OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Same-Claim Billing Rules and Exceptions

Same-Claim Billing Rules, Exceptions, and Enforcement

Humana's stance on same-claim billing, applicable exceptions, and consequences for violations.

When applicable regulatory standards exist, providers must comply with those standards specifying which services must or must not be on the same claim.
If no applicable regulatory standards exist, providers must submit all charges for the same encounter on a single claim.
When submitting a professional claim, report all charges for same-day services on that professional claim, regardless of whether they pertain to the same encounter.

    Exceptions

    • Reference laboratories may submit a distinct claim for testing performed in distinct, specific service facility locations; this does not permit multiple claims for testing for the same service facility location.
    • TEFRA hospital interim claim billing is permitted consistent with Original Medicare guidance.

    ALL of the following

    • Outpatient hospital repetitive services furnished in multiple encounters during the same calendar month may be reported on the same outpatient hospital claim.
Consequences: Humana may, at its sole discretion, reject, deny or recoup reimbursement for submissions that violate these same-claim billing requirements and prohibitions.
Timely filing: Timely filing standards apply to each charge on a multi-charge claim; an untimely charge may be denied even if other charges on the same claim are timely.

Policy summaryEach charge on a claim is evaluated for timely filing independently; an untimely charge may be denied even if other charges on the same claim are timely.
ScopeStandards apply to all charges, including charges reported on the same claim for services furnished in multiple encounters.
ExampleFor hospital outpatient repetitive services furnished during a month, Humana will deny a charge that was not submitted timely even if another repetitive services charge on that claim is timely.
EnforcementHumana may deny individual charges on a multi-charge claim for failure to meet timely filing standards.

    Provider Responsibilities and Plan Requirements

    Documentation Required

General plan requirements for claim submission

Providers must follow all plan requirements when submitting claims, including meeting medical necessity and reasonableness standards and any applicable referral or authorization requirements. When no specific regulatory standard governs same-claim billing, submit all charges for the same encounter on a single claim; for professional claims, report all same-day services on that claim. Failure to comply with these plan requirements may result in claim processing consequences (rejection, denial, or recoupment).

    • Comply with medical necessity and reasonableness requirements and any applicable referral/authorization rules before submitting claims.
    • When no regulatory standard applies, submit all charges for the same encounter on one claim.
    • For professional claims, report all charges for same-day services on that professional claim.
    • Noncompliance may lead to claim rejection, denial, or recoupment.
