OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Step Therapy

    Bisphosphonate Trial or Avoidance Required

Document either an inadequate response to an oral or injectable bisphosphonate trial of at least 1 year, an intolerable adverse event, or a clinical reason to avoid oral or injectable bisphosphonate therapy. Qualifying reasons to avoid therapy may include esophageal abnormalities, active upper gastrointestinal disease, gastrointestinal malabsorption, inability to remain upright or appropriately time oral dosing, creatinine clearance <35 mL/min, or prior intolerance to an oral bisphosphonate.

    • The qualifying trial, intolerance, or avoidance rationale must be supported in the member’s chart notes.
    • The requirement applies to the applicable postmenopausal osteoporosis pathway.

    Clinical Background

Evenity (romosozumab-aqqg) is indicated for the treatment of osteoporosis in postmenopausal women at high risk for fracture. This includes members with a history of osteoporotic fracture, multiple fracture risk factors, or failure or intolerance of other available osteoporosis therapy. Treatment is limited to 12 monthly doses; if additional osteoporosis therapy remains warranted, an anti-resorptive agent should be considered.
