OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Background and Scope

This document summarizes proposed coverage criteria and prior authorization options for multiple specialty agents. It includes vutrisiran (Amvuttra) for the treatment of the polyneuropathy of hereditary transthyretin‑mediated amyloidosis (hATTR/FAP), with specific initial and renewal requirements including vitamin A supplementation and exclusion of combination TTR‑reducing therapies; sodium phenylbutyrate/taurursodiol (Relyvrio) for amyotrophic lateral sclerosis (ALS) with requirements such as an ≥8‑week trial of riluzole and objective baseline assessment (e.g., ALSFRS‑R); eflapegrastim (Rolvedon) for febrile neutropenia prophylaxis with step requirements of trial/failure of preferred agents; and lenacapavir (Sunlenca) for heavily treatment‑experienced HIV‑1 with class‑level preferences and baseline viral load documentation. The report also outlines class‑level preferred agent selection processes whereby agents not chosen as preferred will be treated as non‑preferred and subject to prior authorization.

    • Provide lab report showing baseline HIV‑1 RNA ≥400 copies/mL while on current ARV regimen.
    Denial Risk

Vutrisiran combination therapy exclusion (denial risk)

Use of vutrisiran in combination with other transthyretin (TTR)‑reducing agents (e.g., inotersen, tafamidis, patisiran) is prohibited and conflicts with approval criteria; concurrent use may result in denial.
