    OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Key Clinical Thresholds and Coding-Related Metrics

    inv-09: Body Surface Area — >= 3% BSA
    Threshold>= 3% body surface area (BSA)
    ContextApplied for determining moderate to severe plaque psoriasis in coverage criteria
    Alternative qualifierInvolvement of crucial body areas (hands, feet, face, neck, scalp, genitals/groin, intertriginous areas) may substitute for BSA threshold
    inv-10: NSAID trial duration — minimum 1-month trial of two NSAIDs at maximally tolerated doses
    Required durationMinimum 1-month trial of two different NSAIDs at maximally tolerated doses
    Examples of NSAIDse.g., ibuprofen, naproxen

    Prior Authorization, Documentation, and Operational Rules

    Prior Authorization

    Pharmacy prior authorization and specialty pharmacy fill required

    Prior authorization is required through the pharmacy benefit and the medication must be filled at a contracted specialty pharmacy.

    Step Therapy

    Document required diagnosis‑specific prior therapy trials

    Diagnosis-specific prior therapy trials must be documented before authorization: for plaque psoriasis a minimum 4-week topical trial (or first-line biologic if severe) plus trial and failure/intolerance/contraindication to two listed systemic/biologic agents; for psoriatic arthritis trial and failure/intolerance/contraindication to two listed agents; for ankylosing spondylitis and nr-axSpA a minimum 1-month trial and failure/intolerance/contraindication to two different NSAIDs at maximally tolerated doses followed by trial and failure/intolerance/contraindication to two listed biologic/other agents; for hidradenitis suppurativa trial and failure/intolerance/contraindication to one listed anti-TNF agent.

    • Plaque psoriasis: minimum 4-week trial of one topical agent (or severe disease qualifying for first-line biologic) AND trial and failure/intolerance/contraindication to TWO of the listed systemic/biologic agents (e.g., Cimzia, Enbrel, Humira/Hadlima/Simlandi/Yuflyma, Otezla/Otezla XR, Skyrizi, Sotyktu, Selarsdi/Steqeyma/Yesintek, Taltz, Tremfya).

    Background on Bimzelx

    Bimzelx (bimekizumab-bkzx) is an interleukin-17A and interleukin-17F antagonist approved for a range of immune-mediated conditions referenced in this policy, including moderate to severe plaque psoriasis, psoriatic arthritis, ankylosing spondylitis, non-radiographic axial spondyloarthritis with objective signs of inflammation, and moderate to severe hidradenitis suppurativa. Authorization criteria in this policy are aligned to diagnosis-specific prior therapy requirements and objective measures of disease activity, and the medication must be obtained via the pharmacy benefit and filled through a contracted specialty pharmacy.
