Carriers are expected to provide coverage for all vaccines and the cost to administer vaccines, without cost-sharing and without the use of any utilization management, for persons who satisfy DPH recommended guidelines.
Carriers may limit vaccines available through a prescription drug formulary only if at least one product on the formulary addresses each DPH-recommended vaccine.
Carriers acting as administrators for non-insured employer-sponsored plans are expected to encourage plan sponsors to follow the Bulletin and DPH guidelines.
Coverage Expectations for Vaccines and Administration
Covered vaccines and administration
Covered when ALL of the following are met
When met, carriers must provide coverage for vaccines and the cost to administer them without cost-sharing or utilization management.
Applies to carriers regulated in Massachusetts; carriers must update Evidences of Coverage and online information to explain access to covered vaccine benefits.
Carrier products that maintain a prescription drug formulary may restrict coverage to vaccines listed on their formulary. Such limitations are permitted only if at least one product on the plan formulary addresses each item on the DPH recommended guidelines for vaccines. Plans that provide non-emergency coverage through a closed provider network may further limit access to vaccination services to in‑network practitioners or pharmacies.
Provider and Payer Operational Expectations
Do not require prior authorization when DPH guidelines are met
Carriers must not impose utilization management (for example, prior authorization) for vaccines or vaccine administration when a person satisfies the Massachusetts Department of Public Health (DPH) recommended guidelines for immunizations.
- Applies to persons covered under Massachusetts-issued insured health plans.
- No cost-sharing and no utilization management expected when DPH guidelines are met.
Ensure at least one formulary product per DPH-recommended vaccine
Carriers may use prescription drug formularies to limit which vaccine products are available, but each DPH-recommended vaccine item must be addressed by at least one product on the plan formulary.
- No additional step-therapy requirements are specified in the Bulletin.
- Ensure at least one formulary product corresponds to each vaccine item in the DPH recommended guidelines.
Clarify vaccine benefits and access in EOC and online materials
Update Evidence of Coverage (EOC) materials and online information to clearly explain vaccine benefits and how eligible persons may access covered vaccine benefits, in accordance with 211 CMR 52.13.
- EOC and online materials must state coverage for vaccines and administration without cost-sharing when DPH guidelines are met.
- Make clear how eligible persons may access covered benefits for vaccines.
Risk of limited coverage when formulary lacks a product for a DPH item
If a carrier’s prescription drug formulary does not include at least one product that addresses each item in the DPH recommended vaccine guidelines, coverage for that vaccine may be limited by the formulary.
- Formulary omission can result in limited coverage for specific DPH-recommended vaccines.
- Carriers must therefore ensure formulary products map to DPH guideline items to avoid limitation.
Closed networks may limit where members can receive covered vaccines
Plans that operate using a closed provider network may limit vaccination access to health care practitioners or pharmacies that are part of their in‑network providers; confirm network availability for patients.
- Closed-network plans may restrict where enrollees can obtain covered vaccines and administration.
- Verify in-network practitioner or pharmacy availability before scheduling vaccination for plan members.

The Massachusetts Department of Public Health (DPH) issues recommended vaccination guidelines intended to protect residents' health and to guide vaccine availability across the Commonwealth.
