    POLICY UPDATE CHANGES

The Lutathera criterion for tumor differentiation and Ki-67 index was revised to cover well-differentiated G1 or G2 neuroendocrine tumors with Ki-67 less than 20% or well-differentiated G3 tumors with Ki-67 less than 55%.

Definitions, including well-differentiated neuroendocrine tumors and Ki-67 index, were updated.

Medicare coverage is directed to LCD L37205 for Lutetium-177 dotatate and Lutetium-177 vipivotide tetraxetan.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • HCPCS A9513 — Lutetium Lu 177 dotatate, therapeutic, 1 mCi.
    Prior Authorization

    Prior Authorization Required: Pluvicto

Obtain prior authorization before billing Pluvicto (lutetium Lu 177 vipivotide tetraxetan).

    • HCPCS A9607 — Lutetium Lu 177 vipivotide tetraxetan, therapeutic, 1 millicurie.
    Step Therapy

    Prior Therapy Required Before Lutathera

Before requesting Lutathera, document disease progression despite prior somatostatin analog therapy or molecularly targeted therapy, such as everolimus.

    Therapeutic Context

Lutetium therapeutic radiopharmaceuticals are targeted systemic radiotherapies in which a radioactive isotope is linked to a molecule that targets specific receptors on tumor cells. Lutetium-177 dotatate (Lutathera) targets somatostatin receptors in selected neuroendocrine tumors, while lutetium-177 vipivotide tetraxetan (Pluvicto) targets PSMA-positive metastatic castration-resistant prostate cancer. The policy focuses on receptor-positive disease, progression after specified systemic therapies, and appropriate disease extent.

    • Include relevant clinical records, imaging reports, pathology or Ki-67 results, treatment history, and documentation of progression.
    Denial Risk

    Lutathera Denial Risk

Lutathera may be denied as experimental and investigational or not medically necessary when the required criteria are not met, when treatment exceeds the FDA-approved regimen, or when used for a non-FDA-approved indication.
