ABA Pre-treatment Assessment

ABA PRE-TREATMENT ASSESSMENT REQUEST MUST MEET ALL OF THE FOLLOWING :

Pre-treatment criteria: 1. The member has a diagnosis of Autism Spectrum Disorder (ASD) based on criteria used in the current DSM from a licensed clinician (e.g., neurologist, developmental pediatrician, pediatrician, psychiatrist, licensed clinical psychologist or medical doctor experienced in ASD).

Documentation must include a clinical note sufficient to allow concordance with DSM criteria; autism screening tools alone do not constitute a diagnosis.

Diagnostic evaluation requirements: 2. A comprehensive diagnostic evaluation that includes an ASD-specific standardized assessment and rules out behavior/medical diagnoses with similar presentations (e.g., neurological disorders, hearing disorders, other developmental delays).

Screening results alone are insufficient; full diagnostic evaluation required.

Assessment hours and standards: 3. Hours requested for pre-treatment assessment are appropriate and not more than required to complete the assessment.

Initial ABA Treatment Authorization

INITIAL ABA TREATMENT AUTHORIZATION REQUEST MUST MEET ALL OF THE FOLLOWING :

Diagnostic and baseline assessment requirements: 1. Diagnostic criteria from the pre-treatment assessment are met. 2. Documentation of psychological assessment including autism-specific testing, adaptive behavior testing, and cognitive/developmental evaluation with a formal report detailing scores and results. 3. Baseline data completed within five (5) years prior to or scheduled within ninety (90) days of the pre-treatment assessment, consisting of: (a) developmental/cognitive evaluation; (b) autism-specific assessment identifying severity; (c) adaptive behavior assessment completed within six (6) months of treatment start; and (d) neurological evaluation as part of a comprehensive physical exam.

Adaptive behavior assessment must be within 6 months of treatment start.

Treatment planning and scope: 4. Treatment goals and clinical documentation focus on active ASD core symptoms, deficits that inhibit daily functioning, and aberrant behaviors requiring Behavior Analyst expertise; goals include plans for stimulus and response generalization. 5. Continued ABA Treatment Authorization

CONTINUED ABA TREATMENT AUTHORIZATION REQUEST MUST MEET ALL OF THE FOLLOWING :

Continuation criteria: 1. All criteria 1–11 in the Initial ABA Treatment Authorization are met. 2. The provider documents clinical or social benefit from treatment, identifies new or continuing treatment goals, and develops a new or continuing treatment plan based on progress evidenced by behavioral changes and increased skill acquisition.
