Overview: This policy (HNCA.CP.MP.61) defines medical necessity criteria for intravenous moderate sedation and deep sedation/general anesthesia for dental procedures in inpatient, ambulatory surgery center, community clinic or dental office settings. Medically Necessary Indications for IV Moderate Sedation and Deep Sedation/General Anesthesia

Considered medically necessary when ALL of the following are met:

ALL of the following

Behavior modification and local anesthesia must generally be attempted first, but may not be required in certain situations depending on the medical needs of the member.
Minimal sedation must be considered (or determined not feasible) prior to IV moderate sedation or deep sedation/general anesthesia, but may not always be required depending on the medical needs of the member.
Documentation required (both 1 and 2)
- Use of local anesthesia to control pain failed or was not feasible based on the medical needs of the member.
- Use of minimal sedation, either inhalation or oral, failed or was not feasible based on the medical needs of the member.
If any ONE of the following (3 through 6) is documented, member must be considered for IV moderate sedation or deep sedation/general anesthetic
- Use of effective communicative techniques and the inability for immobilization (member may be dangerous to self or staff) failed or was not feasible based on the medical needs of the member.
- Member requires extensive dental restorative or surgical treatment that cannot be rendered under local anesthesia or minimal sedation.
- Member has acute situational anxiety due to immature cognitive functioning.
- Member is uncooperative due to certain physical or mental compromising conditions.
Members with certain medical conditions (e.g., moderate to severe asthma, reactive airway disease, congestive heart failure, cardiac arrhythmias, significant bleeding disorders, uncontrolled seizures, sleep-disordered breathing) should be treated in a hospital setting or licensed facility capable of responding to a serious medical crisis, as determined by the provider.
Providers must adhere to all regulatory requirements for preoperative and perioperative care, monitoring and equipment requirements, emergencies and transfers, and monitoring guidelines.

All locations that administer general anesthesia must be equipped with anesthesia emergency drugs, appropriate resuscitation equipment, and properly trained staff able to respond to anesthetic emergencies. Providers must adhere to federal, state, and licensing board regulatory requirements for preoperative/perioperative care, monitoring and equipment, emergencies and transfers, and monitoring guidelines.

Background

Background and scope: This policy provides medical necessity guidelines for selecting the appropriate depth of sedation/anesthesia for dental procedures, ranking techniques from minimal sedation (inhalation or oral), non-intravenous conscious sedation, IV moderate sedation, to deep sedation/general anesthesia. Selection should be individualized using the enrollee’s medical history, physical status, and indications for anesthetic management, with dental and anesthesia providers collaborating on whether criteria are met.

Patient subset and consequences: General anesthesia is necessary only for a small subset of patients — those who may be cognitively immature, highly anxious or fearful, have special needs, or are medically compromised and unable to receive treatment in a traditional office setting. Withholding general anesthesia for these patients can reduce access to quality oral health care, lead to long-term consequences including increased avoidance of dental care and more emergency department utilization, and may decrease diagnostic yield and procedural quality.

Provider, personnel and facility requirements: Providers administering or arranging general anesthesia must be qualified and appropriately trained per state regulations and professional guidelines. All locations that administer general anesthesia (e.g., acute care inpatient facilities and ambulatory surgery centers) must be equipped with anesthesia emergency drugs, appropriate resuscitation equipment, and properly trained staff and must adhere to regulatory requirements for preoperative/perioperative care, monitoring, emergencies, transfers and monitoring guidelines.
