inv-01: Diagnosis of small-fiber neuropathy (ENFD) — Covered when ALL of the following are met

Covered when ALL of the following are met:

ENFD coverage criteria: a. An individual presents symptoms of painful sensory neuropathy; b. There is no history of a disorder known to predispose painful neuropathy (e.g., diabetic neuropathy, toxic neuropathy, HIV neuropathy, celiac neuropathy, inherited neuropathy); c. Physical examination shows no evidence of findings consistent with large-fiber neuropathy (eg, reduced or absent muscle-stretch reflexes or reduced proprioception and vibration sensation); d. Electromyography and nerve-conduction studies are normal and show no evidence of large-fiber neuropathy.

All four conditions must be documented.

Guideline-supported indications: Skin biopsy with IENFD measurement may be considered/performed in patients with clinical signs of small-fiber dysfunction or suspected distal symmetric polyneuropathy, particularly small-fiber sensory neuropathy (supported at Level B/C by specialty societies) and may be used for follow-up or to detect treatment response in diabetic SFN.

Support for multimodal assessment: Guidelines recommend combining skin biopsy/IENFD with autonomic testing and bedside sensory tests; skin biopsy is a validated technique and may be particularly useful when nerve conduction studies are normal.

EFNS/PNS and AAN groups emphasize use alongside other diagnostic modalities.

Measurement of sweat gland nerve fiber density (SGNFD/SGII) from skin biopsy is explicitly excluded by this policy. The policy states: Measurement of sweat gland nerve fiber density DOES NOT MEET COVERAGE CRITERIA.

For Fabry disease, the policy references an international expert panel recommendation that when an accurate diagnostic laboratory test is available, nerve or skin biopsies are not required for diagnosis. Skin biopsy may be used only to detect or quantify small-fiber involvement in otherwise asymptomatic patients.

The revision history documents that literature reviews conducted on 12/03/2025 and 09/04/2024 "did not necessitate any modifications to coverage criteria." This indicates that the current coverage criteria remained unchanged following those reviews.

Epidermal nerve fiber density (ENFD/IENF) measurement from skin biopsy is covered only when all specified diagnostic criteria for small-fiber neuropathy are met; the policy further states that ENFD measurement from skin biopsy does not meet coverage criteria for any situations other than the specified diagnostic criteria. Requests falling outside those documented criteria will be denied.

Guideline panels note that while skin biopsy with IENFD measurement is a validated tool to assess small-fiber neuropathy, skin biopsy has not been useful for identifying the etiology of SFN in many cases. The policy advises that the test’s inability to determine cause should be considered when ordering it for etiologic assessment.

Approval requires documentation that all listed clinical criteria for the diagnosis of small-fiber neuropathy are met. Prior authorization/coverage determination is only granted when the medical record clearly demonstrates each required element (symptoms of painful sensory neuropathy; absence of disorders known to predispose painful neuropathy; physical exam without signs of large-fiber neuropathy; normal EMG/nerve conduction studies).

All clinical criteria documented in the medical record are required for approval.
Requests lacking documentation of any criterion will be denied.

Use Noninvasive Assessment Prior to Biopsy

Consider noninvasive sensory and autonomic testing prior to or alongside skin biopsy. Bedside sensory testing, quantitative sensory testing, and autonomic function testing (e.g., QSART, heart rate variability) are recommended per professional guidelines to support the clinical evaluation of suspected small-fiber neuropathy.

Perform and document appropriate noninvasive assessments before biopsy when clinically feasible.
Include results of autonomic testing in the request to support medical necessity.

CLIA Compliance Risk for Laboratory-Developed Tests

Laboratory-developed tests (LDTs) for epidermal nerve fiber density and related analyses are regulated under CLIA as high-complexity tests. Lack of proper validation or CLIA-compliant operation may present regulatory and compliance risk for laboratories and affect coverage determinations.

Ensure testing laboratory operates under CLIA and appropriate high-complexity certification.
Maintain validation and quality-control documentation for any LDTs used.

Specimen handling and laboratory documentation must specify the histologic stains and morphometric analyses performed. Requests and claims should include documentation that the following were performed and reported: IENF quantification (site and method), special stains or immunohistochemistry as applicable, and morphometric nerve analysis.

Document biopsy site (e.g., distal leg/ankle), specimen adequacy, and processing methods.
Include specific stains/tests performed (e.g., 88313, 88341, 88342, 88344, 88346, 88350) and morphometric analysis (88356) in the report.
Attach laboratory report with numeric IENF density results and interpretation to support medical necessity.
