    Prior Authorization

    CGM Authorization and Pharmacy Claims

Use the Louisiana Uniform Prescription Drug Prior Authorization Form to request clinical authorization for continuous glucose-monitoring (CGM) devices. Pharmacy claims for a preferred CGM bypass clinical authorization when there is evidence of at least one insulin pharmacy claim within the previous 180 days; CGM quantity limits still apply.

    Note

    Six-Month Approval Period

Authorization for CGM initiation or continuation is issued for 6 months.

    Step Therapy

    Preferred CGM Trial or Exception

For a non-preferred CGM, document at least one qualifying exception: the recipient is continuing use of a non-preferred CGM; has tried one preferred CGM and had an inadequate response or intolerance; has limitations of use with the preferred CGM(s); or uses an insulin pump compatible only with a non-preferred CGM. Device samples, coupons, and discount cards do not count as a trial.

    Policy Background

The Louisiana Medicaid Diabetic Supplies List supports diabetes self-management and insulin administration through glucose monitoring, continuous glucose monitoring, insulin delivery, and related supplies. The list also establishes preferred-product requirements, diagnosis-based quantity edits, and clinical authorization requirements for continuous glucose monitors.

    Documentation Required

    Complete the CGM authorization form

Use the Louisiana Uniform Prescription Drug Prior Authorization Form for CGM clinical authorization. State the qualifying clinical condition and, for a non-preferred CGM, document the applicable continuation, trial or intolerance, limitation, or insulin-pump compatibility rationale.

    Documentation Required

    Confirm follow-up for continuation therapy

For continuation therapy, the prescriber must state that the recipient attended a follow-up visit with a healthcare provider to assess ongoing benefit.

    CGM Form and Clinical Documentation

Submit the Louisiana Uniform Prescription Drug Prior Authorization Form and state the clinical information supporting the request, including the applicable initiation criteria and any required non-preferred CGM documentation.

    Documentation Required

    Continuation Documentation

For continuation of therapy, the prescriber must state that the recipient attended a follow-up visit with a healthcare provider to assess ongoing benefits.

    Billing Rule

    Diagnosis Coding for Pharmacy Claims

Submit an appropriate diagnosis code with pharmacy claims for blood glucose test strips and lancets. The diagnosis code determines the applicable quantity limit.

    Denial Risk

    Insufficient Non-Preferred CGM Rationale

A non-preferred CGM request may be denied when the required continuation, preferred-device trial with inadequate response or intolerance, preferred-device limitation, or insulin-pump compatibility rationale is not adequately documented.

    Denial Risk

    Initiation Criteria Not Met

CGM initiation is not approvable unless the request states one qualifying condition: diabetes of any type with at least one insulin pharmacy claim in the previous 180 days; level 2 or level 3 hypoglycemia; or glycogen storage disease type 1a. Non-preferred CGM requests must also meet a qualifying exception.

    Denial Risk

    Follow-Up Assessment Not Documented

Continuation may be denied when the prescriber does not document a follow-up visit assessing the recipient’s ongoing benefit from therapy.

    Billing Rule

    Diagnosis or Quantity-Limit Edit

A pharmacy claim for test strips or lancets may reject when it lacks an appropriate diagnosis code or exceeds the quantity limit associated with the submitted diagnosis.

    • Gestational diabetes, diabetes in pregnancy, or diabetes with insulin use: 200/204 strips or lancets per 30 days.
    • Diabetes without insulin use: 100/102 strips or lancets every 90 days.
    • Use the preferred package size and dispense up to the applicable limit based on directions.
