastic product intended for treatment of adult patients with BCG‑unresponsive non‑muscle invasive bladder cancer (NMIBC) with carcinoma in situ, with or without papillary tumors. This clinical policy provides guidance on medical necessity, dosing, and documentation to support coverage decisions and prior authorization for the product; it does not constitute medical advice or a guarantee of payment.
