    Coverage Criteria

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Personnel may only dispense medications when they are authorized and licensed to do so. Drugs must be dispensed ONLY by a physician, pharmacist, nurse practitioner (NP), certified nurse midwife (CNM), registered nurse (RN), or physician assistant (PA) acting pursuant to applicable law and the order of a licensed physician or surgeon. Staff who are not authorized providers—such as medical assistants, office managers, and receptionists—do NOT dispense drugs.

    No prior authorization required

No prior authorization requirements are specified in this document.

    Step Therapy

    No step therapy required

No step therapy requirements are specified in this document.

    Documentation Required

    VIS and temperature documentation

Document the date the Vaccine Information Statement (VIS) was given and the VIS publication date in the patient’s medical record; maintain refrigerator and freezer temperature logs at least once daily (CA DHCS recommends twice daily).

    • Ensure the most current VIS is available on site and provided prior to each vaccine dose for DTaP, Td, MMR, IPV, Hep B, Hib, Varicella, and Pneumococcal Conjugate.
    • Record and retain daily temperature checks for refrigerated and frozen vaccine storage as documented in site logs.
    Documentation Required

    Controlled substance and expiration logs

Maintain a dose-by-dose controlled substance distribution log that includes date, provider DEA number, controlled substance name, original quantity, dose administered and number of remaining doses, patient name, and name of authorized dispenser; perform monthly expiration checks for all drugs, samples, and formulas.

    • Store controlled substances separately in a secured, lockable space accessible only to authorized personnel.
    • Keep a log or clearly mark outside of packages to show monthly expiration checks; no expired drugs may be distributed.
    Denial Risk

    Site of Care Requirements

    Note

On-site pharmacy licensing and pharmacist oversight

If an on-site pharmacy exists it must be licensed by the California State Board of Pharmacy and have a licensed pharmacist monitoring drug distribution and current policies/procedures for drug storage and dispensing.

    • Licensed pharmacist must monitor drug distribution and ensure policies/procedures for storage and dispensing are current.

    Background

Proper storage, handling, and documentation are required to preserve potency, prevent contamination, and ensure patient safety for medications and immunobiologics. Vaccines and other temperature‑sensitive products must be stored within specified temperature ranges (e.g., refrigerator 2°–8°C (35°–46°F), freezer −15°C or lower) and protected from conditions that would render them adulterated or ineffective.

Facilities must maintain routine temperature and expiration monitoring: refrigerator and freezer temperatures are checked at least daily (with some authorities recommending twice daily), and expiration dates for all drugs, samples, and formulas are checked at least monthly. Temperature logs and expiration logs or clearly marked packaging are acceptable methods of documentation.

When drugs are prepared or reconstituted at dispensing, labels must include expiration information for both the reconstituted and unconstituted product. Maintain documentation of Vaccine Information Statement (VIS) distribution dates and publication dates in the medical record as part of standard vaccine handling and recordkeeping practices.
