OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Background and Intent

This ABA Request Form is designed to support authorization for outpatient Applied Behavior Analysis services for behavioral health conditions commonly addressed with ABA. It prompts providers to document presenting target behaviors (for example: self-injurious behavior, aggression, elopement, communication and social skill deficits, tantrums, and self-stimulatory behavior), complete relevant assessment fields, and attach supporting reports and a full Behavioral Support Plan/Treatment Plan.

The form explicitly captures whether the request is an Initial or Continued Stay authorization and requires a Start Date of Plan of Care. The plan of care is ordinarily subject to a 6-month timeframe unless otherwise noted, and documentation submitted for continued stay review must meet recency requirements (see documentation guidance).

Providers must complete member and provider identification sections, list primary and additional diagnoses, indicate the standardized assessment tool used, and attach the diagnostic evaluation/report and full treatment plan to support medical necessity and authorization processing.
