Coverage Criteria for Burn Debridement and Skin Substitutes

Debridement/Excision Necessity: Burn treatment with debridement and/or excision is medically necessary for either deep partial-thickness burns OR full-thickness burns (or deeper).severity

Skin Substitute Necessity

A: Sufficient autograft is not available at the time of excision or is not feasible due to the physiological condition of the member/enrollee.
B: No evidence of burn wound infection.
C: Burn is deep partial-thickness or full-thickness.
D: Treatment is with an acceptable skin replacement/substitute: (1) allograft (human cadaver); (2) xenograft (porcine); or (3) a tissue-engineered skin substitute used according to FDA indications (examples: Biobrane, TransCyte, Apligraf, TheraSkin, Integra Wound Matrix, Integra meshed Bilayer Wound Matrix, Integra Dermal Regeneration Template, or Epicel if used per FDA Humanitarian Device Exemption [HDE]).

Operational note: Use of skin replacements/substitutes when autograft is available or feasible, when burn is not deep partial-thickness or full-thickness, or when there is evidence of burn wound infection may not meet medical necessity and risk denial.

Skin replacements and tissue-engineered skin substitutes are not supported when there is evidence of burn wound infection. The policy requires absence of infection as a prerequisite for use of a skin substitute. In addition, autograft (skin taken from the patient) must be used when sufficient autograft is available and feasible; skin substitutes are indicated only when sufficient autograft is not available at the time of excision or is not feasible because of the member's physiological condition.

This clinical policy provides guidance on medical necessity but is not a benefit contract. Coverage determinations remain subject to the terms, conditions, exclusions and limitations of the member's coverage documents (for example, evidence of coverage, certificate of coverage, policy or contract of insurance) and to applicable federal and state requirements and Health Plan administrative policies.

Prior policy versions removed a blanket statement that investigational products were not medically necessary. The current policy instead specifies the acceptable skin replacement products (allograft, xenograft, and named tissue-engineered products) and clarifies that Epicel is acceptable only if used in accordance with the FDA Humanitarian Device Exemption (HDE) requirements.

Prior Authorization

Prior authorization / coding requirements

Prior authorization is required for skin substitute products and related procedures. When requesting authorization, include relevant CPT and HCPCS codes (e.g., debridement codes 11000, 11042–11047; skin substitute application 15271–15278) and specific product HCPCS (e.g., Q4100, Q4101, Q4104–Q4105, Q4108, Q4121, Q4182, C9363). Provide wound surface area, anatomic site, and clinical justification linking the request to medical necessity criteria.

Autograft — definition and standard of care

DefinitionSkin graft taken from another area of the same patient’s body (split- or full-thickness); standard of care for burn reconstruction.

When preferredAutograft is considered the preferred/standard treatment for coverage of burn wounds when donor sites and patient physiology allow.

LimitationsAutograft may be infeasible when total body surface area exceeds available donor sites or when tissues are too edematous for graft acceptance.
