    POLICY UPDATE CHANGES

The policy added a medically necessary indication for rhBMP-2 (INFUSE) with qualifying one- or two-level transforaminal lumbar interbody fusion procedures.

The experimental, investigational, or unproven bone graft substitute and stabilization device lists were reviewed and updated.

The policy specifies that CPT 20930 is not payable for MassHealth or Medicare.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity and Coverage Criteria

    Cadaveric or Demineralized Allograft

Spinal bone grafting with a cadaveric allograft or demineralized bone allograft is medically necessary for spinal fusions.

Spinal fusion using a cadaveric allograft or demineralized bone allograft.

rhBMP-2 (INFUSE™) for Qualifying Lumbar Spinal Fusion

Spinal bone grafting with recombinant human bone morphogenetic protein 2 (rhBMP-2, INFUSE™) is medically necessary when ALL of the following criteria are met.

    ALL of the following

    • ONE of the following procedures

      • Anterior Lumbar Interbody Fusion (ALIF) or Oblique Lumbar Interbody Fusion (OLIF) for single-level degenerative disc disease from the second lumbar vertebra (L2) through the first sacral vertebra (S1), causing discogenic back pain, in conjunction with an FDA-approved interbody fusion device.

FDA-approved INFUSE devices include: INFUSE™ Bone Graft/Medtronic Interbody Fusion Device; Divergence-L Anterior/Oblique Lumbar Fusion System; Pivox™ Oblique Lateral Spinal System; Perimeter Interbody Fusion Device implanted via retroperitoneal ALIF from L2 to S1 or OLIF from L5 to S1; Clydesdale Spinal System implanted via OLIF at a single level from L2 to S5; INFUSE™ Bone Graft/LT-CAGE™ Lumbar Tapered Fusion Device; INFUSE™ Bone Graft/Inter Fix™ Threaded Fusion Device; and INFUSE™ Bone Graft/Inter Fix™ RP Threaded Fusion Device.

    • The member is not known or suspected to have malignancy and is not known or suspected to have an active infection at the operative site.
    • The member is 18 years old or older.>= 18 years

    Other or Off-Label rhBMP-2 Use

rhBMP-2 (INFUSE™) is considered experimental and investigational for all other spinal bone grafting indications or approaches, or when used for non-FDA-approved indications (off-label).

    • MassHealth and Medicare: Do not expect payment for CPT 20930.
    Prior Authorization

Prior Authorization for Bone Marrow Aspiration

Bone marrow aspiration used for spine-surgery bone grafting through a separate skin or fascial incision requires prior authorization.

    • Apply to CPT 20939 when aspiration is performed for bone grafting in spine surgery.
    • The aspiration code is reported separately from the primary procedure when applicable.

    Clinical Background

Spinal reconstruction and fusion are intended to achieve solid bone healing to reduce pain and improve function and quality of life. Bone grafting materials may be obtained from the individual, a cadaver or bone bank, or synthetic sources. Bone graft substitutes and related products are used to support or enhance bone formation, while rhBMP-2 is delivered with a carrier such as an absorbable collagen sponge to stimulate natural bone formation.
