    Coverage Criteria — Kidney Transplant

    Kidney transplant medical necessity

Covered when ALL of the following are met:

Primary coverage criteria: Member has completed an evaluation and been accepted by the kidney transplant committee at the kidney transplantation center.

Transplant evaluation results (acceptance) distinguish evaluation from transplantation request and may substitute for individual items when the transplant center has its own selection criteria.

Eligibility and selection: Member meets the transplanting institution's protocol eligibility criteria regarding age.

If the transplanting institution has formal selection criteria, those criteria govern eligibility determinations.

Malignancy status: Absence of active malignancy except non-melanomatous skin cancers or certain low‑risk cancers, OR malignancy has received curative therapy OR estimated risk of recurrence is <10% within the next 2 years (examples provided in policy: renal cell carcinoma post-nephrectomy with 2 years disease-free, prostate cancer with undetectable PSA after treatment, surgically treated colon cancer, thyroid cancer with normal thyroglobulin). Women should have a negative Pap smear within past 3 years and mammography where indicated within past 2 years.

Policy lists illustrative examples and surveillance intervals for common cancers.

Infection and HIV: Absence of systemic infection.
HIV-specific requirements (if applicable): If the member is HIV-positive, ALL of the following must be met: CD4 count >200 cells/mm3 for >6 months; HIV-1 RNA undetectable; on stable antiretroviral therapy >3 months; and no AIDS-related complications (e.g., opportunistic infections, Kaposi sarcoma, resistant fungal infections, tuberculosis).all required

These HIV control criteria must be documented for HIV-positive candidates.

Organ-specific physiologic risk: Attending physician determines there is no prohibitive cardiovascular, pulmonary, or hepatic risk to proceed with transplantation.

Physician assessment of organ‑specific operative risk is required.

Severity of renal disease (one required)

    • On dialysis: Member is already on hemodialysis or continuous ambulatory peritoneal dialysis (CAPD).
    • Anticipated progression to ESRD (precert for cadaveric): Member has chronic renal failure with anticipated deterioration to end stage renal disease and is seeking precertification for cadaveric kidney transplantation.

Given cadaveric waiting times, severe chronic renal failure (creatinine clearance <30 ml/min) with anticipated progression qualifies for consideration.

    • ESRD for living donor precertification: Member has end stage renal disease evidenced by creatinine clearance <20 ml/min or development of uremic symptoms and is seeking precertification for living donor kidney transplantation.creatinine clearance <20 ml/min for ESRD precertification

Severe chronic renal failure is defined as creatinine clearance <30 ml/min; specific living donor precertification threshold is <20 ml/min or uremic symptoms.

Overall requirement: All of the above criteria must be met for kidney transplantation to be considered medically necessary when the transplanting institution does not have its own selection criteria.all conditions required

If the transplanting institution has selection criteria, acceptance by that center may satisfy some items above.

Transplantation is not medically necessary for members who do not meet the transplanting institution’s protocol selection criteria or who have specified disqualifying conditions. Examples of disqualifying conditions include: active vasculitis; age over 70 years with severe comorbidities; life‑threatening extra‑renal congenital abnormalities; ongoing alcohol or drug abuse; severe neurological or mental impairment without adequate social support that precludes adherence to the post‑transplant regimen; and untreated coagulation disorder.

The following therapies, tests, and technologies are explicitly designated experimental and investigational because their effectiveness has not been established and therefore they are not described as covered in this policy: pre‑conditioning therapy for ABO‑incompatible transplantation (e.g., immune‑adsorption or rituximab); urinary biomarkers of acute kidney injury such as NGAL and L‑FABP; FASL mRNA detection; urinary biomarkers for rejection including chemokines (e.g., CXCL9, CXCL10), extracellular vesicles, and MCP‑1/CCL2; donor‑derived cell‑free DNA testing (e.g., Allosure, Prospera); measurement of AT1 receptors or AT1 antibodies; complement inhibitors (e.g., eculizumab); belimumab for antibody‑mediated rejection; genotyping of donors and recipients before transplantation; recombinant human erythropoietin for nephron protection; the TruGraf blood gene expression test; plasminogen activator evaluation as part of hypercoagulable workup; evaluation of urine immunocytology for T cells; pre‑transplant soluble CD30 level measurement; DNA methylation as a biomarker; and artificial intelligence/machine learning methods for predicting graft survival.

Kidney transplantation is not medically necessary when the member fails to meet the transplanting institution’s selection criteria or has one or more of the listed disqualifying conditions. These conditions—such as active vasculitis, untreated coagulation disorders, substance abuse, or lack of adequate social support for regimen adherence—constitute grounds to deny coverage on medical necessity criteria.

The items listed in this section are designated experimental and investigational because the effectiveness of each has not been established for use in renal transplantation. As a result, requests for these tests, biomarkers, pre‑conditioning therapies, or treatments are not supported as standard medical necessity under this policy and may be denied on that basis.

    Kidney Transplant Candidate Criteria

    Kidney transplant candidate criteria

Candidate acceptance and disease severity requirements

Candidate acceptance: Member must be evaluated and accepted by the transplant committee at the transplant center and must meet the transplanting institution's protocol eligibility criteria including age.all required

Center acceptance may substitute for some items if center has its own selection criteria.

Medical suitability and exclusions: Member must have no active systemic infection and must have no prohibitive cardiovascular, pulmonary, or hepatic risk as determined by the attending physician.

Documented absence of disqualifying conditions required for approval.

HIV‑positive candidates: If HIV‑positive, candidate must have CD4 >200 cells/mm3 for >6 months; HIV‑1 RNA undetectable; be on stable antiretroviral therapy for >3 months; and have no AIDS‑related complications (opportunistic infections, Kaposi sarcoma, resistant fungal infections, tuberculosis, or other neoplasms).

HIV control parameters must be met and documented.

Disease severity (one of): Candidate is on hemodialysis or CAPD; OR has chronic renal failure with anticipated progression to ESRD (precertification for cadaveric transplant); OR has ESRD evidenced by creatinine clearance <20 ml/min or uremic symptoms (precertification for living donor transplant).Severe chronic renal failure < 30 ml/min; ESRD for living donor precertification < 20 ml/min

Severity requirement is mandatory for candidacy.

    Contraindications

Absolute and relative contraindications include active vasculitis; age over 70 years with severe comorbidities (which may render a candidate unsuitable depending on overall risk); and life‑threatening extra‑renal congenital abnormalities that compromise survival or transplant benefit.

Other exclusionary conditions noted by the transplant center that preclude medical necessity are ongoing alcohol or drug abuse, severe neurocognitive or psychiatric impairment without adequate social support to ensure adherence, and untreated coagulation disorders; these should be documented in the patient record and in the transplant center evaluation.

    Evaluation and Documentation Requirements

    Documentation Required

Document completed evaluation and transplant committee acceptance

Member must have completed a full evaluation and been formally accepted by the kidney transplant committee at the transplant center. For HIV-positive candidates, document sustained CD4 >200 cells/mm3 for >6 months, undetectable HIV-1 RNA, stable antiretroviral therapy >3 months, and absence of AIDS complications.

    • Completed evaluation and acceptance by the kidney transplant committee at the transplant center (required).
    • HIV-positive candidate requirements: CD4 >200 cells/mm3 for >6 months; undetectable HIV-1 RNA; on stable antiviral therapy >3 months; no AIDS complications.
    Documentation Required

Confirm center evaluation and HIV-specific criteria

Member must have completed the transplant center evaluation and been accepted by the kidney transplant committee; HIV-positive candidates must meet the documented virologic and immunologic control criteria before transplant can be considered.

    • Evidence of committee acceptance after full evaluation is required.
    • HIV control criteria as specified (CD4, viral load, stable therapy, no AIDS complications).
    Documentation Required

Evidence of committee acceptance and HIV candidate documentation

Provider must show the member completed evaluation and was accepted by the kidney transplant committee at the transplant center; HIV-positive candidates must meet the specified control criteria to satisfy this requirement.

    • Acceptance by transplant committee documented in medical record.
    • For HIV-positive members, include CD4 and viral load data and duration of stable therapy.
    Note

Evaluative tests (genotyping, plasminogen activator, AT1) considered investigational

Mentioned evaluative items such as genotyping of donors/recipients, plasminogen activator evaluation, and measurement of AT1 receptors/antibodies are listed as investigational; if submitted, recognize they are not established components of standard pre-transplant evaluation.

    • Genotyping donors and recipients before transplantation is considered experimental/investigational.
    • Plasminogen activator evaluation as part of hypercoagulable workup is considered experimental/investigational.
    • Measurement of AT1 receptors or AT1 antibodies for candidate/recipient evaluation is considered experimental/investigational.
    Documentation Required

Document evaluation completion and center acceptance

Provider must document that the member completed an evaluation and was accepted by the kidney transplant committee at the transplant center and that the member meets the transplanting institution's protocol eligibility criteria (including age).

    • Record of transplant committee acceptance in member chart.
    • Documentation that member meets center protocol eligibility criteria (age and others).

    Provider Actions and Prior Authorization

    Prior Authorization

Prior authorization must include transplant committee acceptance and evaluation

Submit prior authorization with evidence of transplant center committee acceptance and documentation of the supporting evaluation demonstrating absence of disqualifying conditions and that severity criteria are met.

    • Include transplant committee acceptance letter or note.
    • Include evaluation findings showing no uncontrolled malignancy or systemic infection and evidence of dialysis or creatinine clearance thresholds where applicable.
    Prior Authorization

Experimental/investigational items: prior authorization posture and rationale

The listed interventions and tests are designated experimental/investigational; prior authorization requests for these items should reflect the policy stance that effectiveness has not been established and include rationale if requested.

    • Flagged items include pre-conditioning therapies (e.g., immune-adsorption, rituximab), biomarkers, donor-derived cell-free DNA tests, complement inhibitors (eculizumab), belimumab, genotyping, TruGraf, and others listed in the policy.
    • PA submissions for these items should acknowledge the experimental/investigational designation and provide supporting rationale if pursuing coverage.
    Step Therapy

No step therapy specified for transplant eligibility

No step therapy sequencing requirements apply in this portion of the policy; providers should not expect mandated therapy sequences for the covered transplant determination here.

    Step Therapy

No step therapy pathways for investigational items

No step therapy pathways are specified for the experimental/investigational therapies and tests listed in the policy; these items are individually designated investigational rather than part of a step sequence.

    Denial Risk

Denial risk when member fails to meet center selection criteria or has disqualifying conditions

Transplantation is not considered medically necessary for persons who do not meet the transplanting institution's protocol selection criteria or who have listed disqualifying conditions; such failures to meet criteria are common triggers for denial.

    • Common disqualifying conditions include active vasculitis; age >70 with severe comorbidities; life‑threatening extra‑renal congenital abnormalities; ongoing substance abuse; severe neurological/mental impairment without social support; and untreated coagulation disorder.
    Denial Risk

Experimental/investigational requests for pre-conditioning therapies, biomarkers, tests may be denied

Requests for the listed pre-conditioning therapies, biomarkers, diagnostic tests, or treatments (e.g., immune-adsorption, rituximab for ABO-incompatible transplant; donor-derived cell-free DNA; AT1 receptor/antibody measurement; complement inhibitors; belimumab; genotyping; TruGraf; plasminogen activator evaluation; urine immunocytology; soluble CD30; DNA methylation; AI/ML methods) are considered experimental/investigational and may be denied on that basis.

    • These items are explicitly designated experimental/investigational because effectiveness has not been established.
    • Coverage/PA for these items may be denied consistent with the policy.
    Documentation Required

UM staff training and documentation responsibility

Medical UM associates are responsible for reading and comprehending this procedure and for contacting management or Privacy and Compliance with any questions; UM staff must follow the documentation and training expectations stated in the policy.

    • UM staff must be familiar with the procedure and contact management or Privacy and Compliance with questions.

    Background

Kidney transplantation is indicated for patients with end‑stage renal disease (ESRD) or severe chronic renal failure with anticipated progression to ESRD. Medical necessity requires completion of the transplant center evaluation and formal acceptance by the kidney transplant committee, adherence to the center's protocol eligibility (including age criteria), absence of active systemic infection, and fulfillment of disease severity thresholds such as being on hemodialysis or CAPD or having a creatinine clearance consistent with severe chronic renal failure.

For precertification purposes, the policy references severity thresholds including severe chronic renal failure defined as creatinine clearance < 30 ml/min and ESRD evidenced by creatinine clearance < 20 ml/min or uremic symptoms for living donor precertification; these clinical findings, along with center acceptance, underpin the medical necessity determination.
