    Coverage and Admission Criteria

Eligibility for IRSH / Regional Funding

Covered when ALL of the following are met

IRSH eligibility: Individual is age 18 or older; has a diagnosis of mental illness (MI) or intellectual disability (ID); may include developmental disability (DD) or brain injury (BI) when funding is available; individual income is 150% of federal poverty level or below (may be covered on a sliding scale up to 250% FPL); and results of a standardized functional assessment support the need for services.

Providers must be enrolled with Medicaid as HCBS habilitation or HCBS ID waiver supported community living and meet additional criteria in 441 - subrule 25.6(8).

    Admission Criteria

Covered when ALL of the following are met

Admission requirements: Adult with diagnosis of severe and persistent mental illness (SPMI) or multi-occurring conditions; standardized functional assessment and screening for multi-occurring conditions completed within 30 days prior to application; authorized at LOCUS/CALOCUS disposition level six (medically managed residential services) with H2016 U7 billing; three or more areas of significant impairment in ADLs/IADLs; needs 24-hour supervised and monitored treatment to maintain or improve functioning and avoid higher level of care; demonstrated lack of progress or regression after an adequate trial of active treatment at a less intensive level of care; at risk for significant functional deterioration without IRSH.

Members approved under ID Waiver SCL are authorized for H2016 U7; reimbursement negotiated between MCO and provider.

Additional risk indicators (meet at least one): 3+ psychiatric emergency room visits in past 12 months OR 3+ psychiatric hospitalizations in past 12 months OR 90+ psychiatric hospital days in past 12 months OR 30+ medically unnecessary psychiatric hospitalization days in past 12 months OR residing in a state resource center with SPMI OR being served out of state due to unavailability of services in Iowa OR SPMI and being released from a correctional facility or jail OR homeless or precariously housed.

These indicators support need for highest level (level 6) residential habilitation.

Providers may deny admission or request an unplanned discharge under the IRSH No Reject / No Eject (NENR) protocol only for specific, documented reasons. Allowable reasons include when placement would jeopardize the safety of other residents; the individual shows obvious signs of illness or injury requiring immediate medical attention; the individual requires current or immediate inpatient psychiatric services; the individual is a registered sex offender and the residence is within proximity to protected locations; or the individual requires ongoing criminal intervention. Providers must follow the NENR procedure for documenting and communicating denials or discharges and should contact the individual's MCO for the complete protocol.

IRSH providers must maintain clinical oversight and community partnerships to support safe, coordinated care. Specifically, programs must have clinical oversight by a mental health professional who consults on behavioral health services, service plans, behavior intervention and crisis plans, coordinates monthly interdisciplinary team meetings, and provides staff training. Providers are required to have written cooperative agreements with an outpatient mental health provider and a substance abuse treatment program. Additionally, evidence supporting initial designation and ongoing participation in regional review, reporting, and quality activities must be submitted to the MHDS region, and providers must work with the Medicaid Quality Improvement Organization to ensure IRSH settings do not isolate residents or become institutional.

Individuals who have not demonstrated an adequate trial of active treatment at less intensive levels or who are not at risk of functional deterioration do not meet IRSH admission criteria. IRSH admission requires that the person has exhibited lack of progress or regression after an adequate trial at a less intensive level of care and is at risk of significant functional deterioration without 24-hour supervised and monitored treatment.

    Provider Requirements, Documentation, and Operations

    Prior Authorization

Meet 441—subrule 25.6(8) for regional designation and annual review

Organizations seeking regional designation as an IRSH provider must meet the criteria in 441—subrule 25.6(8) at initial application and annually; regions will collaborate with MCOs to develop consistent criteria and referral pathways.

    • Submit evidence to the MHDS region to support initial designation and participate in annual review and reporting.
    Step Therapy

Document inadequate response to less intensive treatment before step‑up

Before stepping up care to IRSH, the individual must have shown lack of progress or regression after an adequate trial of active treatment at a less intensive level of care.

    • Document the prior less‑intensive treatment, duration, and clinical response to demonstrate inadequate progress or regression.

    Level-of-Care Definitions and Criteria

    Residential - Intensive

Covered when ALL of the following are met

Residential - Intensive criteria: Individual requires 24-hour supervised residential care with intensive behavioral health services; supervision provided 24/7 with staffing ratio approximately 1 staff per 2.5 clients; continued need for intensive residential habilitation to maintain or improve functioning and avoid higher level care; placement does not jeopardize safety of others and medical/behavioral needs remain within provider scope.

IRSH providers accept court-ordered individuals and operate on a no reject/no eject basis with limited allowable exceptions per NENR protocol.

    Intensive Residential (IRSH)

Covered when ALL of the following are met

IRSH level-of-care criteria: Adult with SPMI or multi-occurring conditions authorized at LOCUS/CALOCUS level six (medically managed residential) and billed as H2016 U7; standardized assessment within 30 days; three or more areas of significant impairment in ADLs/IADLs; need for 24-hour supervised and monitored treatment to maintain or improve functioning; prior inadequate response to less intensive treatment; at risk for significant functional deterioration without IRSH.

    Staff Training and Clinical Oversight

    Staff training / behavioral interventions

Staff competencies and required interventions: Staff must have at least one year of experience working with individuals with mental illness or multi-occurring conditions and complete a minimum of 48 hours of training covering applied behavioral analysis; autism spectrum disorders; brain injury diagnoses; crisis management and de-escalation; motivational interviewing; psychiatric medications; substance use disorders and treatment; and other diagnoses or conditions present in the population served.48 hours

Training requirement is twice the non-IRSH requirement.

    Staff training and clinical oversight

Training hours and clinical oversight: Programs must provide clinical oversight by a mental health professional who consults on behavioral health services and service plans, reviews behavior intervention and crisis intervention plans, coordinates monthly interdisciplinary team meetings, and provides staff training; providers must also maintain written cooperative agreements with outpatient mental health and substance use treatment programs.

    Program Background and Scope

Intensive Residential Services (IRSH) provide community-based, 24-hour supervised residential habilitation for adults with the most severe and persistent mental health conditions and accompanying functional impairments. IRSH is intended for individuals with complex needs who require intensive behavioral health services, continuous supervision, and supports to maintain or improve functioning and avoid higher levels of care. The program emphasizes staff training, clinical oversight, and regional coordination to meet the needs of adults with severe mental illness, intellectual or developmental disabilities where funding permits, and co-occurring conditions.

    Prior Authorization

Prior authorization at LOCUS/CALOCUS Level 6 and H2016 U7 billing

IRSH admission requires prior authorization at the highest LOCUS/CALOCUS disposition (level six — medically managed residential services) and must be billed using the H2016 U7 habilitation code for Level 6 IRSH. Members approved under the ID Waiver SCL service are authorized for H2016 U7 and reimbursement is negotiated between the member's MCO and the IRSH provider.

    • Obtain LOCUS/CALOCUS actual disposition of level six (medically managed residential services) prior to admission.
    • Bill IRSH Level 6 services with CPT/HCPCS code H2016 U7; ID Waiver SCL-approved members also use H2016 U7.
    • Reimbursement arrangements for ID Waiver SCL members are negotiated between the member's MCO and the IRSH provider.
    Documentation Required

Follow verbal then written notification and evidence‑based rationale for denial/unplanned discharge

To request denial of admission or an unplanned discharge the provider must indicate intent, provide a verbal description, and follow with a written rationale transmitted by the end of the following business day that includes evidence why they cannot admit or continue serving the individual.

    • For habilitation‑funded individuals, notify the Integrated Health Home (IHH) Care Coordinator by the end of the following business day upon receipt of referral.
    • IHHs shall notify the funding MCO's program supervisor/manager of the request.
    • If unresolved, a designated MCO director/manager or designee will review the case; decisions on unplanned discharge are made by the end of the next business day after the provider's written request.
    Documentation Required

Submit complete referrals with recent LOCUS and maintain CASH documentation

Submit complete referrals that include a LOCUS completed 30 days or less prior to application and maintain a complete, current, and descriptive CASH and other relevant documentation to inform the LOCUS.

    • Secure releases of information (ROIs) for needed clinical records.
    • Check bed availability prior to referral and include all required documentation to demonstrate IRSH eligibility per 441‑25.6(8).
    • Provide evidence to the MHDS region to support initial designation and participate in annual reporting.
    Denial Risk

Allowed denial/discharge reasons per NENR protocol

Providers may deny admission or discharge an IRSH recipient only for allowable NENR reasons such as placement that would jeopardize safety of others; immediate medical needs requiring higher level care; need for current inpatient psychiatric services; registered sex offender proximity concerns; ongoing criminal intervention; medical needs exceeding provider scope; or no longer meeting funder level of service.

    • Incomplete referrals lacking required information will not be considered a reject.
    • Contact the individual's MCO for the complete NENR protocol; the lists in policy are not comprehensive.
    Prior Authorization

    Admission prevented without LOCUS/CALOCUS level‑6 authorization (H2016 U7)

Lack of authorization at the required LOCUS/CALOCUS disposition (level six medically managed residential services) will prevent IRSH admission; Level 6 admissions must be authorized for H2016 U7.

    • Ensure LOCUS/CALOCUS actual disposition documents level six and authorization is in place prior to admission.
    Note

Ensure complete authorization, documentation, and regional coordination

Confirm and document all required designation, authorization, and referral elements (LOCUS/CALOCUS, LOCUS within 30 days, CASH documentation, MCO coordination) prior to admission; maintain open communication with IHH and MCO to reduce unplanned discharges.

    • Participate in regionally developed referral pathways and reporting mechanisms.
    • Reassess frequently and plan for discharge when the individual stabilizes.

H2016 U7 is the CPT/HCPCS billing designation for Level 6 IRSH; members approved under ID Waiver SCL are authorized for H2016 U7.

Continued stay: Ongoing documented need for 24-hour supervised and monitored treatment, periodic reassessment with LOCUS, and stabilization planning with goal of discharge when clinically appropriate.

Decisions about unplanned discharge follow NENR protocol and required documentation processes.
