    POLICY UPDATE CHANGES

Policy establishes coverage criteria for the intrathecal formulation Itvisma (onasemnogene abeparvovec-brve) for patients aged 2 years and older with genetically confirmed SMN1 mutation.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Itvisma (Intrathecal Onasemnogene Abeparvovec)

    Initial Approval Criteria

Authorization may be granted when ALL of the following are met:

Initial Approval: Member has a genetically confirmed diagnosis of spinal muscular atrophy (SMA) with mutation in the SMN1 gene; member is at least 2 years of age; member's vaccination schedule will be adjusted to accommodate concomitant corticosteroid administration before and after Itvisma injection; member is clinically stable; member does not have any active infections.all criteria required

Dosing: recommended single dose is 1.2x10^14 vector genomes administered over 1 to 2 minutes.

Requests for Itvisma will not be authorized while a member has an active infection or is clinically unstable. These conditions are explicit exclusions to initial approval and must be resolved prior to authorization; documentation demonstrating infection resolution and clinical stability should be provided with the prior authorization request.

Intrathecal Itvisma criteria apply only to members who are at least 2 years of age. Requests for patients younger than 2 years do not meet the intrathecal Itvisma coverage criteria in this document (the policy does not address intravenous dosing here).

Prior Authorization, Documentation, and Denial Conditions

    Prior Authorization

Prior authorization required — submit via plan-specific, CMS-aligned channels

Prior authorization is required for Itvisma for all plan lines; submit requests through the member’s plan-specific channel consistent with CMS-aligned processes.

    • Medicare Advantage and One Care/SCO: follow CMS guidance and Medicare manuals for medical necessity determinations.
    • Mass General Brigham ACO: submit prior authorization requests to the MassHealth Drug Utilization Review Program (see Table 76).
    • Commercial and Qualified Health Plans: prior authorization is managed by Prime Therapeutics; refer to the Prime Therapeutics policy.
    Step Therapy

    No step therapy requirements

No step therapy requirements are specified for Itvisma in this document.

    Key Definitions

Itvisma (onasemnogene abeparvovec-brve) — intrathecal one-time gene therapy
Product definitionItvisma (onasemnogene abeparvovec-brve) — an AAV vector–based one-time gene replacement therapy delivering a functional SMN1 gene
Route of administrationIntrathecal — fixed-dose delivered directly into cerebrospinal fluid
Indication scopeTreatment of spinal muscular atrophy (SMA) with genetically confirmed SMN1 mutation for patients aged 2 years and older
Dosing formatFixed single dose (recommended 1.2 x 10^14 vector genomes administered over 1–2 minutes)

Background on Spinal Muscular Atrophy and Therapy

Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disease caused by biallelic loss-of-function mutations in the SMN1 gene, which result in deficient SMN protein, progressive motor neuron degeneration, and muscle weakness. Intrathecal onasemnogene abeparvovec (Itvisma) is a one-time gene replacement therapy that delivers a functional SMN1 copy directly to the cerebrospinal fluid to treat eligible patients.

    2025-11regulatory_approval

FDA approved intrathecal onasemnogene abeparvovec (Itvisma) in November 2025, enabling fixed-dose intrathecal administration and expanding access beyond prior weight-based intravenous dosing limits.

    Documentation Required

Required prior authorization pathways and supporting documentation

Include supporting clinical documentation with the prior authorization request per the member’s plan line.

    • Genetic confirmation of SMN1 mutation and patient age (≥ 2 years).
    • Evidence of clinical stability and absence of active infection.
    • Plan for vaccination adjustment around corticosteroid administration.
    • Use the MassHealth Drug Utilization Review Program pathway for ACO members and Prime Therapeutics for Commercial members; Medicare/One Care/SCO follow CMS guidance.
    Denial Risk

Denial risk if initial approval criteria not documented

Requests that do not document that all initial approval criteria are met may be denied.

    • Missing genetic confirmation of an SMN1 mutation.
    • Patient age under 2 years (does not meet intrathecal Itvisma criteria).
    • Active infection or clinical instability at the time of request.
    • No documented plan to adjust vaccination schedule for concomitant corticosteroid use.
