    Medical Necessity Criteria

    Lipedema surgery

Lipectomy, liposuction, and excision of excessive skin for the treatment of lipedema may be considered medically necessary when all criteria are met.

    Lipedema surgical treatment

    • Physical examination findings

      • Characteristic fat distribution: Bilateral, symmetric disproportionate fat deposition in the lower extremities with sparing of the hands and feet.
      • Ankle cuff sign: An abrupt demarcation of fatty tissue at the ankle with the dorsum of the feet spared.
      • Tenderness to palpation is present.
      • Pitting edema is absent, unless there is concomitant lymphedema.Absent unless concomitant lymphedema
    • Photographs support the physical examination findings.
    • Functional impairment: The individual has a significant physical functional impairment, such as difficulty ambulating, performing activities of daily living, or intractable pain.Significant impairment
    • Conservative treatment failure

      • ONE or more treatment options

        • Medical-grade compression garments.
        • Manual lymphatic drainage.
        • Intermittent pneumatic compression.
        • Conservative treatment is contraindicated due to a condition such as active infection, deep vein thrombosis, or congestive heart failure.
    • Weight-loss efforts have failed to reduce fat distribution in lipedema-affected areas.Failed reduction in affected-area fat distribution
    • Postoperative care includes the use of compression garments to maintain the surgical benefits.Compression garments required

    Lymphedema excisional procedures

Excisional procedures, including suction-assisted protein lipectomy (SAPL) and debulking, for stage II or stage III non-pitting lymphedema may be considered medically necessary when all criteria are met.

    Lymphedema excisional treatment

    • Conservative treatment failure

      • ONE or more treatment options

        • Manual lymphatic drainage (MLD).
        • Medical-grade compression garments.
        • Complete decongestive therapy (CDT).
    • Diagnosis and stage photographs: Photographs support the diagnosis of stage II or stage III non-pitting lymphedema.

    EPSDT consideration

For individuals under 21, EPSDT coverage applies when the service is medically necessary to correct or ameliorate a qualifying defect, illness, or condition.

EPSDT special provision: The individual is under 21, and the requested service, product, or procedure is medically necessary health care to correct or ameliorate a defect, physical or mental illness, or a condition identified through a screening examination.Under 21

The applicable definition of medical necessity is set forth in Connecticut General Statutes Section 17b-259b.

    Authorization and Documentation Requirements

    Prior Authorization

    Prior Authorization Required Before Surgery

Submit prior authorization before the surgical procedure for covered individuals enrolled in HUSKY A, HUSKY B, HUSKY C, or HUSKY D. This requirement applies to prior authorization requests submitted on or after November 1, 2026.

    Documentation Required

    Case-Specific Surgical Review

Coverage requests are reviewed using the requested and submitted case-specific clinical information, including the completed online authorization request and supporting documentation.

    Step Therapy

    Lipedema Conservative-Treatment Trial

    Condition Overview

Lipedema is a chronic, progressive adipose tissue disorder characterized by bilateral, symmetric accumulation of abnormal subcutaneous fat, primarily in the lower extremities and sometimes the upper arms. The hands and feet are typically spared, producing a characteristic cuffing appearance at the ankles and wrists. Lymphedema is a chronic disorder of lymphatic transport that causes accumulation of protein-rich fluid in the interstitial tissues, resulting in persistent swelling that may affect the extremities, trunk, head, neck, or genitalia.

This policy addresses surgical treatment when the condition causes significant physical functional impairment and conservative management has been unsuccessful or is contraindicated. Procedures considered under the policy include lipectomy, liposuction, excision of excessive skin, suction-assisted protein lipectomy, and debulking. Coverage review is based on the individual’s clinical needs and supporting documentation, including relevant examination findings, photographs, treatment history, and the planned postoperative use of compression garments.

    Denial Risk

    Lipedema Examination Criteria Not Met

Lipedema coverage may be denied when the required examination pattern is not documented, including bilateral symmetric disproportionate fat deposition in the lower extremities with sparing of the hands and feet, the ankle cuff sign, tenderness to palpation, and absent pitting edema unless concomitant lymphedema is present.

    Denial Risk

    Insufficient Conservative Treatment

Coverage may be denied when the individual has not failed at least three months of appropriate conservative treatment, unless an applicable contraindication to conservative treatment is documented for lipedema.

    Denial Risk

    Significant Functional Impairment Required

Coverage may be denied when significant physical functional impairment is not documented. Examples include difficulty ambulating, difficulty performing activities of daily living, recurrent cellulitis for lymphedema, or intractable pain.

    Denial Risk

    Weight-Loss Criterion Not Met

For lipedema, coverage may be denied when documented weight-loss efforts have not failed to reduce fat distribution in the affected areas.

    Denial Risk

Lymphedema Stage or Phenotype Not Eligible

Lymphedema excisional procedures may be denied when the condition is not documented as stage II or stage III non-pitting lymphedema.

    Denial Risk

    Postoperative Compression Plan Required

Coverage may be denied when the postoperative plan does not include compression garments to maintain the surgical benefit for lipedema or lymphedema.

    Denial Risk

    Incomplete Prior Authorization Submission

Missing authorization information, required clinical documentation, or photographs can result in a denied or delayed review. Verify that the submission is complete and addresses the applicable criteria before sending it.
