Coverage Summary
This policy defines medical necessity for implantable hypoglossal (upper airway) nerve stimulation systems (UAS) — specifically the Inspire® and Genio® devices — as a covered_with_criteria therapy for select patients with moderate to severe obstructive sleep apnea (OSA) who have failed or are intolerant of positive airway pressure (PAP) therapy. Device-specific eligibility requirements include documented polysomnography (PSG) within 24 months of first consultation, specified age and body mass index (BMI) thresholds (Inspire: BMI ≤ 40 kg/m2; Genio: BMI ≤ 32 kg/m2), and device-specific AHI ranges (Inspire adults: AHI ≥ 15 and ≤ 100; Genio: AHI ≥ 15 and ≤ 65; special pediatric/Down syndrome provisions apply in the Inspire pathway).

PSG timing documentation
Document that a diagnostic polysomnography (PSG) used for selection was performed within 24 months of the patient’s first consultation for hypoglossal nerve stimulator implantation.
AHI and BMI documentation
Document the patient’s apnea-hypopnea index (AHI) and body mass index (BMI) values and confirm they meet the device-specific thresholds for the requested system (Inspire: AHI and BMI per Inspire criteria; Genio: AHI and BMI per Genio criteria).
PAP therapy failure/intolerance documentation
Document attempts and reasons for PAP failure, refusal, or intolerance. Include objective PAP usage data (hours/night and nights/week) consistent with the policy definitions (e.g., >4 hours/night; >5 nights/week or at least five nights/week as applicable) or documentation of return/nonuse of PAP equipment.
Device-specific request specification
Specify the device requested (Inspire® or Genio®) in the prior authorization request and demonstrate that all device-specific eligibility criteria are met and that none of the device-specific contraindications apply.
Contraindication assessment
Document assessment demonstrating absence of listed contraindications, including proportion of central/mixed apneas, anatomical findings that would compromise UAS (e.g., complete concentric collapse of the soft palate), presence of incompatible implanted devices, MRI requirements beyond MR-conditional labeling, pregnancy or plans for pregnancy, rhabdomyolysis, and conditions compromising neurological control of the upper airway.

Background, evidence, and definitions
Two device options are addressed: the Inspire® Upper Airway Stimulation system (a unilateral implanted pulse generator with sensing lead and external programmers) and the Genio® System (a bilateral, battery-free implantable stimulator with an external wearable controller). Evidence from uncontrolled studies and registries demonstrates clinically meaningful reductions in AHI and improvements in symptoms and quality of life (for example, meta-analysis reports ~50–57% reductions in AHI; ADHERE registry median AHI decreased from 34 to 7 with median ESS reduction from 12 to 7 at 12 months). The DREAM trial for Genio reported a median 70.8% reduction in AHI, a 63.5% AHI responder rate, and 82% of participants achieving AHI < 15.
Limitations of the evidence include a relative lack of large randomized comparative trials and longer-term randomized data; ongoing device and procedural refinements mean additional research is needed to refine selection criteria, long-term outcomes, and comparative effectiveness.
