Cervical Spine MRI — Coverage Criteria

    inv-01: Acute cervical pain

Covered when ANY of the following acute conditions are met

    A. Acute cervical radicular pain without red flags: Acute cervical radicular pain (radiating into one or both arms) WITHOUT red flags — MRI of the cervical spine is not indicated; initial medical management is recommended.onset <= 6 weeks

MRI without contrast not indicated for this scenario

    B. Acute cervical pain with red flags: Acute cervical pain with radiating pain into the arm AND one or more red flag conditions (progressive objective neurological signs; evidence of spinal instability or fracture on other imaging; compelling clinical concern for malignancy, infection, immunosuppression, or bone–disc margin destruction) — cervical MRI may be indicated.onset <= 6 weeks

Use MRI without contrast for instability/fracture; with or without contrast for malignancy/infection per clinical concern

    C. Urgent surgical or interventional need: Patient clinical presentation indicates need for urgent surgery or other intensive intervention as determined by a surgeon or interventional specialist — MRI may be indicated even without 6 weeks of conservative treatment.any

Surgeon or interventional specialist determination required

inv-02: Subacute and Chronic cervical pain

Covered when ALL of the following for subacute/chronic presentations are met as applicable

Subacute (>6 weeks) conservative therapy: Patient has had at least 6 weeks of medical/conservative treatment for the current episode, which must include at least 4 weeks of physical therapy (initial PT evaluation plus at least one follow-up within the specified timeframe) with no significant improvement (remote past PT does not qualify).>6 weeks

PT must be for the current episode; initial eval + follow-up required

Subacute clinical findings: Clinical evaluation demonstrates ONE or more of: abnormal reflexes or motor deficits in the C5, C6, C7, or T1 nerve territory on one side; prior neck surgery with significant new neurological signs compared to maximal postop recovery baseline; evidence of spinal instability or fracture on other imaging; complex congenital anomaly or deformity; strong suspicion for cervical spinal cord stenosis/myelopathy (e.g., bowel/bladder changes, abnormally increased reflexes, positive Babinski, spastic gait/ataxia); imaging or clinical findings suggesting new adverse surgical effects (e.g., hardware failure, concern for epidural scarring/arachnoiditis).clinical exam or imaging

If criteria met, MRI without contrast unless specific indication for contrast

    inv-03: Condition-specific indications

Covered when specific condition-based criteria are met

Multiple Sclerosis (MS): Suspected MS: cervical MRI (with contrast) is indicated for staging along with brain MRI. Known MS (confirmed by neurology): MRI with contrast is approved for annual surveillance with brain MRI; after clinical flare; 3–6 months after radiologic evidence of a flare; and at 3–6 months and/or 6–12 months after changing disease-modifying therapy as directed by neurology.per neurology

Neurology involvement recommended for staging and surveillance

Ankylosing Spondylitis (AS): Advanced imaging of the spine for suspected AS is medically necessary when ALL of: non-diagnostic radiographs; inflammatory back pain present >=3 months with at least four defining features (younger than 40, insidious onset, improvement with exercise, no improvement with rest, night pain improving on getting up); and imaging ordered by or with a rheumatologist. For confirmed AS, advanced imaging is medically necessary when ordered by the managing rheumatologist for unclear disease activity after full clinical and laboratory evaluation (progression on MRI should lead to change or cessation of biologic therapy).AS suspected or confirmed

Ordered by or in conjunction with rheumatology

Functional MRI is explicitly excluded from coverage for cervical spine imaging. Functional MRI is not covered except for presurgical planning, in which case authorization and documentation must follow the separate Presurgical Planning Policy.

MRI performed for acute cervical radicular pain within the first 6 weeks of symptom onset is not medically necessary when there are no red flag conditions or an urgent surgical indication. Initial management should be conservative medical care; requests in this timeframe without documented red flags or surgeon-determined urgency may be denied.

For subacute or chronic cervical pain, MRI is considered not medically necessary when the patient has not completed the required conservative therapy for the current episode. The policy requires at least 6 weeks of medical/conservative treatment including at least 4 weeks of physical therapy (initial PT evaluation plus at least one follow-up within the last 6 months) before MRI is authorized, unless red flags or urgent surgical indications are present.

inv-16: Appropriate use criteria for cervical spine MRI — 3 top-level nodes

Appropriate use criteria for cervical spine MRI

Red flags for acute MRI: Progressive objective neurological signs on repeat in-person exam; evidence of spinal instability or fracture on other imaging; or compelling clinical concern for malignancy, infection, immunosuppression, or bone–disc margin destruction — these red flags may indicate need for acute MRI (use MRI without contrast for instability/fracture; with or without contrast for malignancy/infection).any

Red flag definitions per clinical exam and imaging

Conservative therapy then MRI for subacute/chronic pain: At least 6 weeks of conservative care for the current episode, which must include at least 4 weeks of physical therapy (initial PT eval plus at least one follow-up) and no significant improvement, plus clinical findings such as abnormal reflexes or motor deficit (C5–T1), prior neck surgery with new significant neuro signs, evidence of instability/fracture on other imaging, complex congenital anomaly/deformity, or suspicion for cord stenosis/myelopathy — MRI without contrast unless specific indication for contrast.>6 weeks

PT must be for current episode; remote PT does not qualify

    Provider Requirements and Prior Authorization

    Prior Authorization

    Prior Authorization Required

Prior authorization is required per plan for cervical spine MRI when medical necessity criteria apply. Verify authorization requirements by plan and code prior to scheduling; use the Pre-authorization Code Check for specific CPT verification. High-end imaging site-of-care review may be required for hospital-based imaging departments.

    • Affected CPT codes: 72141, 72142, 72156 (list may not be all-inclusive)
    • High-end imaging site-of-care review for hospital-based imaging departments
    Documentation Required

Documentation Incomplete — Risk of Delay or Denial

Requests missing required documentation may be delayed or denied. Ensure submission includes the last 6 months of clinical notes from the requesting provider and/or specialist and documentation of conservative therapy when applicable.

    • Last 6 months of clinical notes from requesting provider and/or specialist

    Key Definitions and Terms

    inv-14: Red Flags
DefinitionProgressive objective neurological signs on repeat in-person exam (e.g., progressive motor weakness)
DefinitionEvidence of spinal instability or spinal fracture on other imaging (e.g., plain films or cervical CT)
DefinitionRadiating neck pain with compelling clinical concern for malignancy, infection, immunosuppression, or bone–disc margin destruction on radiographs (may require MRI with or without contrast)
    inv-15: Conservative treatment
Minimum durationAt least 6 weeks of medical/conservative treatment for the current episode of neck pain
Physical therapy requirementMust include at least 4 weeks of physical therapy, including an initial PT evaluation and at least one follow-up visit (remote past PT does not qualify)

    Imaging Interval Guidance

    inv-17: MRI
ConditionNo objective neurologic findings and surgeon requests another MRI prior to surgery
Frequency limitPrior cervical MRI must be at least 1 year old before obtaining another MRI when no objective neurologic findings are present
ContextApplies when patient has been determined a surgical candidate but lacks progressive objective neurological change; ensures interval imaging is not obtained more frequently than annually in this scenario

    Preauthorization and Plan Verification

    Prior Authorization

Verify MRI prior authorization per plan and CPT code

MRI prior authorization is required per plan and CPT code; use the Pre-authorization Code Check to verify requirements for the requested MRI service.

    • Confirm plan-specific prior authorization requirements via the Pre-authorization Code Check

Use of Gadolinium Contrast — When Contrast is Indicated

    Billing Rule

Follow contrast rules when red flags, MS, or oncologic indications are present

Use contrast-enhanced MRI when red flags suggest infection, malignancy, or immunosuppression, or when evaluating suspected multiple sclerosis or certain neoplastic processes; choose MRI with or without contrast based on the specific clinical concern.

    • Red-flag conditions (e.g., malignancy, infection, immunosuppression) may require MRI with or without contrast per the clinical scenario
    • Suspected MS and oncologic staging commonly require contrast-enhanced studies

    Clinical Background and Rationale

Most acute cervical radicular pain resolves with conservative management and many bulging discs retract spontaneously, so immediate imaging has limited value in the absence of concerning findings. MRI provides greatest value when results would change urgent management (for example, when red flags — progressive objective neurologic deficits, evidence of instability or fracture, or concern for infection/malignancy — are present) or when documenting neurologic deterioration or surgical planning.

    Services Not Covered

Functional MRI is a non-covered service for cervical spine indications except when used specifically for presurgical planning as governed by the Presurgical Planning Policy. Requests for functional MRI outside that presurgical planning context will be denied as not covered.
