OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Seat Lift Mechanism

    Initial coverage criteria

Covered when ALL of the following are met as documented on the CMN:

    Primary clinical conditions

    • Either condition: Severe arthritis of the hip or knee OR severe neuromuscular disease (see CMN items 1–2).
Functional limitation: Patient is completely incapable of standing up from a regular armchair or any chair in the home (CMN item 3).
Ambulation status: Once standing, patient has the ability to ambulate (CMN item 4).

If the patient cannot ambulate after standing, documentation should clarify the therapeutic goals for the device.

Conservative therapy: All appropriate therapeutic modalities to enable transfer (e.g., medication, physical therapy) have been tried and failed and are documented in the medical record (CMN item 5).
Documentation: Completed CMN with physician signature (stamps are not acceptable) and a typed office note with pertinent information must be submitted.

Typed office note is listed under requested information on the CMN (Rev. 06/28/2011).

The CMN explicitly states that physician stamps are not acceptable for signature. A valid physician signature is required on the completed CMN; documentation submitted with a stamped signature will not meet the signature requirement.

    Provider Submission and Documentation Requirements

    Documentation Required

CMN submission requirement — Completion and signature

Completion of the Certificate of Medical Necessity (CMN) with all clinical questions answered, the physician's handwritten signature (stamps are not acceptable), and a supporting typed office note is required to establish medical necessity. Incomplete or missing affirmative answers to the CMN clinical questions, a missing or stamped physician signature, or absence of the required typed office note may result in noncoverage or denial of the request.

    • CMN must be fully completed: all clinical questions answered (Y/N/D)
    • Physician signature required on CMN; signature stamps are not acceptable
    • Provider must submit a typed office note documenting pertinent clinical information and rationale
    Denial Risk

Therapeutic modality trial — Document and demonstrate failure

Documented trial and failure of appropriate therapeutic modalities (for example, medication and physical therapy) to enable the patient to transfer from a chair to a standing position must be clearly described in the medical record and included with the submission. Lack of documentation of such trials or failure to document that therapies were tried and failed may lead to noncoverage.

    Medical Necessity Determination

DME medical necessity — Medical necessity is established when the following are documented on the CMN.

Medical necessity is established when the following are documented on the CMN:

A: Severe underlying condition: Severe arthritis of the hip or knee OR severe neuromuscular disease (diagnosis documented on CMN if present).

    CMN items 1–2.

B: Inability to rise: Patient is completely incapable of standing up from a regular armchair or any chair in the home (CMN item 3).
C: Ambulates once standing: Patient retains the ability to ambulate after standing (CMN item 4).
D: Conservative management tried: All appropriate therapeutic modalities to enable transfer have been tried and failed and are documented in the medical record (CMN item 5).

Documents to Include with the Request

    Note

Attach typed office note and physician-signed CMN with request

Ensure the request includes a typed office note with pertinent information and a completed CMN with the physician's signature; physician stamps are explicitly not acceptable.

    Severe functional limitation — definition
DefinitionPatient is completely incapable of standing up from a regular armchair or any chair in the home (CMN item 3).
Where askedRecorded as question 3 on the Certificate of Medical Necessity (CMN) for Seat Lift Mechanism.
    Response optionsCMN records responses as 1 = Y (Yes), 2 = N (No), 3 = D (Declined/Unknown).
Clinical contextUsed to establish functional limitation necessary for coverage when combined with other CMN criteria (e.g., underlying diagnosis, ambulation status, and failed conservative therapy).

    Background

Seat lift mechanisms (lift chairs) are intended to assist members who cannot rise from a chair due to disabling conditions such as severe arthritis of the hip or knee or severe neuromuscular disease. The CMN captures the presence of these underlying conditions, the patient’s ability to stand and ambulate once standing, and whether conservative therapeutic modalities (e.g., medication, physical therapy) have been tried and documented.

The form requires completion of specific clinical items (items 1–5) and submission of a typed office note with pertinent information in support of the request; these elements together establish the clinical rationale for the device and support coverage determinations.

    Noncoverage Situations

Requests will not meet coverage criteria when the CMN clinical questions are not affirmatively answered as required (for example, absent documentation that the patient is completely incapable of rising from a regular armchair or that conservative therapies have been tried and failed).

Additionally, required documentation must accompany the CMN. The policy requires a typed office note with pertinent information and a completed CMN bearing a physician signature (remember: stamps are not acceptable). Missing or incomplete documentation will result in the request failing to meet coverage criteria.

    • Documentation must specify which therapeutic modalities were tried, dates, duration, and outcome
    • Medical records must demonstrate failure of conservative treatments prior to approval
    Documentation Required

Required documentation — Typed office note and signed CMN

Provider must include a typed office note with pertinent clinical information and the completed CMN bearing the physician's original signature. Contact information (contact name and phone) and any additional clinical rationale should be provided as applicable. Failure to include the typed office note or to provide a signed CMN will delay processing or result in denial.

    • Typed office note with pertinent information is required
    • CMN must include physician signature and date (no stamps)
    • Include contact name and phone number and any additional clinical rationale
    Denial Risk

CMN clinical criteria gaps — Resolve discrepancies to avoid noncoverage

If the CMN clinical questions are not answered affirmatively where required, or if there are gaps between CMN responses and supporting documentation in the office note, the request may be determined not medically necessary. Providers should ensure consistency between the CMN answers and the submitted medical record documentation before submission.

    • Ensure CMN answers align with supporting medical record documentation
    • Address any gaps or discrepancies in the additional clinical rationale or office notes prior to submission
