Basaglar (insulin glargine), NDC labeler code 00002, will be removed from the Medi-Cal Rx Contract Drugs List (CDL) effective August 9, 2026.
A prior authorization demonstrating medical justification must be submitted for coverage consideration when Basaglar is used instead of a CDL-listed product.

Provider Actions and Prior Authorization Submission
Prior authorization required for Basaglar NDC 00002
Claims for Basaglar (insulin glargine) NDC labeler code 00002 with dates of service on or after August 9, 2026 will deny with Reject Code 606 unless an approved prior authorization demonstrating medical justification for use over a CDL-listed product is obtained. Providers must obtain and have an approved PA in place for DOS on/after the effective date to avoid claim denial.
