Monitor payer policy activity
Defines MassHealth's retention bonus amounts, service commitment requirements, billing codes, documentation, and payment schedule for CSN agencies and qualifying nurses for specified service quarters through June 2024.
This bulletin supersedes and replaces Continuous Skilled Nursing Agency Bulletin 11 and extends the dates of service for which the Retention Bonus program will be accessible through June 2024.

Retention bonus coverage criteria
Covered when ALL of the following are met:

Service-hour thresholds
- Nurse completed 480 CSN service hours during the designated service quarter; retention bonus amount = $3,250.
- Nurse completed 360 CSN service hours during the designated service quarter; retention bonus amount = $2,000.
- Nurse completed 260 CSN service hours during the designated service quarter; retention bonus amount = $1,200.
ALL of the following
- Counted hours must be CSN services provided directly to MassHealth members enrolled in the Community Case Management (CCM) program.
Additional acceptable counted hours
- CSN services provided to MassHealth members enrolled in OneCare or Senior Care Options (SCO).
- CSN services provided to CCM members that are included in their individualized education programs (IEPs).
- CSN hours provided to CCM members that are reimbursed by a commercial healthcare insurance plan.
ALL of the following
- Each claim must be attached to a CCM member whom the qualifying CSN nurse has served and must include a completed Retention Bonus Claim Attachment Form specific to the qualifying nurse and dates of service.
- Claims submitted without the completed attachment will be automatically denied.

- Each unit billed will include a 10% rate add-on above the retention bonus amounts to offset agency administrative costs.

- Agencies must retain records that demonstrate each qualifying nurse met the service commitment requirements and that the full retention bonus amount was paid directly to the qualifying nurse (payments by credit, gift certificate, etc., are not reimbursable).
- Failure to adhere to bulletin requirements may result in sanctions or overpayment recovery in accordance with 130 CMR 450.238 and 130 CMR 450.235.

Billing, Documentation, and Claim Requirements
Submit completed Retention Bonus Claim Attachment Form
A completed Retention Bonus Claim Attachment Form for Continuous Skilled Nursing must be included with each retention bonus claim. The attachment must be completed in its entirety, be specific to the qualifying CSN nurse and the dates of service, and demonstrate the nurse met the service commitment for which the agency is billing. Claims submitted without the required attachment will be automatically denied.

Bill each service quarter in the following calendar month
CSN agencies must submit claims for each service quarter during the calendar month immediately after the applicable service quarter according to the published billing months (for example, April–June 2023 claims are billed in July 2023).
- April 2023–June 2023 → Billing Month: July 2023
- July 2023–September 2023 → Billing Month: October 2023
- October 2023–December 2023 → Billing Month: January 2024
- January 2024–March 2024 → Billing Month: April 2024
- April 2024–June 2024 → Billing Month: July 2024
