OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Indication-specific dosing and response-based continuation

Covered when dosing regimens and maintenance/response-based continuation per indication are followed.

Prurigo Nodularis Dosing: Initial: Adult patients weighing < 90 kg: 60 mg SC initially, followed by 30 mg SC every 4 weeks; Adult patients weighing ≥ 90 kg: 60 mg SC initially, followed by 60 mg SC every 4 weeks.weight-based (90 kg)

    Maintenance doses: <90 kg = 30 mg every 4 weeks; ≥90 kg = 60 mg every 4 weeks.

Atopic Dermatitis Dosing: Initial dosing: 60 mg SC once, followed by 30 mg SC every 4 weeks.response assessment at 16 weeks

After 16 weeks, for patients who achieve clear or almost clear skin, recommend 30 mg SC every 8 weeks. Use with topical corticosteroids and/or topical calcineurin inhibitors; discontinue topical therapies when disease has sufficiently improved. Maximum/maintenance dose: initial 60 mg; maintenance 30 mg every 4 weeks (with option to extend to every 8 weeks after 16 weeks for responders).

Contraindication: Patients with a known hypersensitivity to nemolizumab-ilto or any of its excipients should not receive nemolizumab-ilto. This contraindication reflects the product safety information and is categorized under the policy's contraindications/boxed warnings.

Coding references in this clinical policy are provided for informational purposes only. Inclusion or exclusion of any codes does not guarantee coverage; coverage determinations remain subject to the member's benefit plan, applicable coverage rules, and any required prior authorization. Providers should reference current professional coding guidance when submitting claims.

    Provider Actions and Authorization Requirements

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for therapy prior to initiation. Providers must submit PA requests using the appropriate HCPCS/NDC or unclassified codes and confirm coding with current professional guidance prior to claim submission.

    • Prior authorization required before drug initiation
    • Verify and use current coding guidance when billing (J3590, C9399 as informational)
    Step Therapy

Redirection to Preferred Agents for Atopic Dermatitis

For patients with atopic dermatitis, initial approval requests will be redirected to preferred agents. When submitting for initial therapy the prescriber should consider Dupixent (dupilumab) or Rinvoq (upadacitinib) per payer preferred-agent policy; authorization for the agent requested may be denied or redirected at the payer's discretion.

    • Atopic dermatitis — initial therapy: prescribers redirected to consider Dupixent or Rinvoq
    • Requests for other agents for initial AD therapy may be routed to step/preferred-agent requirements or denied

    Background

Nemolizumab-ilto is a biologic therapy studied and included in this policy for pruritic skin disorders including prurigo nodularis and atopic dermatitis. Dosing differs by indication and patient weight: for prurigo nodularis adults weighing <90 kg, the regimen is an initial 60 mg subcutaneous dose followed by 30 mg every 4 weeks; for adults weighing ≥90 kg, the regimen is an initial 60 mg followed by 60 mg every 4 weeks. For atopic dermatitis the recommended initial dosing is 60 mg subcutaneously once, followed by 30 mg every 4 weeks with response assessment at 16 weeks and consideration of extending to 30 mg every 8 weeks for patients who achieve clear or almost clear skin. Patient-reported itch measures such as the Peak Pruritus Numerical Rating Scale (PP-NRS)/Worst Itch NRS (WI-NRS) scored 0–10 are used for baseline assessment and treatment response monitoring.

    Documentation Required

Required Clinical Documentation (Baseline and Follow-up)

Prior authorization submissions must include baseline and follow-up documentation of itch severity using validated scales and related clinical information to support medical necessity.

    • Baseline PP-NRS or WI-NRS score (0–10) at therapy initiation
    • Follow-up PP-NRS or WI-NRS scores as requested for continuation review (document change from baseline)
    • Relevant diagnosis, prior therapies tried and responses, weight (for dosing), and concomitant topical therapies for atopic dermatitis
    • If applicable, documentation of rationale when not using preferred agents (Dupixent or Rinvoq) for initial AD therapy
    Documentation Required

    Operational and Dosing Documentation Notes

Operational note: Before submitting an authorization, confirm dosing based on patient weight per dosing guidance and include dosing rationale and treatment plan. For atopic dermatitis, note recommended concomitant topical therapy use and reassess dosing frequency after 16 weeks if appropriate.

    • Include patient weight to determine recommended dosing (<90 kg vs ≥90 kg)
    • For atopic dermatitis, document use of topical corticosteroids or calcineurin inhibitors and plan to reassess at ~16 weeks (possible change to 30 mg every 8 weeks if clear/almost clear)
    • Failure to provide required documentation may delay or result in denial of authorization
