OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity and Coverage Criteria

    Abdominal Panniculectomy

Coverage is determined according to the applicable criteria for the member's line of business.

    Applicable abdominal panniculectomy criteria

    • For Commercial and Medicare lines of business, HNE has adopted InterQual criteria: CP:Procedures, Panniculectomy, Abdominal.
    • For members under 18 years of age, the request is reviewed on a case-by-case basis.
    • For the Medicaid line of business, HNE uses MassHealth Guidelines for Medical Necessity Determination for Excision of Excessive Skin and Subcutaneous Tissue.

Commercial Excision of Excessive Skin and Subcutaneous Tissue

For the Commercial line of business, excision of excessive skin and subcutaneous tissue is medically necessary when documentation confirms ALL of the following criteria.

    ALL required commercial criteria

    • Weight loss has resulted in significant excess or redundant skin or skin folds.
    • The redundant skin or skin folds directly cause ALL of the following

      • A physical functional impairment that interferes with activities of daily living, including physical exercise.
      • Persistent symptomatic intertriginous ulceration(s) or maceration(s) that have been refractory to good personal hygiene and physician-supervised local treatment over a period of three months.

Documentation must include a detailed description of all physician-supervised skin treatment.

      • Recurrent skin infections, defined as at least two episodes within 12 months, requiring systemic antibiotics and directly related to the redundant skin.

Documentation must confirm that the episodes were refractory to at least a full course of antibiotic treatment.

    Cosmetic or Nonqualifying Indications

Indications outside the applicable criteria are considered cosmetic and are not medically necessary.

    Indications outside applicable criteria

    • Abdominal panniculectomy for indications other than those meeting the applicable InterQual or MassHealth criteria is cosmetic and not medically necessary.
    • Excision of excessive skin and subcutaneous tissue for indications other than the stated criteria, including but not limited to a suprapubic lift, is cosmetic and not medically necessary.

    Medicare Cosmetic-Surgery Rule

Under Medicare, cosmetic surgery is excluded from coverage, subject to the following functional or accidental-injury exceptions.

    Medicare cosmetic-surgery rule

    • The CMS Benefit Policy Manual, Chapter 16, §120 states that cosmetic surgery or expenses incurred in connection with such surgery are not covered. Cosmetic surgery includes any surgical procedure directed at improving appearance.
    • Exceptions

      • The procedure is required for the prompt repair of an accidental injury, meaning as soon as medically feasible.
      • The procedure is required for the improvement of the functioning of a malformed body member.

Lipectomy, which removes excess fat deposits using a liposuction cannula, may be performed as a component of panniculectomy. When performed alone, lipectomy or liposuction is considered cosmetic and not medically necessary.

Abdominal panniculectomy for indications outside the applicable InterQual or MassHealth criteria is considered cosmetic and therefore not medically necessary.

Excision of excessive skin and subcutaneous tissue for indications outside the stated criteria, including but not limited to suprapubic-related indications such as a suprapubic lift, is considered cosmetic and therefore not medically necessary.

For the Commercial line of business, excision of excessive skin and subcutaneous tissue that does not meet all required medical-necessity criteria is considered cosmetic and not medically necessary.

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for excision of excessive skin and subcutaneous tissue and abdominal panniculectomy procedures listed below. Submit the request with the required clinical documentation and supporting evidence.

    • CPT 15830 — Abdomen, infraumbilical panniculectomy
    • CPT 15832 — Thigh
    • CPT 15833 — Leg
    • CPT 15834 — Hip
    • CPT 15835 — Buttock
    • CPT 15836 — Arm
    • CPT 15837 — Forearm or hand
    • CPT 15838 — Submental fat pad
    • CPT 15839 — Other area
    • CPT 15847 — Abdomen/abdominoplasty; not covered

    Clinical Background

Panniculectomy removes an apron of hanging fat and skin and may be performed with or without abdominoplasty, also called a tummy tuck. Abdominoplasty tightens the abdominal wall muscles, removes excess skin and fat, and contours the waistline. Lipectomy or liposuction may be included as a component of panniculectomy; however, when performed alone, it is considered cosmetic and not medically necessary.

    Use InterQual for Abdominal Panniculectomy

For Commercial and Medicare members, direct abdominal panniculectomy requests to HNE's adopted InterQual procedure criteria: CP: Procedures, Panniculectomy, Abdominal. Members younger than 18 are reviewed case by case.

    • Registered HNE providers can obtain InterQual SmartSheets through the HNE provider portal; providers without portal access may call (413) 787-4004.
    Note

    Apply Medicaid Criteria

For Medicaid—BeHealthy members, apply the MassHealth Guidelines for Medical Necessity Determination for Excision of Excessive Skin and Subcutaneous Tissue when reviewing applicable requests.

    • MassHealth guideline: https://www.mass.gov/doc/guidelines-for-medical-necessity-determinationfor-excision-of-excessive-skin-and-subcutaneous-tissue-0/download
    Documentation Required

    Submit Clinical Evidence

The treating physician or primary care provider must submit clinical evidence to HNE showing that the patient meets the applicable treatment, testing, or surgical-procedure criteria. Without this documentation, HNE cannot properly review the prior-authorization request.

    Documentation Required

    Document Commercial Medical Necessity

For Commercial members, document that weight loss caused significant excess or redundant skin or skin folds and that the redundant tissue directly causes the qualifying medical-necessity indications.

    • Physical functional impairment interfering with activities of daily living, including physical exercise.
    • Persistent symptomatic intertriginous ulcerations or macerations refractory to good personal hygiene and physician-supervised local treatment for three months.
    • Recurrent skin infections—at least two episodes within 12 months—requiring systemic antibiotics and directly related to the redundant skin, with documentation that episodes were refractory to at least a full antibiotic course.
    Documentation Required

Detail Prior Skin and Infection Treatment

When documenting intertriginous ulcerations or macerations, provide a detailed description of all physician-supervised skin treatments and show that symptoms remained refractory despite treatment over three months.

    • Include treatment history in addition to documentation of good personal hygiene and the relevant skin findings.
    Denial Risk

    Outside Medical-Necessity Criteria

Abdominal panniculectomy requests for indications outside the adopted InterQual criteria or applicable MassHealth criteria are considered cosmetic and not medically necessary.

    Denial Risk

All Commercial Criteria Must Be Met

For Commercial excision requests, failure to meet any required criterion results in the service being considered cosmetic and not medically necessary.

    • All required elements must be documented: significant excess or redundant skin following weight loss; functional impairment; refractory intertriginous ulcerations or macerations; and qualifying recurrent infections when applicable.
    Denial Risk

    Medicare Cosmetic-Service Exclusion

Medicare does not cover cosmetic surgery or expenses incurred in connection with cosmetic surgery. Cosmetic surgery is excluded unless it is required for the prompt repair of an accidental injury or to improve the functioning of a malformed body member.

    • Apply the limited exceptions for accidental injury or functional impairment when evaluating Medicare requests.
    Billing Rule

    CPT 15847 Is Not Covered

CPT 15847 is listed as not covered. In addition, CPT/HCPCS code inclusion or exclusion is informational and does not guarantee reimbursement or payment; coverage is determined by the member's benefit plan, applicable law, and other policies or guidelines.

    • CPT 15847 — Excision of excessive skin and subcutaneous tissue, abdomen (e.g., abdominoplasty), including umbilical transposition and fascial plication; listed as not covered.
    • Code lists may not be all-inclusive, and inclusion of a code does not imply that the described service is covered.
