OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Back Brace Coverage Requirements

    Initial Back Brace Authorization

Nurse utilization management staff may authorize a back brace when ALL of the following requirements are met:

    ALL of the following

    • The cost of the brace is $1,200 or less.
    • ALL of the following

      • The request is for services with an in-network provider or in-network DME vendor.
      • The requested back brace is on the Maryland Medicaid Fee Schedule.
      • The requested orthoses are rigid or semi-rigid.
      • The request is signed by a clinician who has evaluated the member for the back-brace indication within the past 90 days.
        • ONE of the following indications

          • To reduce back pain by restricting mobility of the trunk.
          • To facilitate healing following an injury to the spine or related soft tissues.
          • To facilitate healing following a surgical procedure on the spine or related soft tissue.
          • To otherwise support weak spinal muscles and/or a deformed spine.

Orthoses not listed on the Maryland Medicaid Fee Schedule are excluded from coverage.

Elastic or other fabric support garments are excluded from coverage.

    Authorization and Documentation Requirements

    Prior Authorization

Meet the $1,200 threshold or request medical exception review

Nurse utilization management staff may authorize payment only when the brace costs $1,200 or less and all coverage criteria are met. Requests that do not specifically meet the criteria may be submitted with supporting medical records and literature for Medical Director review as a Medical Exception.

    Documentation Required

    Document the recent clinical evaluation

Submit a request signed by a clinician who evaluated the member for the back-brace indication within the past 90 days. Provide medical records documenting at least one qualifying indication within the past 90 days.

    Policy Scope and Clinical Context

This policy addresses rigid or semi-rigid back orthoses designed to restrict trunk mobility and reduce back pain, facilitate healing after an injury or surgical procedure involving the spine or related soft tissues, or support weak spinal muscles and/or a deformed spine. Coverage requires documentation of a qualifying indication and a clinician evaluation within the past 90 days.

    Denial Risk

    Avoid authorization deficiencies

Requests may be denied when the brace costs more than $1,200, the provider or DME vendor is out of network, or the orthosis is not rigid or semi-rigid. Denial may also result when the request does not meet the fee-schedule, evaluation, signature, or qualifying-indication requirements.

    Denial Risk

Verify the orthosis is not excluded

Orthoses not on the Maryland Medicaid Fee Schedule are excluded. Elastic or other fabric support garments are also excluded.
