OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity Criteria

Shoulder joint replacement is not covered when there is an active infection of the joint, active systemic bacteremia, or an active skin infection or open wound within the planned surgical site. An exception is noted for recurrent cutaneous Staphylococcus infections. These conditions must be resolved prior to consideration of arthroplasty because they increase the risk of prosthetic joint infection and postoperative complications.

Total shoulder arthroplasty or hemiarthroplasty are not covered when clinical conditions would place excessive stress on the implant. Examples include neuropathic (Charcot) joints and paralytic conditions of the shoulder. Procedures performed under these circumstances risk early implant failure and are excluded from coverage.

    Clinical Thresholds and Coding-related Criteria

Conservative therapy duration — minimum documented period required prior to surgery
Minimum duration3 months of documented non-surgical management (start and end dates, results) prior to consideration of shoulder arthroplasty
Required conservative strategiesDocumentation must include a combination of strategies and at least 2 of: activity modification, physical therapy, supervised home exercise, medications (NSAIDs/analgesics), intraarticular corticosteroid injection(s)
Therapy documentation detailsEach therapy must have start and end dates and results; PT requires complete therapy progress notes; corticosteroid injections require procedure report and outcomes
Symptom duration threshold — At least 6 months of pain and loss of function interfering with daily activities
Minimum symptom durationAt least 6 months of pain and loss of function that interferes with daily activities
Associated exam findingsLimited range of motion or crepitus of the glenohumeral joint on physical examination must be documented
Relationship to procedure typesApplies to hemiarthroplasty, total shoulder arthroplasty, and reverse shoulder arthroplasty advanced-disease criteria
Radiographic evidence threshold — 2 or more radiographic features required
Radiographic requirementRadiographic evidence of destructive degenerative joint disease demonstrated by 2 or more features
Acceptable radiographic featuresIrregular joint surfaces; glenoid sclerosis; osteophyte changes; flattened glenoid; cystic changes in the humeral head; joint space narrowing
ContextRequired as part of 'advanced joint disease' criteria for hemiarthroplasty, total shoulder arthroplasty, and reverse shoulder arthroplasty
Glycemic control consideration — preferred HbA1c threshold and documentation expectations
Preferred HbA1c thresholdHbA1c <= 8.5% is preferred prior to elective joint replacement
If HbA1c > 8.5%Documentation that every effort was made to optimize glycemic control prior to elective surgery, or documentation that the surgeon reviewed increased risks with the member
Documentation expectationMedical record must show attempts to optimize glycemic levels or surgeon discussion of risks when control is poor

    Preauthorization and Documentation Requirements

    Prior Authorization

Preauthorization and 3‑month conservative therapy documentation required

Preauthorization is required for shoulder joint replacement. Requests must include documentation of a 3-month trial of non-surgical management with start and end dates, outcomes, and a clinical assessment within 3 months of the request.

    • Documentation of non-surgical medical management measures for a period of 3 months for the condition(s)
    • Start and concluding dates for each therapy and results of therapy
    • Documented clinical assessment by the requesting provider within 3 months of the request
    Step Therapy

Document ≥3 months of conservative therapy with ≥2 strategies

Document a reasonable attempt at conservative therapy for at least 3 months that includes two or more distinct strategies (unless conservative management is inappropriate and the reason is documented).

    Background and Scope

Shoulder arthroplasty is indicated when nonsurgical management fails to control pain or functional impairment from glenohumeral joint destruction. Indications include intractable joint pain and unacceptable decline in function despite appropriate conservative measures. Preoperative optimization factors—such as tobacco cessation and, for patients with diabetes, efforts to achieve or document attempts to optimize glycemic control (with elevated HbA1c > 8.5% prompting documented optimization efforts or surgeon risk discussion)—should be addressed prior to elective arthroplasty. Preauthorization is required for shoulder joint replacement.

    Procedure Definitions

    Hemiarthroplasty — definition
DefinitionA surgical procedure where the head of the humerus is replaced with a metal ball and stem (similar to the component used in a total shoulder replacement)
Typical indicationsUsed for proximal humerus fracture not amenable to internal fixation, malignancy involving the glenohumeral joint, or advanced joint disease when glenoid reconstruction is not appropriate
Relation to total arthroplastyHemiarthroplasty replaces only the humeral head rather than both humeral and glenoid surfaces
    Reverse shoulder arthroplasty — definition
DefinitionA surgical procedure where a metal ball is attached to the shoulder (glenoid) bone and a plastic socket is attached to the upper arm (humeral) bone, allowing the deltoid to substitute for a torn rotator cuff

  2. Therapy start and end dates and outcomes must be recorded
  3. If conservative management is inappropriate, document the reason
  4. Documentation Required

Required clinical documentation to accompany request

Submit clinical documentation demonstrating anticipated functional demands, intact deltoid, adequate bone stock for implant fixation, symptom duration/severity, specific functional limitations, and the type, duration, and results of prior therapies.

    • Anticipated level of function and related demands on the shoulder
    • Deltoid muscle integrity
    • Adequate bone stock to allow firm fixation of the implant
    • Symptom duration and severity and specific functional limitations
    • Procedure reports for intraarticular corticosteroid injections and therapy progress notes
    Denial Risk

    Obtain preauthorization before scheduling surgery

Preauthorization is required for shoulder joint replacement and absence of preauthorization may result in claim denial.

    • Policy explicitly marks Pre-Authorization Required: Yes
    • Preauthorization is required for this service
Used for glenohumeral osteoarthritis with irreparable rotator cuff tear, reconstruction after tumor resection, failed prior arthroplasty, or non-reconstructable fracture
Functional mechanismDesign reverses the normal ball-and-socket orientation to enable the deltoid muscle to lift the arm when the rotator cuff is deficient
Shoulder joint replacement (arthroplasty) — general definition
DefinitionA surgical procedure where arthritic joint surfaces are replaced with a highly polished metal ball attached to a stem and a plastic socket
ScopeIncludes procedures that replace humeral and glenoid surfaces — total shoulder arthroplasty and related reconstructive shoulder replacements
Indications overviewIndicated when non-surgical measures fail to control pain or dysfunction from joint destruction and advanced joint disease criteria are met
