    Medical Necessity and Coverage Criteria

    Medically necessary septoplasty

Covered when ANY one of the following septoplasty criteria is met.

    Septoplasty indications

    • Surgical access: An asymptomatic septal deformity prevents access to another intranasal area required to perform a medically necessary procedure, such as an ethmoidectomy.
    • Recurrent sinusitis: Documented recurrent sinusitis is thought to be due to a deviated septum and has not been relieved by appropriate medical and antibiotic therapy. At least 3 episodes over a 12-month period
    • Trauma: Nasal septum trauma resulted in significant functional deformity that was not present before the injury.
    • Post-removal reconstruction: Reconstruction is needed after removal of a tumor, nasal polyps, or part of the ethmoid bone.
    • Sleep apnea and PAP intolerance: The patient has obstructive sleep apnea and difficulty tolerating CPAP because of significant nasal obstruction unresponsive to conservative management, and septoplasty is intended to enhance CPAP or BiPAP effectiveness.
    • Recurrent epistaxis: Recurrent nosebleeds are thought to be related to an underlying septal deformity.
    • Persistent nasal obstruction: Septal deviation causes continuous nasal airway obstruction and difficulty breathing through the nasal passages despite appropriate medical therapy. At least 4 weeks of appropriate medical therapy
    • Cleft repair: Septoplasty is performed in association with cleft lip or palate repair.

    Initial extracorporeal septoplasty

Covered when BOTH the procedure-specific condition and the underlying septoplasty criteria are met.

    • Extremely deviated septum: The septum is extremely deviated and cannot be adequately corrected through an intranasal approach.
    • Underlying septoplasty eligibility: The member meets the septoplasty clinical criteria.

At least one septoplasty indication must be met.

    Limited medically necessary rhinoplasty

Rhinoplasty may be considered medically necessary only under the following limited pathways.

    Rhinoplasty indications

    • Cleft or nasal dermoid deformity: Rhinoplasty corrects a nasal deformity secondary to congenital cleft lip and/or palate or is performed for removal of a nasal dermoid.

Photographic evidence of the anatomical abnormality is required.

    • Vestibular stenosis

      • Not correctable by septoplasty or turbinectomy alone: Airway obstruction will not respond to turbinectomy and septoplasty alone.
      • Expected functional benefit: Functional impairment is expected to be resolved by rhinoplasty.

Cosmetic rhinoplasty and/or septoplasty performed solely to enhance appearance is not covered by MassHealth. Coverage is directed to procedures that address a documented functional problem or reconstructive need; any cosmetic enhancement must be incidental to the medically necessary treatment.

Septoplasty is experimental and investigational for all other indications, including allergic rhinitis, because authoritative evidence is insufficient to establish clinical efficacy. Coverage is therefore limited to the medically necessary indications specifically described in this guideline.

Extracorporeal septoplasty performed as revision of a deviated septum is experimental and investigational because authoritative evidence is insufficient to establish clinical efficacy. This limitation is distinct from initial extracorporeal correction of an extremely deviated septum when the applicable septoplasty criteria are met.

All other rhinoplasty indications are experimental and investigational because authoritative evidence is insufficient to establish clinical efficacy. Examples include repair of nasal valve collapse; absorbable nasal implants such as the Spirox Latera implant; concentrated growth factor for post-rhinoplasty nasal septal mucosal defects; septal swell-body treatment for chronic rhinitis; chronic sinusitis; and nasal pyriform aperture reduction, or pyriform turbinoplasty, for nasal obstruction.

    Prior Authorization and Documentation Requirements

    Prior Authorization

    Prior Authorization Required

MassHealth requires prior authorization (PA) for rhinoplasty and septoplasty. Approval does not guarantee payment; payment remains subject to member eligibility, other insurance, and all applicable MassHealth program conditions.

    Documentation Required

    Performing Surgeon Must Submit

The PA request must be submitted by the MassHealth-enrolled surgeon who will perform the procedure. The request must include clinical documentation supporting medical necessity, including the qualified licensed health professional’s assessment and referral(s) for surgery.

    Unsupported block type: documentation

    Documentation Required

    Clinical Background

Reconstructive rhinoplasty corrects an external nasal deformity, damaged nasal structures, or lost tissue while maintaining or improving physiological nasal function. Reconstructive septoplasty corrects defects or deformities of the nasal septum by altering, splinting, or removing obstructive tissue while preserving or improving nasal function. Septoplasty is primarily used for symptomatic nasal airway obstruction and may also address selected recurrent epistaxis, sinusitis associated with septal deviation, nasal trauma, access to another intranasal region, or cleft palate repair; cosmetic enhancement alone is not covered.

Significant symptoms: Nasal airway obstruction causes significant symptoms, such as difficulty breathing or chronic rhinosinusitis.
  1. Failed conservative management: Obstructive symptoms persist despite conservative management, including nasal steroids or immunotherapy when appropriate. At least 4 weeks
  2. Persistent obstruction: There is persistent and prolonged obstructed nasal breathing.
  3. Photographic deformity evidence: Photographs demonstrate an external nasal deformity.
  4. Physical examination confirms moderate to severe vestibular obstruction.Moderate to severe
  5. Objective obstruction evidence: Significant obstruction of one or both nares is documented by CT, nasal endoscopy, or another appropriate imaging modality. One or both nares
  6. Rhinoplasty with medically necessary septoplasty: Rhinoplasty for nasal airway obstruction is performed as a component of medically necessary septoplasty, and gross nasal obstruction is documented on the same side as the septal deviation.
  7. Severe disease-related deformity: There is severe deformity and difficulty breathing related to inflammatory disease, abscess, osteomyelitis, or removal of nasal malignancy that caused difficulty breathing and severe deformity.
  8. Obstructive symptoms must persist despite conservative management, including nasal steroids or immunotherapy when appropriate
    Vestibular obstruction severity
Required severityModerate to severe vestibular obstruction

    Document the Required History

Document at least one qualifying history element supporting the requested procedure. The record should identify the symptom or condition, its relevant duration and severity where applicable, and its relationship to the proposed surgery.

    • Asymptomatic deformity that prevents surgical access to another intranasal area, such as for an ethmoidectomy
    • Atypical facial pain of nasal origin; a positive response to topical anesthetic when a deformed septum contacts a turbinate supports, but does not prove, a septal cause
    • Frequent nosebleeds
    • Nasal airway obstruction or difficult nasal breathing causing mouth breathing, snoring, sleep apnea, or recurrent sinus infections

Submit the PA request and all supporting documentation electronically through the MassHealth Provider Online Service Center (POSC), unless the provider has a currently approved electronic claims waiver. Providers using an approved waiver must submit the PA-1 Form with all supporting documentation.

    • POSC is the standard submission method
    • For an approved waiver, use the MassHealth Prior Authorization Request (PA-1 Form) and include all supporting documentation
    • POSC access questions: MassHealth Customer Service Center at (800) 841-2900; TDD/TTY: 711
    Denial Risk

    Cosmetic-Only Surgery Is Not Covered

Cosmetic rhinoplasty or septoplasty performed solely to enhance appearance is not covered by MassHealth. Any cosmetic enhancement must be incidental to a covered functional or reconstructive indication.

    Denial Risk

    Unlisted or Revision Septoplasty Indications

MassHealth considers septoplasty experimental and investigational for indications outside the listed criteria, including allergic rhinitis. Extracorporeal septoplasty for revision of a deviated septum is also considered experimental and investigational.

    • Unlisted indications, including allergic rhinitis, lack authoritative evidence of clinical efficacy under this guideline
    • Extracorporeal septoplasty for revision of a deviated septum is experimental and investigational
    Denial Risk

    Unlisted Rhinoplasty Indications

MassHealth considers other rhinoplasty indications experimental and investigational when they fall outside the listed medical-necessity criteria. Requests for these indications may be denied for lack of authoritative evidence establishing clinical efficacy.

    • Repair of nasal valve collapse
    • Absorbable nasal implants, such as the Spirox Latera Absorbable Nasal Implant
    • Concentrated growth factor from blood plasma for repair of a nasal septal mucosal defect following rhinoplasty
    • Septal swell bodies for chronic rhinitis
    • Chronic sinusitis or nasal pyriform aperture reduction (pyriform turbinoplasty) for nasal obstruction
    Denial Risk

    Missing or Inadequate Authorization Documentation

A request may be denied when the required PA is missing or when the submission does not adequately support medical necessity. Approval also remains subject to MassHealth’s general payment conditions.

    • PA must be submitted by the MassHealth-enrolled performing surgeon
    • Include the assessment, referral(s), and all applicable clinical documentation
    • Incomplete or inadequate authorization documentation may prevent approval or payment
    Denial Risk

    Document Functional Obstruction Completely

Failure to document functional obstruction and the required supporting evidence creates denial risk. The record must connect symptoms to the anatomic abnormality and include the examination, photographs, and objective testing required for the specific indication.

    • Document obstructive symptoms, their duration and severity, and response to conservative management
    • Provide complete anterior and posterior nasal examination findings and relevant upper-airway findings
    • Provide standard four-way preoperative photographs when an external nasal deformity is present
    • Provide CT, nasal endoscopy, or other appropriate imaging when required by the applicable criterion
    • For vestibular stenosis, document moderate-to-severe obstruction on examination and significant obstruction of one or both nares
Maintains or improves the physiological function of the nose
    Vestibular stenosis
DefinitionChronic non-septal nasal airway obstruction caused by collapsed internal nasal valves
Underlying causesMay result from a congenital defect, trauma, or disease
Coverage relationshipAddressed under the guideline’s limited rhinoplasty pathway and considered only upon individual case review when all specified criteria are met
    Extremely deviated nasal septum
DefinitionA septum that is extremely deviated and cannot be adequately corrected through an intranasal approach
Procedure contextExtracorporeal septoplasty may be medically necessary for initial correction
Eligibility requirementThe member must also meet the guideline’s septoplasty criteria
